[The effects of a dietary fiber (white lupine bran) in the treatment of non-insulin-dependent diabetes].

We evaluated the effect of dietary fiber from sweet lupine hull (SLH) upon metabolic control in 16 non insulin dependent diabetics (11 males, mean age 52). Body mass index was 23 +/- 2 and non fasting glucose levels were < 200 mg/dl. Following a 2 month control period, patients received SLH, 10 g th 3rd month and 20 g the fourth. Eleven patients received in addition oral hypoglycemic drugs. Intake of crude dietary fiber increased from 4.1 +/- 1.7 to 8.4 +/- 2 and 13.1 +/- 1 g from control to the 3rd and 4th month. Blood glucose increased and cholesterol level decreased in the 3rd month (p < 0.05). During the 4th month, blood glucose decreased to control levels. No changes were observed in body mass, HDL cholesterol, triglycerides, glycosylated Hb and non-fasting glucose and insulin levels in relation to fiber intake. We conclude that dietary fiber intake is low in our patients but a supplement of 10 to 20 g of this particular fiber did not improve metabolic control.

Hyperpigmentation of the clavicular zone: a variant of friction melanosis.

This paper reports on clinical, laboratory, and pathologic findings of 10 cases of an idiopathic form of hyperpigmentation of the clavicular zone, a peculiar entity of circumscribed, glistening, pigmented lesions affecting young Latin women. Pathologic findings consisted of focal to extensive necrosis of the epidermis, focal areas of junctional cleavage, and melanin deposition within the epidermis, dermis, and melanophages, which places this lesion within the group of brown hypermelanoses. The authors feel that friction with clothing or with scrub pads made of sedge (a very common practice amongst mexicans in the bath room) against clavicular protuberances is fundamental in its pathogenesis.

Comparison of 1 mg and 5 mg sodium cromoglycate metered dose inhalers in the treatment of asthma: a 12-week double-blind, parallel group trial.

A double-blind, parallel group trial of a 1 mg sodium cromoglycate metered dose inhaler, a 5 mg formulation and a placebo aerosol was undertaken in 139 asthmatic patients with extrinsic allergic asthma. None of the patients had previously been treated with sodium cromoglycate and few (15%) were familiar with the use of a pressurized aerosol device. Each test treatment was taken at a dose of 2 'puffs' 4-times per day for a period of 12 weeks. Response to treatment was assessed by analysis of symptom scores, medication usage and 3-times daily home measurements of PEFR recorded on diary cards, and by assessment of asthma severity and lung function in the clinic at the beginning and end of a 2-week baseline period and at intervals of 3 weeks throughout the trial. Analysis of patient-generated data (symptom scores and PEFR) demonstrated statistically significant differences in favour of each active treatment compared with placebo treatment. Clinic assessments of asthma severity and the investigator's opinion of treatment also showed the superiority of each sodium cromoglycate treatment regimen over placebo. These differences were statistically significant after 9 weeks (high dose) and 12 weeks (both doses). No statistically significant differences were demonstrated between the two active treatment groups in the clinic assessments of asthma severity, or in the home or clinic measurements of PEFR. There was, however, a consistent trend in favour of the higher dose formulation with respect to diary card symptom scores. In general, improvement in the low-dose sodium cromoglycate treatment group was slower than in the high-dose group suggesting an advantage for the 5 mg sodium cromoglycate inhaler in terms of onset of therapeutic response. The results indicate that sodium cromoglycate administered by pressurized aerosol at a dose of 2 x 1 mg or 2 x 5 mg 4-times daily is effective in the treatment of asthma and suggest that the higher dose formulation may provide more rapid control of symptoms.

[Prevalence of insulin dependent diabetes mellitus in school children of the metropolitan area].

The prevalence of insulin dependent diabetes mellitus was determined in 1,078,109 students of 2,150 schools between 5 and 19 years of age. 147 diabetic females and 107 diabetic males were found, for a prevalence of 25/100,000 in females and 20/100,000 in males (p less than 0.01). Prevalence of diabetes among 3 age groups, 5-9, 10-14 and 15-19 years, was 17, 22 and 19 per 100,000 in males and 22, 32 and 28 per 100,000 in females, respectively. The higher prevalence in females was significant at all age levels. Overall prevalence according to age was 19, 27 and 24 per 100,000 in the corresponding age groups (significantly higher in the 10-14 year old age group, p less than 0.01).

Epidemiologic aspects of asthma in Latin America.

The prevalence of asthma in Latin America is reviewed. There are many limitations to using official national statistics, and the data presented are from surveys of restricted populations in Uruguay, Peru, Mexico, Brazil, Venezuela, Argentina, and Chile. Asthma prevalence appears to be quite variable, ranging from 0.4 percent in Peru to 4.3 percent in Brazil. As reported elsewhere in the world, in children rates were higher in boys than girls, and elderly asthmatic patients have higher rates. Further information on the clinical and etiologic characteristics of asthma in Latin America are presented from a survey of chronic allergic disease conducted in seven Latin American countries. Pre-coded forms were completed during consultations at cooperating allergy departments, and all were analyzed centrally at the Pan American Health Organization. Eighty-six percent of the patients studied were atopic and 46 percent of asthmatic patients had a history of other associated allergic conditions (allergic rhinitis, atopic dermatitis). This is higher than reported for developed countries.

[Formulation, preparation and evaluation of shortening, laminated and cut biscuits, for diabetic patients].

Laminated and cut cookies formulated with natural and/or artificial sweeteners as substitutes of sucrose, are presented as a new alternative of choice for persons on a restricted diet. According to data in the literature, market availability and technological and economic limitations involved in the use of pure sweeteners, four mixtures were selected for the formulation of the cookies, instead of sucrose. Their composition and relative sweetness were as follows: (table; see text) After the statistical analysis of results, formulations presenting significantly superior quality characteristics were selected. As observed, all alternatives subjected to evaluation were grade 1. A study of acceptability by diabetic patients was carried out with these products through a ranking test, in order to determine which were the formulations preferred. This revealed a significant preference for the cookies containing saccharin-sorbitol = 0.35:99.65, at a 1% level of significance. Their nutritional and caloric values, as well as the chemical composition of the selected formulations were then determined. The results showed a 10.9% decrease in caloric contribution.
