Direct cytogenetic studies by needle aspiration of Burkitt's lymphoma in Ghana, West Africa.

With the use of a needle aspiration technique for obtaining tumor material, the direct karyotype of Burkitt's lymphoma cells was examined from 15 Ghanaian patients. The 14q+ chromosome (in addition to other chromosomal abnormalities) was present in 8 (57%) of the 14 patients; 6 of these 8 patients had t(8;14) (q24;q32). One of the remaining 2 patients had a #8 chromosome deleted, whereas the second had 2 normal-appearing #8 chromosomes. Other numerical and structural abnormalities were frequent. No correlation between type or frequency of abnormality and clinical status or survival was found. Within cells of the same tumor aspirate, the karyotype abnormalities were generally similar. In 3 patients, karyotypes from two separate tumor sites (abdomen-bone marrow, 2 cases; right maxilla-left mandible, 1 case) showed identical markers. The same marker was seen in specimens from the initial and second relapse abdominal tumors of 1 patient. The similarity of chromosomal markers from two sites or in serial studies further supports the concept that African Burkitt's lymphoma is a disease of monoclonal origin.

A computer-managed instruction system applied to dietetic education.

The emergence of competency-based programs in dietetic education has introduced the need for a systematic method of managing student performance information. A computer-managed instruction system which performs many of the routine clerical tasks usually assumed by instructors is being utilized in Emory University's Dietetic Internship Program. Preliminary use of the system indicates that it was well received by both instructors and students. Its most obvious benefits were the immediate feedback on performance and decreased demands on faculty time for evaluation and record-keeping processes.

Embryotoxicity in mice of phosphonacetyl-L-aspartic acid (PALA), a new antitumor agent. I. Embryolethal, teratogenic, and cytogenetic effects.

The embryolethal and teratogenic effects of phosphonacetyl-L-aspartic acid (PALA), a new antitumor agent, were evaluated in pregnant Swiss albino mice that received multiple IP injections of drug on days 7-11 of gestation. The effects of single doses of PALA given IP on days 4, 5, 6, 7, 8, 9, or 10 of gestation were also examined. Cytogenetic studies were performed on cells from embryos exposed in utero to embryotoxic doses of PALA, as well as on human peripheral leucocytes incubated with PALA for six or 48 hours. When given on days 7-11 of gestation, marked embryolethal effects were noted at PALA doses of 1.5 mg/kg/day, and no embryos survived doses of 6.25 mg/kg/day. Studies on the effects of PALA administered as a single dose on various days of gestation revealed that its embryolethal effects were gestation stage-specific; 80% of embryos were killed in utero after exposure to a dose of 12.5 mg/kg on day 8, whereas doses nearly 20-fold higher (200 mg/kg) were required to exert equivalent embryolethal effects on day 6 or 10 of gestation. PALA treatment induced an increase primarily in the incidence of malformations which occur spontaneously in control mice rather than in malformations not normally seen in controls. PALA embryotoxicity in mice apparently does not derive from major cytogenetic damage, as the incidence of chromosomal aberrations was not significantly increased by in vitro or in vivo exposure to PALA.

Proctocolectomy for ulcerative colitis.

The majority of patients with ulcerative colitis can be managed satisfactorily throughout their lives by medical treatment, but a minority require colectomy. In severe attacks, emergency surgery is often required to save the patient's life. In chronic disease not responding well to medical treatment, elective colectomy will restore the patient to good health. Colectomy is also necessary for certain local complications, such as cancer, severe fistulas, and strictures. Prophylactic colectomy is indicated in patients with long-standing disease in whom there are factors associated with a high risk of developing cancer of the large bowel. For all these indications, it is our opinion that single-stage proctocolectomy is the operation of choice in the great majority of patients. Finally, we consider that the best results are obtained when the physician and the surgeon work together in the closest possible cooperation.

Serial cytogenetic studies of nonendemic Burkitt's lymphoma cell lines.

Cytogenetic study of nonendemic Burkitt's lymphoma (BL) cell lines showed a translocation of chromosomes #8 and #14, t(8;14), present in 16 of the 18 lines examined. Serial cytogenetic studies of nine of these lines showed the t(8;14) to be stable and present in all cells examined. Although many other chromosome aberrations were present, they did not demonstrate the stability or the pervasiveness of the t(8;14). The significance of these results and previously reported cytogenetic studies on the etiology of BL was discussed.

Cytogenetic studies in non-African Burkitt lymphoma.

A particular translocation between chromosomes 8 and 14 has been found repeatedly in cytogenetic studies of Burkitt lymphoma, both of African and non-African origin. We report here our findings in cytogenetic studies of direct tumor preparations from 18 non-African Burkitt lymphoma patients, 9 of whom also had cell lines available for study. A t(8;14) was found in direct tumor material in 10 of the 18 patients. Seven of the 9 cell lines had a t(8;14). A total of 15 patients had either a t(8;14) or a 14q+ present in tumor material and/or cell lines. In addition, 8 patients had a peculiar marker chromosome 1. The t(8;14) was not found in every malignant cell and, where present, it was rarely the sole karyotypic abnormality. The relationship of the t(8;14) to the evolution of the tumor is discussed.

Localisation of insulinomas.

Small insulinomas may be undetectable by arteriography or palpation of the pancreas. They can be identified, however, by the point at which high insulin levels are detected in the venous effluent sampled at several sites by catheterisation of the splenic and portal veins at laparotomy or via the percutaneous transhepatic route. Diagnosis by catheterisation techniques made it possible to resect only that part of the pancreas containing the tumour (in one patient the head and in another the body of the pancreas). Improved localisation is essential in planning an operation, and often removes the need for lengthy trials of medical therapy before laparotomy.

Factors which influenced postoperative complications in patients with ulcerative colitis or Crohn's disease of the colon on corticosteroids.

A retrospective analysis was undertaken of the records of 107 patients with Crohn's disease of the colon or with ulcerative colitis who underwent 162 operations under steroid cover. The study revealed no correlation between steroid dosage and postoperative morbidity or mortality. The incidence of wound dehiscence and incisional hernia compared favourably with the reports of other unselected series of similar patients. Contamination did significantly influence results. Septic complications were more frequent when the operative field was contaminated and both delayed wound healing and mortality were related to this sepsis. A ;clean and dirty' technique was effective in controlling contamination during elective bowel division but preoperative bowel perforation and accidental entry into the lumen of the bowel during dissection were potentially avoidable sources of contamination. Primary healing of the perineal wound after proctocolectomy was seldom achieved in contaminated patients where a drain tube was brought out through the main perineal incision. When perineal sinuses or fistulae followed a proctocolectomy, patients with Crohn's disease had a significantly slower rate of healing than did patients with ulcerative colitis. However, there was no difference in the healing of abdominal wounds in relation to the primary pathology. Even abdominal incisions which were used on more than one occasion healed as well as those which were used for the first time. A prophylactic antibiotic regime of either ampicillin or tetracycline offered little protection against postoperative sepsis. The organisms which caused such infections were often insensitive to the two antibiotics.

Mutant lines of guinea pig L2C leukemia. II. Comparative cytogenetic studies and banding analyses of normal and leukemic karyotypes.

Chromosomes of normal guinea pig (strain 2) cells and of cell lines dervied from a spontaneously arising leukemia were analyzed in detail. All cell lines studied, LG-L2C, GH-L2-C, BZ-LC, and EN-L2C, contained one M1 marker and two X chromosomes, in addition to other chromosome abnormalities specific for each cell line. The presence of the M1 marker and the two X chromosomes confirmed that all of these leukemic cell lines are derived from one ancestral line. Comparison of the chromosome markers and immunologic characteristics of these lines revealed that possibly the gene involved in the determination of C3 receptor sites is located on the terminal portion of the long arm of chromosome No. 2, but no other correlations could be made.
