[Transdermal salicylate absorption and behavior of corticosteroid plasma levels during epidermal administration of an antiphlogistic ointment].

After repeated epidermal application of an ointment containing salicylic acid, mucopolysaccharidepolysulfate and suprarenal extract (Mobilat) the transdermal absorption of salicylate was determined in seven healthy volunteers by measuring the renal excretion of salicylate as well as the plasma level. The determination of the corticosteroid plasma level was to elucidate a possible effect caused by the suprarenal extract contained in Mobilat ointment. By means of high pressure liquid chromatography the concentrations of salicylic acid and salicyluric acid in the collected urine and in the plasma were established. The quantitative determination of hydrocortisone, 11-desoxycortisol and 17 alpha-hydroxyprogesterone, selected as indicator steroids, was performed by radioimmuno assay. After repeated application of Mobilat ointment a constant level of salicylic acid in plasma of approximately 0.2 microgram/ml was observed. The total excretion of salicylate reaches a constant level of approximately 12 mg/day. About 6.9% of the amount of salicylate was renally excreted after 7 days. The corticosteroid plasma level showed no significant change.

[Current status of the treatment of kidney injuries].

Injuries of the urinary tract are of increasing interest. Infusion urography, reno-vasography, szintigraphy give a better chance now to diagnose and classify these mostly blunt traumata. In the last 15 years 278 patients with renal injuries were seen at the Urological Dept. of the Friedrichshain-Hospital Berlin. The authors emphasize organ preserving procedures.

The exstrophy support team: a new concept in the care of the exstrophy patient.

The concept of the exstrophy support team is reported. It has extended the care of children with exstrophy into a lifelong continuum of effective assistence, not just by way of medical and surgical interaction but through knowledgeable and sympathetic coordination. There has been a spectacular improvement in over-all lifetime survival and much greater patient interest and appreciation with better followup and maintenance so that we can help quickly and effectively when problems arise. Now that we have achieved an increased longevity for these patients, we must be able to assure them the best possible quality of life.

Group psychotherapy on a children's urology service.

This paper describes an ongoing program, in a large metropolitan hospital, of weekly group meetings for hospitalized children on the Babies Hospital Urology Service, led by a pediatric psychiatrist and a psychiatric social worker. The purpose of the program is to help children cope with the multitude of fears and anxieties that can arise in the course of hospitalization. Although not described in detail, corollary weekly group meetings are also held with parents of the children. The authors describe the format of their meetings with child patient groups, whose composition is, by its nature, a constantly changing one. Examples are excerpted from typical group sessions, indicating the kinds of problems that surfaced and how they were dealt with. Ways in which group interaction and support contributed to the positive results of such meetings are also shown. The authors suggest that their program can be utilized in a variety of hospital settings.

[Experiences with new methods for the preoperative localization and surgical removal of complicated kidney calculi].

In a survey is described the inpatients suffering from urolithiasis of the urological clinic Berlin-Friedrichshain of the last three years from 1 April 1975 to 31 March 1978. Especially the cast calculi of the renal calyx were analysed and the therapeutic approach was discussed. For the complete removal of coral calculi in our clinic preoperatively made tomographies and adequate graphs of the cast concrement in its relation to the hollow system at two levels also stood the test as the intraoperative use of the X-ray device "Renodor". The two methods were described in detail. Also in multiple and movable concrements in the system of the renal calyx the Renodor stood the test. Here we could make good experiences also with the fibrin pyelolithothomy--the indications and results of these methods are represented.

[The effect of nitrates on the function of intact and ischemic myocardium (author's transl)].

The effects of Sodium Nitroprusside (NPN), of Isosorbidedinitrate (ISDN), and of a combined therapy with NPN and the intraaortic balloon pump (IABP) on left ventricular function and on local contraction patterns of normal and ischemic myocardial regions were studied. In acute experiments on 16 dogs the anterior descending branch of the left coronary artery was stenosed to produce a hypokinesia or dyskinesia of the anterior wall of the left ventricle. Regional myocardial function was assessed using the ultrasound transit time method. When the LV filling pressure was initially norman (LVedP 5.7 +/- 0.6mmHg) nitrate infusion (120 microgram/minn NPN: 800 microgram/min ISDN) resulted in an aggravation of the ischemic dysfunction. Conversely, with high initial filling pressure (LVedP 15.4 +/- 1.3MMHg) regional myocardial function in the ischemic area returned to normal under therapy. Additional application of the IABP further improved regional myocardial mechanics. It is concluded, that reduction of coronary perfusion pressure due to nitrate therapy becomes critical when coronary reserve is depleted. Compensation is however possible if ventricular wall stress can be reduced and thus the oxygen consumption and the extravascular component of coronary resistance are decreased. Only when the initial filling pressure is high these conditions are met.
