[Iatrogenic sinus node disease and iatrogenic AV block].

The appearance of long lasting bradycardia due to damage of the sinus node (iatrogen sick sinus syndrome; or the atrioventricular conduction system, iatrogen AV block) is not a rare event after open heart surgery. In the course of 5093 heart operations the development of iatrogen sick sinus syndrome was observed in 234 patients (4.6%) and iatrogen AV block in 91 cases (1.8%). The incidence rate of iatrogen sick sinus syndrome or iatrogen AV block was quite divergent depending mostly on the type of operation. Comparing the data of our earlier (1977-1982) and later (1983-1991) cardiac surgical interventions, the prevalence of iatrogen sinus node disease and iatrogen AV block seems to decrease, mainly due to the progress in techniques of cardiac surgery. With the appearance of iatrogen sick sinus syndrome or AV block, urgent temporary pacing is indicated to prevent the deleterious hemodynamic effect of bradycardia. In the cases of persistent iatrogen sick sinus syndrome and iatrogen AV block, permanent pacemaker implantation is needed. It is remarkable that although the incidence rate of iatrogen sick sinus syndrome is greater than that of iatrogen AV block, in iatrogen sick sinus syndrome the regression is quite frequent, while in iatrogen AV block the 2nd or 3rd degree AV block is usually permanent. We performed pacemaker implantation in 56 cases of iatrogen sick sinus syndrome (24%) and in 57 patients of iatrogen AV block (63%). With pacemaker therapy the outcome of iatrogen sick sinus syndrome and iatrogen AV block is very favourable.

[Importance of carotid artery studies prior to coronary surgery].

The incidence of cerebral complications following extracorporal cardiac operations is reported to be approximately 2%. One of the possible reasons behind these complications is the presence of significant carotid stenosis as coexistent disease to the cardiac illness requiring surgery. Because of the common etiology carotid stenosis coexists mainly with a coronary artery disease. The authors make known their own screening methods based on the correct exploration of medical history and on the proper physical examinations. Coexistent significant carotid stenosis was revealed in 3.6% of 1056 patients who underwent coronary surgery within the period of 41 months. In all of these cases prophylactic carotid endarterectomy was performed. In twenty cases within this group the carotid reconstructions were performed simultaneously with myocardium revascularization. Staged endprocedures were performed in the other 18 cases. There was no surgical mortality and only one patient suffered major stroke. The authors emphasize the importance of carotid screening among patients awaiting coronary surgery especially in patients who previously sustained cerebral ischemic attacks, the presence of carotid bruits or any other known localization of obliterate arterial disease and finally in all patients over 60 years of age.

[Coronary surgery at the Cardiovascular Surgical Clinic (1976-1990)].

In the Cardiovascular Surgical Clinic of the Semmelweis Medical University of Budapest the first coronary artery bypass grafting (CABG) procedure was performed in 1975. Since that time coronary artery surgery has become a routine everyday practice representing more than half of the total workload of adult cardiac surgery. The analysis of 1347 operations performed between 1976-1990 on patients with coronary heart disease showed the followings: the first few years--so called learning curve of CABG operations is characterised by high mortality. With passing time the number of cases performed each year increased rapidly and the surgical technique has improved too. At the same time the operative mortality figures showed decreasing tendency--it was 2.1% for the last 609 cases. All observed parameters showed some progress: in 1990 the average number of grafts per patient was 3.09, internal mammary artery usage 15 percent and the mean aortic cross clamp time per anastomosis 24.5 minutes. Complete myocardial revascularisation is the key point of coronary artery surgery. In order to achieve this target in all operated cases further technical improvement is necessary.

[Development and results of valve replacement surgery at the Városmajor Clinic 1976-1990 (retorospective clinical analysis)].

In Hungary valve replacement is still a major indication for heart surgery in adults. In the Cardiovascular Surgical Clinic of Semmelweis Medical University of Budapest from 1976 to 1990 2435 patients were operated for valve disease. Majority of the cases had single (aortic n = 856, mitral n = 912) or double (aortic + mitral n = 513) valve replacement. Over this 15 years period there have been many alterations in patients characteristics and surgical technique as well. In spite of the increasing mean age of patients the operative mortality has decreased (in the last 5 years period it was 2.7%, 5.5% and 7.9% in the three groups respectively). At the same time the number of patients requiring valve re-replacement or combined valve + coronary procedure has increased. The use of bioprosthetic valves has fallen below 10 percent from the 60--80 percent observed between 1976--1980. The analysis showes excellent surgical results in the field of valve replacement in Hungary.

[Experience with the surgical management of constrictive pericarditis].

61 patients were operated on for constrictive pericarditis at the Cardiovascular Surgical Clinic of Semmelweis Medical University in the last 33 years. The average hospital mortality of the surgical pericardiectomy was 4.9%. The final conclusions of this retrospective and follow up study are as follows: pericardiectomy is the method of choice in the treatment of constrictive pericarditis, since it does not has any therapeutic alternative, its hospital mortality is low and it results excellent early and late postoperative functional effects.

Congenital complete heart block associated with QT prolongation.

The coexistence of congenital complete heart block and QT prolongation represents a special type of arrhythmia. The electrophysiological and clinical characteristics of this syndrome were studied in eight patients suffering from congenital AV block and QT prolongation. Data from 22 patients suffering from congenital complete heart block only, served as a control. In the study group, the appearance of a torsade de pointes type of ventricular tachycardia could regularly be observed and the tachycardial attack could usually be provoked by ventricular extrastimuli. The corrected QT time was markedly prolonged; on ventricular stimulation, at higher pacing rates the QT interval shortened, but remained significantly higher than in the control group. Syncopal attacks--with the character of polymorphic tachycardia--appeared in each patient of the study group while occurring in only three patients from the control group. Patients were given pacemaker implants (using a higher pacing rate) and long-term administration of beta-receptor blockers. The outcome was favourable; no ventricular tachycardia or syncopal attack was observed in the follow-up period.

Biological valve prosthesis replacement--experiences and considerations.

Between 1976 and 1991. 1.017 tissue valve prostheses were implanted in 801 patients. During the same period 1876 mechanical valve prostheses were replaced. The hospital mortality was 8.1%. Till 1991 230 bioprostheses (22.6%) in the case of 166 patients (20.7%) had to be removed and replaced by other valve prostheses. There are no significant differences concerning the mortality between the first (8.1%) and the second operations (9.1%), and the durability of the various types of bioprostheses used, however, calcification, degeneration and other complications occurred more frequently and earlier in the case of mitral (24.5%) than in aortic (18.9%) bioprostheses, and in younger patients than in the older ones, as well. The mean age of patients was 46 years at the time of the first and 49 years at the time of the second operation. The incidence of reoperations was the highest in the seventh year after the first surgical intervention. In general, one size smaller prostheses were used in valve replacements after the removal of the first bioprostheses.

[Acute dissection of the ascending aorta diagnosed by two-dimensional echocardiography].

The authors present a case of acute dissection of ascending aorta diagnosed by two-dimensional echocardiography. Echocardiographic findings were subsequently confirmed by angiography. The patient was successfully operated and has been asymptomatic since the operation. The authors point out the importance the echocardiographic examination in case of any clinical suspicion. The authors have not found any previous report on the echocardiographic diagnosis of aortic dissection in the Hungarian literature.

The acute effect of ajmaline on ventricular arrhythmia after cardiac surgery.

The acute effect of ajmaline was investigated in the treatment of postoperative ventricular arrhythmias. Ajmaline (Glurytmal--Giulini Pharma GMBH) was applied intravenously in 15 patients suffering from ventricular premature beats (Lown II--IV/b), tachycardia and/or ventricular fibrillation after open heart surgery. Ajmaline infusion produced in 40% of the cases a total suppression and in 100% a significant improvement of malignant ventricular arrhythmia. In patients with recurrent and resistant ventricular tachycardia, ajmaline proved effective even when other antiarrhythmic drugs had failed. It was shown to be effective in reducing ventricular premature contractions and recurrent ventricular tachycardia after heart surgery, without haemodynamic side effects. Because of recurrent ventricular premature beats in 11 patients after acute ajmaline administration further oral application of prajmalium bitartarate (Neo-Gilurytmal) was necessary. The maintenance of these patients on oral ajmaline treatment seemed important.

The electrophysiological peculiarities of childhood tachyarrhythmias.

Electrophysiological analysis of 20 patients in child or adolescent age affected by paroxysmal tachycardia has been performed. In 16 cases the tachycardia was supraventricular and in 4 cases it was ventricular. Evaluation of the electrophysiological parameters revealed certain characteristics of childhood tachyarrhythmias, viz. (1) adequate sinus node function, absence of sinus dysfunction, (2) absence of brachycardia, (3) acceleration of anterograde and retrograde atrioventricular impulse conduction systems.

Adams-Stokes attacks associated with carotid sinus syncope. Pathogenesis and therapy of the carotid sinus syncopes.

The pathophysiology and therapy of the carotid sinus syncope were studied by means of electrophysiological investigations and clinical observations in 84 patients suffering from carotid sinus syncope. The most important data and conclusions of the study are the following: 1. The most frequent cause of the carotid sinus syncope (Adams-Stokes) attack is the sinus arrest and a sinus block. 2 degrees-3 degrees av block can be observed less frequent in the course of syncope attack. 2. In a patient suffering from carotid sinus syncopes, disorders of impulse formation and impulse conduction of the heart are frequent. First of all, the association of the sick sinus syndrome and the carotid sinus syncope can be often detected. 3. It is not rare at all that the carotid sinus syncope occurs with extracranial obliterative disorders. Transitory cerebral ischaemic attack frequently develops on the stimulation of the carotid sinus area. 4. The drug -- atropine -- therapy of the carotid sinus syncope is usually insufficient. Generally, pacemaker implantation is the adequate treatment. Late results are very good after pacemaker implantation.

Changes in the pacemaker function of the sinus node after closure of atrial septal defect. Temporary and stable iatrogenic sick sinus syndrome.

The alterations in the clinical symptoms and the pacemaker function of the sinus node following open heart operation were studied in 26 patients suffering from atrial septal defect. The most characteristic electrophysiological change was the prolongation of the sinus node recovery time as consequence of surgical repair of the atrial septal defect. The clinical symptoms of the sick sinus syndrome could be detected in all 26 cases after the open heart surgery. In 16 patients the symptoms of iatrogenic sick sinus syndrome (SSS) were only transitory, while in 10 patients a stable iatrogenic SSS developed after the operation. In the basis of these data the pathological mechanism of the iatrogenic SSS is discussed.
