Chronic idiopathic hyperphosphatasia and fibrous dysplasia in the same child.

A generalized skeletal dysplasia with features of chronic idiopathic hyperphosphatasia and fibrous dysplasia of the mandible were observed in a 6-year-old child. The abnormal development of the bones resulted from enhanced remodeling and the failure of mature bone to form. The occurrence of the two lesions in the same child and a review of the literature support the hypothesis that pathogenesis of fibrous dysplasia and idiopathic hyperphosphatasia reflect an underlying common defect in the control of bone cell activity.

Fanconi's anemia: ultrastructural observations on erythroblasts.

A case of Fanconi's anemia is reported. Particular attention was given to the ultrastructure of the erythroblasts. The main abnormalities are summarized as follows: (1) striking polymorphism of all the erythroblasts, (2) multiple nuclear cisternae showing several breaks, (3) admixture of nucleus and cytoplasm, (4) myelinic figures inside nuclear cisternae and cytoplasm, (5) moth-eaten nucleus, and (6) total absence of normal erythroblasts. These abnormalities were still visible after therapy in spite of clinical and hematologic improvement.

Two incidental cases of abdominal aortic aneurysm and gallbladder cancer. Further data influencing the management of patients affected by aneurysm and gallbladder disease.

Abdominal aortic aneurysm and cholelithiasis are two common diseases in the elderly population. The prevalence of abdominal aortic aneurysms ranges between 1.8 and 6.6% in autoptic series and it's estimated that 2.5% of the over sixty year old population is affected. Carcinoma of the gallbladder is the most common malignant tumor of the biliary tract and in the United States is the fifth most frequent digestive tract malignancy; it's incidence ranges between 2 to 10 cases of 100,000 persons/year. No adequate guidelines are now available to assist the surgeon, in the case of concomitant gallbladder disease and abdominal aortic aneurysm. In this paper the management of abdominal aortic aneurysm in a patient with gallbladder disease is discussed in order to assist the surgeon deciding whether to perform concomitant aneurysm resection and cholecystectomy. In 162 aneurysmectomies (1987-1997) 18 (11.11%) patients underwent combined aneurysmectomy and cholecystectomy operation. The patients ranged in age from 49 to 88 years (average 69 years). In two cases the anatomo-histological specimen examinations (twelve sections) demonstrated a gallbladder carcinoma. The overall mortality rate was 5.56% either for aneurysmectomy alone or for combined therapy. In case of abdominal aortic aneurysm and concomitant gallbladder disease, in choosing simple endoaneurysmectomy, the surgeon has to consider the risk of early and late complications of leaving a diseased gallbladder in place. In case of concomitant performance of both operations, the risks of a possible septic graft contamination must be considered. We believe that the patient may be best served by performing the vascular and nonvascular procedures in the same operation. In this paper a new proof, till now never considered in the international literature, is presented to support our opinion: the possibility of concomitant unknown cancer or precancerous lesions in a lithiasic gallbladder. Diagnosis of these lesions is, indeed, not easy to perform in the preoperative phase and is often a postoperative anatomo-histological detection.

[Results of lumbar gangliectomy interventions. Report of cases and current indications].

The role of lumbar gangliectomy is examined by analysing cases of vascular interventions on the aorto-ilaco-femoral sector at the Department of Surgical Clinical Medicine II and at the Department of Surgical Anatomy and Surgery Studies of the University of Milan during the period 1966-1973. The steady, marked decrease in the incidence of operations on the Vegetative Nervous System, parallely with a notable rise in reconstructive operations is noted. The extension of indications to reconstructive operations alters the attitude to be assumed with regard to lumbar gangliectomy. Analysis of long-term results of indirect operations, however, confirms the validity of the method as a function of the restrictive attitude of present-day indications.

Age-related changes in interleukin 2 responsiveness of resting and activated human mononuclear cells.

BACKGROUND: Although the "T cell model" on the immunological decline associated with aging is gaining increasing support, the relationship between response to IL2 and aging has not yet been investigated. METHODS: Human peripheral blood mononuclear cells (PBMC) from healthy elderly donors were cultured with increasing concentrations (1-1000 U/ml) of recombinant interleukin 2 (rIL2). RESULTS: PBMC from older donors proliferated as much as those from younger donors until the fifth day of culture, but showed a reduced capability to proliferate in the following days (8th and 10th). Culturing PBMC with PHA we obtained the opposite result. In fact, the known depressed proliferative response of elderly people to the mitogen PHA is detectable only in the early days of culture. Furthermore, the response to rIL2 of 1% or 0.125% PHA-stimulated PBMC showed an important impairment of the proliferative rate in the elderly population with respect to younger controls, although no differences in the number of IL2 receptor-positive cells were detected. This phenomenon was demonstrable mainly in the first several days of culture. CONCLUSIONS: These data suggest a delay in the activation rather than a real functional impairment of PBMC from old donors.

Anticardiolipin syndrome in childhood: a report of two cases.

We describe two children with an otherwise unexplained deep vein thrombosis associated with high titer anticardiolipin antibodies (ACA) of the IgG class and circulating lupus anticoagulant (LAC). One of these patients had persistent proteinuria but neither had systemic lupus erythematosus. Our observation suggests that ACA and LAC assays should be performed in children with thromboembolic disease even when no underlying autoimmune disease can be found.

Interleukin 2 and mitogen driven proliferation in human peripheral mononuclear cells: a kinetic analysis.

The role of the T lymphocyte growth factor, interleukin 2 (IL2), on human lymphocyte proliferation was investigated. The results define the extent and the chronological sequence of IL2 effects on unstimulated mononuclear cells and on an activated population enriched in IL2 receptor positive cells. Important differences were observed in the proliferative response to the mitogen PHA when the IL2 concentration in the culture medium was varied. The data suggest that IL2 concentration is critical in the responsiveness to a mitogenic stimulus.

A Multimeric Synthetic Peptide Combinatorial Library.

We describe here a novel type of synthetic peptide library, named Multimeric Synthetic Peptide Combinatorial Library (M-SPCL), where multiple small peptide ligands are tied together in the same molecule. The advantage of using small peptides in the form of M-SPCL is two-fold: first, the high density assembly of the sequences on the branching scaffold leads to signal amplification, thereby effectively lowering the binding threshold for the selection of ligands; second, to interfere with protein-protein interactions, multimericity has been shown to be a desirable feature per se. The M-SPCL is prepared by solid-phase peptide synthesis, based on the structure of Multiple Antigen Peptides. When prepared in Positional Scanning format [C. Pinilla, J. Appel, P. Blanc and R.A. Houghten. 1992. BioTechniques 13: 901-905], selection is based on the amplified interaction of a single residue in a sequence-defined position. The usefulness of the new library was demonstrated by the selection of octameric peptides, which inhibit the binding of the cytokine human interleukin-6 to its receptor, with an apparent nanomolar affinity. Tetrameric, but not dimeric, branched peptides with the same sequences were also active with comparable affinity. The success of this approach is noteworthy, since screening of the corresponding monomeric pentapeptide SPCL did not lead to the selection of any inhibitory compound in the same system.

[The incidence of Raynaud's phenomenon in 1920 residents in Milan].

We use the term Raynaud Phenomenon (RP) to define episodic digital ischemia with colour changes involving the digits of the hands and feet, induced by cold or emotional stresses. Surprisingly little information is available concerning the incidence of RP in the general population. In Italy, studies of this purpose have never been carried out and the few investigations, we find in literature, are discording, showing an incidence between 0.83% and 27%. The interest in knowing the incidence of RP is, beside the usual reason which get to an epidemiological study, RP indeed may be associated with severe and disabling clinical conditions as connective tissue diseases. The RP may be the initial complaint on an otherwise silent systemic illness and may even influence the course of these clinical conditions, assuming the role of prognostic index. It seemed useful for us to investigate on a model of population resident in our city the incidence of RP and the frequency of the idiopathic and secondaries forms. We have prepared a set of questions which was distributed to some students attending the IV year of the School of Medicine and to specialized doctors. The students randomly interviewed as many people as they could, while the doctors had to fill the questionnaire for companions of patients coming to three different Sanitary Units in Milan. All those subjects that at first instance were diagnosed as RP were sent to our ambulatory where more experienced doctors provided to collect a precise history.(ABSTRACT TRUNCATED AT 250 WORDS)

[Critical evaluation of the reconstruction of the intestinal continuity after total gastrectomy by the Moricca technique].

The authors analyze a series of 48 total gastrectomies for gastric carcinomas performed between 1987 and 1994. Seven out of these 48 gastrectomies were performed adopting the double circuit esophagojejunoduodenal plasty according the procedure described in 1976 by Francesco Moricca. The double jejunal loop represents a sort of "digestive reservior" allowing to avoid the sensation of epigastric fullness after eating. Moreover, this technique permits a better absorption of the liquids and the partial mixing of the bolus with the bilio-pancreatic secretion stimulates the duodenal secretin and cholecystochinin release. The possibility of a "dumping syndrome" is diminished by the presence of the double possibility of diversion of the alimentary bolus. On the contrary the segment of jejunum transposed between the esophagus and duodenum is antiperistaltic and this fact can cause sometimes an esophagitis at the lower third of the esophagus due to the prolonged alkaline reflux. This study is aimed at verifying the entity and frequency of the esophagitic complication in patients who underwent a total gastrectomy and at analyzing the procedures adopted to cure this severe complication that seriously affects the wellness and the nutritional status of the gastrectomized patients.
