Sulindac (Clinoril; Frosst-MSD), a non-steroidal anti-inflammatory drug was compared with aspirin in a randomized double-blind cross-over controlled study in 30 patients with juvenile chronic arthritis. Sulindac was found to be safe and effective. Although it has the advantage of a twice-a-day dose regimen, both patient and doctor may prefer to be guided by pain relief and cost.
Ketoprofen (Orudis; Maybaker), whose use has not previously been reported in choldren, was compared with indomethacin in a double-blind crossover trial in 30 children with juvenile chronic arthritis. Both drugs proved to be safe and effective analgesics and anti-inflammatory agents although indomethacin emerged as the preferred drug. Perhaps higher doses of ketoprofen would be safe and more effective. Side-effects were few and mild. The problems of patient compliance and the assessment of pain in children are discussed.
A 6-year-old boy presented with gynaecomastia. There was no clinical or biochemical evidence of excessive androgenic or glucorticoid activity, but urinary oestrogen levels were raised. An adrenocortical adenoma, demonstrated by x-ray, was surgically removed. Oestrogen levels fell immediately. 3 years later the boy shows complete regression of the gynaecomastia and no signs of recurrence.

Twelve sets of conjoined twins have been born in Southern Africa since February 1974. This possibly represents a significant increase in the incidence of Siamese twinning and may be the result of unknown environmental agents. Several of the sets of twins were diagnosed antenatally. In view of the current circumstances a high index of suspicion is probably justified in the presence of maternal hydramnios or fetal malpresentation.

Eleven sets of conjoined twins have been born in Southern Africa in a period of just over twelve months. Analysis of the circumstance pertaining to the conception and delivery of these children revealed considerable ethnic, geographic, and socioeconomic differences. It is possible that a ubiquitous environmental agent is a precipitating factor in this situation.
This study represents an attempt to collect observations from co-operating countries to evaluate the previously suggested criteria for juvenile chronic arthritis during the onset periods 0-3 and 3-6 months, preferably studied prospectively. Only 267 of 378 forms returned were satisfactory for inclusion because of failure to observe exclusion criteria or insufficient information. Despite this it was possible to conclude that systemic disease represented 25% of the group and that these features tended to decline during the 6 month period. Gut inflammation was seen in 4.5% and the frequency of chronic iridocyclitis increased during the observation period to 4.5%. Variation in joint state during this period suggests that course classification may be important, as 23 of 118 who were pauciarticular at 3 months became polyarticular by six months, but only 46 of 76 who were polyarticular at 3 months remained in this state at six months.
