[Immediate hypersensitivity in brucellosis. New data (author's transl)].

The presence of clinical manifestations of the "immediate hypersensitivity" process has been frequently described in veterinarians, bacteriological laboratory technicians and some patients with chronic brucellosis. We have looked for the presence of specific IgE in these conditions. Twenty-four patients, three months at least after the acute phase of their brucellosis, were given exploration of their humoral, cellular and specific immunity against brucellosis. For this purpose, beside usual classical tests: serodiagnosis, total and specific IgE assays, intradermoreaction and lymphoblastic transformation test with specific antigen (PI fraction) and an adaptation of human basophil degranulation test (HBDT) with PI fraction were performed on all 24 patients. Exploration of immediate hypersensitivity allows us to show neither an abnormal increase of hyper-IgE frequency nor an increase of anti-brucella specific IgE. HBDT is positive in 41 p. cent of the patients. It confirms that immediate hypersensitivity exists. HBDT gives justification for trying desensitization which has been proposed in the past. HBDT will possibly allow us to evaluate desensitization effectiveness.

The antifungal activity of carrier peptides, L-arginyl-X-L-phenylalanine, containing amino acid antagonists or atypical non-biogenic D-amino acids in the central position.

Eleven analogues of L-arginyl-D-allo-threonyl-L-phenylalanine, a naturally occurring peptide with antifungal activity, were synthesized. Two tripeptides of the form L-arginyl-X-L-phenylalanine (X = p-F-DL-phenylalanine or m-F-DL-tyrosine) inhibited Aspergillus fumigatus and Aspergillus flavus. In comparison with the free antagonists, the tripeptide-bound antagonists were more active against Candida-albicans-isolates which means that the amino acid sequence may serve as carrier function: it enhances delivery or uptake of the antimetabolite.

Systemic infection with Trichosporon cutaneum in a patient with acute leukemia: report of a case.

A case of disseminated infection with Trichosporon cutaneum, a fungus that causes white piedra, is described. The patient, a 58-year-old barber with acute leukemia, had fever, myalgias and skin lesions. He was receiving cytotoxic drug therapy and prednisone, was severely neutropenic and was being treated with broad spectrum antibiotics. Blood cultures and a biopsy of the skin lesion grew T. cutaneum. He died despite amphotericin B therapy. At autopsy, widespread infection with T. cutaneum was present. T. cutaneum is another fungus capable of causing widespread systemic disease in the immunocompromised host.

[Discriminant factors of risk of infection in routine digestive surgery. Trial apropos of 308 cases].

Postoperative infection in routine digestive surgery is a common complication, its prevalence being variously assessed between 10 and 30%. Study of criteria of infectious risk, whether inherent to the subject or his environment, has not been the subject of overall evaluation. The present study involved 308 subjects in two departments of surgery with different geographical and architectural features. A comparison was first made between the two departments in terms of the surgical population and the reaction rate. The presence was then sought of a relationship between infection, whether general or local (urinary tract, pulmonary, septicaemia) and factors concerning the bacteriological environment and factors indicative of the resistance of the subject. The statistical technique of discriminant analysis was used to determine the preponderance of certain factors which might be indicative of at risk subjects.

[Pneumocystis carinii Delanoe and Delanoe, 1912. The different methods of identification (author's transl)].

Pneumocystis carinii Pneumonia diagnosis relates on the direct detection of parasites from samples giving the best productivity. The various methods of identification do not detect the same structures. Their efficacity is evaluated using parasites from immunodepressed rat lungs. Our experience confirms that Toluidine Blue O stain may be first realized, eventually associated with Grocott's method.
