[Tuberculosis control in lower Franconia 1995 - 2001. Case-finding and treatment outcome].

BACKGROUND: How useful is active case-finding for the controlling of tuberculosis (TB) in Lower Franconia? What treatment outcome is being achieved and which factors reduce this success? METHODS: All 867 TB cases reported to the public health departments in Lower Franconia between 1995 and 2001 were used as anonymous data for a prospective study. RESULTS: Average age, gender, country of origin and rates of bacterial resistance of the patients coincided to a large degree with the data of the DZK study. The percentage of resettlers from the former Soviet Union with TB was, however, almost double. Therapeutic success was reached in approx. 79 % of the contagious TB and thus surpassed the rates of published investigations slightly. A negative impact upon the treatment outcome was caused by the following circumstances: A number of asylum seekers disappeared into illegality before the treatment was completed; certain TB-patients stopped taking the medication after surgical removal of the focus; only half of the sick persons who died of the disease were diagnosed when still alive. CONCLUSION: Active TB case-finding is useful mainly among asylum seekers and resettlers. An improvement of the treatment outcome in Lower Franconia is desirable but appears to be only possible within strict limits.

Testicular function after radioiodine therapy for thyroid carcinoma.

Radiotherapy can cause infertility in both men and women. However, few data are available concerning the effects of radioiodine therapy for thyroid carcinoma on testicular function. We investigated 25 men (age 23-73 years) with differentiated thyroid carcinoma in a longitudinal prospective trial. Follicle-stimulating hormone (FSH), inhibin B, luteinising hormone (LH) and testosterone were measured before (n = 25) and 3 months (n = 11), 6 months (n = 18), 12 months (n = 22), and 18 months (n = 18) after radioiodine therapy [radioiodine dose (mean +/- SEM): 9.8+/-0.89 GBq]. Before therapy, FSH was 5.4+/-0.77 IU/l; it increased significantly (P<0.001) to 21.3+/-2.4 IU/l after 6 months and fell to 7.4+/-1.3 IU/l after 18 months (normal range: 1.8-9.2 IU/l). Inhibin B was significantly decreased (P<0.001) from 178+/-25.3 pg/ml before therapy to 22.2+/-5.5 pg/ml after 3 and 29.4+/-5.7 pg/ml after 6 months and rose to 154+/-23.3 pg/ml after 18 months (normal range 75-350 pg/ml). LH and testosterone were within the normal range during the whole study (1.6-9.2 IU/l and 10.4-34.7 nmol/l, respectively). LH was significantly increased (P<0.001) from 2.8+/-0.33 IU/l before therapy to 5.9+/-0.69 IU/l 6 months after therapy and then fell slowly to 4.0+/-0.45 IU/l after 18 months. Total testosterone was significantly increased (P<0.01) from 12.8+/-0.99 nmol/l at baseline to 19.8+/-1.7 nmol/l after 12 months and 19.6+/-1.7 nmol/l after 18 months. The testosterone/LH ratio (normal range: 3.3-17.9 nmol/IU) fell from 5.8+/-0.66 nmol/IU to 3.0+/-0.36 nmol/IU after 3 months (P<0.01); it remained close to the latter value after 6 months (3.4+/-0.49 nmol/IU) and then rose to 5.5+/-0.6 nmol/IU after 18 months. In conclusion, 3 and 6 months after radioiodine therapy all patients showed elevated FSH and decreased inhibin B levels, reflecting severely impaired spermatogenesis. At the same time a compensated insufficiency of the Leydig cell function was observed. Eighteen months after the last radioiodine therapy, mean values of gonadal function had completely recovered.

Molecular cloning and functional expression of the bumetanide-sensitive Na-K-Cl cotransporter.

By mediating the coupled movement of Na, K, and Cl ions across the plasma membrane of most animal cells, the bumetanide-sensitive Na-K-Cl cotransporter (NKCC) plays a vital role in the regulation of ionic balance and cell volume. The transporter is a central element in the process of vectorial salt transport in secretory and absorptive epithelia. A cDNA encoding a Na-K-Cl cotransport protein was isolated from a shark rectal gland library by screening with monoclonal antibodies to the native shark cotransporter. The 1191-residue protein predicted from the cDNA sequence has 12 putative transmembrane domains flanked by large cytoplasmic N and C termini. Regulatory phosphoacceptor residues in isolated peptides are identified as Thr-189 and Thr-1114 in the predicted sequence. Northern blot analysis identified a 7.4-kb mRNA in rectal gland and most other shark tissues; a 5.2-kb mRNA was restricted to shark kidney. Homology with an uncharacterized gene from Caenorhabditis elegans and with the thiazide-sensitive Na-Cl cotransporter of flounder urinary bladder was found over most of the coding region; shorter stretches of homology were found with a C. elegans cDNA and with an uncharacterized gene of cyanobacterium. Human HEK-293 cells have been stably transfected with the shark cDNA and shown to express Na-K-Cl cotransport activity with the bumetanide sensitivity of the shark protein. The expressed transporter is functionally quiescent in the host cells and can be activated by depleting the cells of chloride.

[Central venous approach in pediatric cancerology].

Between 1-04-1980 and and 1-01-1982, 50 catheters have been inserted in children from 6 months to 16 years of age. The silicone right atrial catheter is implanted using surgical technique under local or general anesthesia in a cervical or brachial vein. The mean duration of catheter life was 6 weeks. Catheters have been inserted according to the exact nature of cancer, the intensity of the treatment and the preexistent malnutrition (acute Leukemia, malignant lymphoma, bone marrow transplant, ...). The mechanical complications and the infections risk were studied. These incidents must not contraindicate this technique because the setting up of a central venous line allows in a treated child: 1. To maintain a correct nutritional state. 2. To slow the functioning of the digestive tract. 3. To further the administration of the therapeutic and the hematologic supervision in good conditions of security and comfort.

Stability of the individual globin genes during erythroid differentiation.

The genes of sheep betaA, betaC, and gamma globin were all present in DNA from erythroid cells which synthesized only betaC globin. Similarly, selective excision of non-expressed genes was shown not to occur during human erythroid differentiation. In contrast, evolutionary deletion of the betaC gene accounts for the inability of many sheep to make this globin.
