A morphometric expression of differentiation in fine-needle biopsies of breast cancer.

An automated method of particle sizing in fine-needle biopsies of human breast carcinomas is described. The average size of clusters of cells aspirated from breast carcinomas was strongly correlated with cytologic differentiation of the tumors and also with the recurrence rate 3 years after mastectomy so that smaller cluster size was associated with poor differentiation and a poor prognostic outcome. It thus seems possible to express in quantitative terms the degree of differentiation of breast carcinomas.

Multifocal breast carcinoma.

The present study comprises 173 breast cancers in women living in Sweden. Mastectomy was performed and the surgical specimens were thoroughly scrutinized histopathologically with special attention given to the mammary tissue outside the dominant mass. Fifty-two patients (30 percent) had multifocal growth in the same breast as the dominant breast cancer. The patients have been followed up for 3 years after operation and compared with age-matched controls. The multifocal growth was not correlated with age, size of the tumor or death of the patient within 3 years. A previous diagnosis of benign breast disease was significantly more common among the cancer patients than among the controls, although it was unrelated to multifocal growth. This study stresses the importance of considering the high incidence of multifocal growth of breast cancer when discussing treatment by operation less radical than mastectomy.

Levels of metrifonate and dichlorvos in plasma and erythrocytes during treatment of schistosomiasis with Bilarcil.

A method for the simultaneous quantitation of metrifonate (0,0-dimethyl-(1-hydroxy-2,2,2-trichloroethyl)-phosphonate) and dichlorvos (2,2-dichlorovinyl dimethyl phosphate, DDVP) in human blood has been worked out. It is based upon multiple labelling of the compounds with deuterium and gas phase analysis using the mass spectrometer as a selective detector. The amount of DDVP in plasma is about 1% of the amount of metrifonate. In erythrocytes the corresponding amount of DDVP is 0.5% or less of metrifonate. Both compounds reach peak levels in blood within two hours after oral dosing and are detectable for at least eight hours. Cholinesterase activity in plasma reaches zero levels within 15 min. and remains inhibited for more than eight hours. Red blood cell cholinesterase is inhibited only 60-80%. According to kinetic calculations, clearance of metrifonate occurs primarily via dichlorvos. If dichlorvos is the only active component, which in all likelihood it is, it's slow release may be important in the schistosomicidal effect. Clinical data in seven metrifonate treated patients revealed that mild vertigo subsiding in a few hours was the most common side effect.

Asymptomatic congenital Chagas' disease in a 5-year-old child.

A case of asymptomatic Chagas' disease is a 5-year-old boy is described. The boy was born in Romania in 1975 and had never visited an area where Chagas' disease is endemic. It is important bear in mind the possibility of transplacental transmission, in view of increasing number of immigrants and refugees coming from South and Central America.

The effect of timolol maleate on the disruption of the blood-aqueous barrier in the rabbit eye.

A disruption of the blood-aqueous barrier in rabbit eyes was elicited by use of topical prostaglandin E2(PGE2), infrared irradiation of the iris, or by subcutaneous alpha-melanocyte-stimulating hormone (alpha-MSH). The aqueous flare provoked was measured quantitatively with a photoelectric instrument. The effect of the (topical) beta-adrenergic antagonist timolol maleate on the breakdown of the blood-aqueous barrier was tested. Timolol applied topically in very large doses had no effect on exogenously administered PGE2. However, even in a very small concentration applied topically, timolol reduced the flare response to both infrared irradiation and alpha-MSH. These results support the theory that the effect of alpha-MSH and infrared irradiation on the blood-aqueous barrier is dependent on intact beta-adrenergic receptor sites.

TULIPS: the Uppsala-Linkoping Image Processing System.

The Uppsala-Linkoping Image Processing System, TULIPS, is described. TULIPS, a hardware-software system designed for cell image processing, was developed at Uppsala University Hospital in cooperation with the Department of Electrical Engineering at Linkoping University. The hardware part of the image processing system is built around a high-speed data bus with a capacity of about 40 M byte/sec connected to a PDP-11/55 host computer. An image memory, an LSI-11 microcomputer and a video interface for displaying the image memory content on a TV monitor are also connected to the high-speed bus. An automated microscope and a "Poulsen processor" for low resolution segmentation, both to be attached to the high-speed bus, are being developed. A monitor and an interpreter for an image processing language have been implemented on the host computer. This software system allows interactive, as well as batch, processing. The degree of user interaction is easily adapted to the user's needs. The image processing language is command oriented, and it is easily expanded by adding new commands. The system has been used both for studies in the field of quantitative microscopy and as a platform for development and testing of new image processing algorithms.

Plasma levels of metrifonate and dichlorvos during treatment of schistosomiasis with bilarcil.

Metrifonate (0,0-dimethyl-[1-hydroxy-2,2,2-trichloroethyl]-phosphonate), and its rearrangement product dichlorvos (2,2-dichlorovinyl dimethyl phosphate) (DDVP), were studied in plasma from two patients with schistosomiasis who were treated with Bilarcil. A mass fragmentographic technique was used. Isotopic variants of the substances were used as internal standards and to compensate for DDVP formed during the workup procedure. The results were related to erythrocyte and plasmacholinesterase determinations. The method described makes it possible to study pharmacokinetics in man and to relate this information to therapeutic effects. It is proposed that metrifonate acts as a slow release formulation for DDVP.

Malaria and haemoglobin A2 levels in northern Liberia.

The influence of malaria on HbA2 levels was investigated in two groups of children aged two to nine years from the Mano tribe of northern Liberia. One group, 174 children living in a town where there is malaria control, had a parasite rate of 6.5%, only a few having palpable spleens, but 282 children living in an area of intense malaria transmission had a parasite rate of 92%. All but one child in this group had enlarged spleens. However, the difference in proportions of elevated HbA2 values within the limits for beta-thalassaemia, 8% and 10.3% respectively, was not statistically significant (0.5 greater than P greater than 0.1). It was concluded that the influence of malaria on HbA2 levels is not significant and that this parameter is valid for detecting beta-thalassaemia trait in this population. Further, iron deficiency may be a more important factor than malaria to consider when assessing the results of HbA2 estimations.

A pocket of controlled malaria in a holoendemic region of West Africa.

Yekepa, a mining town in northern Liberia, has been built entirely since 1960 and now has a population of 16 000 inhabitants including 1500 expatriates. Although situated in a holoendemic region with constant human movements in and out of the town, the mining company has succeeded in controlling malaria in Yekepa. Furthermore, there is a constant threat of the vector in the close surroundings to the town. Control is maintained by regular residual insecticide sprayings with DDT, regular larviciding with fuel oil and fortnightly issue of amodiaquine chemoprophylactic to all workers. A Malariometric survey showed that the spleen and parasite rates were 11% and 13% respectively in the controlled areas and 95% and 67% respectively in surrounding regions not subjected to control measures. The dominant malaria parasite in the area was Plasmodium falciparum. No adult vectors were found in the town. In the surrounding villages the average room density of adult vectors was 3.8 and the sporozoite rate in a village very close to the town was 9.2%. The dominant vector of the area was Anopheles gambiae with A. Funestusalso being present. The annual per capita cost, including all control activities, is about 4--5 US dollars.
