Segmental living related small bowel transplantation in adults.

The advent of small bowel transplantation has provided selected patients with chronic intestinal irreversible failure with a physiologic alternative to total parenteral nutrition. Recently a standardized technique for living related small bowel transplantation (LR-SBTx) has been developed. Three patients with short bowel syndrome underwent LR-SBTx at our institution. All donors were ABO compatible with a good human leukocyte antigen match. A segment of 180 to 200 cm of ileum was harvested and transplanted with its vascular pedicle constituted by the ileocolic artery and vein. The grafts were transplanted with a short cold and warm ischemia time. The immunosuppression regimen consisted of oral FK-506, prednisone, and intravenous induction with atgam. Serial biopsies of the intestinal grafts were performed to evaluate rejection or viral infections. The postoperative course was uneventful for all donors. All of the recipients are currently alive and well. Two of three patients are off total parenteral nutrition and tolerating an oral diet with no limitations on daily activity. In the third patient, the graft was removed 6 weeks after transplantation. At the time of enterectomy, no technical or immunologic complications were documented. Absorption tests for D-xylose and fecal fat studies were performed showing functional adaptation of the segmental graft. All biopsies were negative for acute rejection. A well-matched segmental ileal graft from a living donor can provide complete rehabilitation for patients with short bowel syndrome. Our initial experience suggests that the risk of acute rejection and infection is greatly reduced compared to cadaveric bowel transplantation. Further clinical application of this procedure is warranted.

On the oxy analogues to the 4 leads to 1 intramolecularly hydrogen-bonded peptide conformations.

The occurrence of the oxy analogue to the type II' 4 leads to 1 intramolecularly hydrogen-bonded nonhelical peptide conformation, recently proposed for t-BOC-Gly-L-Pro-OH in the solid state by Deber on the basis of infrared absorption evidence, has been disproved by x-ray diffraction analysis. This type of folding is also absent in solvents of moderate or high polarity. The latter conclusion is in agreement with Deber's results. However, in solvents of low polarity this intramolecularly hydrogen-bonded form could account for the strong negative Cotton effect near 230 nm observed in the circular dichroism spectrum.

On the oxy analogues to the 3 leads to 1 intramolecularly hydrogen-bonded peptide conformations.

The possible occurrence of the oxy analogues to the 3 leads to 1 intramolecularly hydrogen-bonded peptide conformations (main feature of the gamma turn), recently proposed in solution by several authors, has been investigated in a number of N-tert-butyloxycarbonyl-alpha-amino acids by x-ray diffraction, infrared absorption, proton magnetic resonance, and circular dichroism techniques. These folded conformations are absent in the solid state in all cases so far examined; however, they seem to be present in solution, the extent of the population of these forms in the conformational equilibrium mixtures being solvent, temperature, and structure dependent. In the solid state N-tert-butyloxycarbonyl-D-valine has the urethane-CONH-group in the cis configuration; this is the first time such a configuration has been found in the solid-state for a secondary amide group in a linear compound.

Solid-state and solution conformation of N-tert-butyloxycarbonyl-L-prolylglycine.

The occurrence of the oxy analogue in the trans-II 4 leads to 1 intramolecularly hydrogen-bonded nonhelical peptide conformation, recently proposed for t-BOC-L-Pro-Gly-OH in the solid state on the basis of infrared absorption evidence, has been disproved by x-ray diffraction analysis. This type of folding is also absent in polar solvents. However, in solvents of low polarity the presence of this folded form cannot be excluded.

Pre-emptive renal transplantation.

We report our experience with pre-emptive renal transplantation and review the literature. While eliminating the cost, complications and inconvenience of dialysis, transplantation prior to dialysis therapy can be performed safely and effectively as it does not pose any additional immunological hazards to allograft outcome. It is safe regardless of the immunosuppressive agents employed and is successful without early rejection even in the nonuremic state.

Pancreatic transplantation: a review.

Pancreatic transplantation recently became a routine treatment for Type I diabetic patients with uremia or for those who previously received a kidney transplant with 1 year graft and patient survival of over 80% and 90%, respectively. Despite the life-long need for immunosuppression, this is clearly acceptable when compared to the need for dialysis and insulin therapy, and it reduces the evolution of diabetic complications. Isolated pancreatic transplant is less commonly applied because of the need for immunosuppression and the high rate of complications. However, this can still be an acceptable option for individual patients with brittle diabetes and hypoglycemic unawareness. Despite the fact that pancreas transplantation is an effective treatment for selected Type I diabetics, it remains a difficult surgical procedure with many potential complications and with several issues still subject to debate. In this article, the authors describe the procedure in all of its aspects and variations, and offer, through a review of the recent literature, insights on the current status of this transplant

[Poverty and nutrition: a survey on the under age sons of the beneficiaries of the R.M.I. in the L'Aquila District Area].

The alimentary education, let alone the relative behaviour, represent an important preliminary statement for a children perfect growth. The results of a not healthy nutrition or alimentary mistakes can produce several problems. The study aims to analyze the child and adolescent nutrition for families with economic difficulties that receive a (social and economic) benefit called "Reddito Minimo di Inserimento" (RMI), within the L'Aquila district area. For the valuation of nutrition state, it has been used the BMI (Body Mass Index), with the necessary corrections for sex and age. From the made analysis, the children's about 33% is classifiable as "at risk of soprappeso" or as "soprappeso", with prevalence for the males, above all in the classes of younger ages, like the most recent studies on the children's nutrition.

A second polymorph of a helical decapeptide.

A crystal-state structural analysis of the terminally blocked apolar decapeptide pBrBz-(Aib-L-Ala)5-OMe bis-dimethylsulfoxide solvate was performed by x-ray diffraction. The peptide molecules are basically alpha-helical with five 1<--5 C = O... H-N intramolecular H bonds. Near the C terminus the regularity of the alpha-helix is disrupted in favor of the formation of intramolecular H bonds of the 1<--4 (beta-bend) and 1<--6 (pi-bend) types. Differences in conformation, solvation and association with the published structure of the tetrahydrate decapeptide polymorph are discussed.
