De novo concurrent 5p deletion and distal 17q duplication identified by fluorescence in situ hybridization (FISH).

A de novo case of unbalanced 5p/17q translocation showing a concurrent 5p deletion and distal 17q duplication has initially eluded a definitive cytogenetic identification based on chromosome banding patterns and clinical features. But it is subsequently unequivocally identified by fluorescence in situ hybridization technique using various whole chromosome painting probes.

Effects of parenteral diclofenac sodium on upper gastrointestinal motility after food in man.

In experimental animal models nonsteroidal anti-inflammatory drugs may influence gastrointestinal motility, but as evidence is lacking in man. The effect of diclofenac sodium 75 mg i.m. on the motor response of the upper gastrointestinal tract to food has been studied by manometry in 9 healthy volunteers. Diclofenac had no effect on the motor activity of the stomach, duodenum, or jejunum after a 605 kcal meal.

[Fever caused by penicillin. Description of a clinical case].

A case of drug-provoked fever is described in a scleroderma patient on penicillamine. The drug was withdrawn because of the severity and recurrence of the fever. Both our personal experience and a review of the literature, show this side effect to be markedly less frequent than reported by German researchers.

Oesophageal motor activity in rheumatoid arthritis: a clinical and manometric study.

Oesophageal motor function was clinically and manometrically investigated by means of a low-compliance system in 24 nonselected rheumatoid arthritis (RA) patients and 15 healthy controls. Dysphagia was referred by 37.5% of RA patients. Oesophageal motor abnormalities were discovered to be significantly present in a high percentage (62.5%) of patients with RA. However, there was no correlation between oesophageal motor abnormalities and any disease characteristic. It is concluded that a high percentage of RA patients have an oesophageal motor dysfunction, even in the absence of dysphagic symptoms.

Imipenem/cilastatin in the treatment of severe hospital infections.

Imipenem/cilastatin as a single agent or in combination with amikacin was used as empirical treatment of severe hospital infections. Twenty-five patients were evaluable for efficacy and the overall response rate was 62% with imipenem/cilastatin alone and 80% with imipenem/cilastatin in combination with amikacin. The highest response rate was obtained in urinary tract infection (75%) and in pneumonia (70%) and the lowest response rate (50%) was observed in bacteremia of unknown origin and in skin and soft tissue infections. Eight failures were observed and seven of them occurred in patients treated with imipenem/cilastatin alone. Two deaths occurred, both in patients with bacteremia. Imipenem/cilastatin treatment was interrupted early in 3 patients because the pathogen developed resistance during therapy and in 2 other patients because of side effects. In our study imipenem/cilastatin proved to be efficacious and well tolerated. The addition of an aminoglycoside to imipenem/cilastatin might improve its efficacy and prevent pathogens from becoming resistant during therapy. Therefore this association would seem to be advisable for the therapy of bacteremic infections and for those caused by difficult pathogens.

Treatment of infections by staphylococci and other gram-positive bacteria with teicoplanin: an open study.

Teicoplanin, 200-400 mg (3-6 mg/kg) daily iv or im, was used to treat 71 episodes of infection. The average duration of treatment was 22 days. The 64 evaluable episodes comprised 24 skin/soft tissue, 20 osteoarticular, ten urinary tract and one ventriculo-atrial shunt infections; one case of primary bacteraemia, three of endocarditis, two of pneumonia and three of pleural empyema. Fifty-five episodes were treated with teicoplanin monotherapy and nine with teicoplanin in association to other antibiotics. Overall 61% (39/64) of the evaluable infections were cured, 25% (16/64) improved and 14% (9/64) failed. Staphylococcus aureus was the most frequent pathogen, with 46 isolates. Infections by both methicillin-sensitive and resistant Staph. aureus strains showed favourable clinical and microbiological responses to teicoplanin. Side effects were observed in eight of the 64 episodes (12.5%). Bronchospasm was observed in two other cases at the beginning of therapy and the antibiotic administration was discontinued. Teicoplanin is an effective and well tolerated antibiotic for infections by Gram-positive bacteria, and it is effective against methicillin-resistant staphylococci.

Determination of iodine-131 at low levels in milk: collaborative study.

The official method for the determination of iodine-131 in milk has a lower limit of detection (LLD) of 10 pCi/L (0.37 Bq/L). The Nuclear Regulatory Commission had recommended that a method claiming to have an LLD of <0.3 pCi/L (<1.1E-02 Bq/L) be used. That method, which is capable of measuring iodine-131 below the level of detection of the Official Method, was collaboratively studied. The method uses a palladium iodide precipitate to concentrate the iodine-131 and measures (counts) its 364 keV gamma energy in coincidence with the beta decay or with a low-background beta-counting system. The study was performed by using 3 concentrations of iodine-131 in milk: 2.6, 5.0, and 8.0 pCi/L (9.62E-02, 1.85E-01, and 2.96E-01 Bq/L, respectively). Eleven laboratories agreed to participate in the study. Eight laboratories submitted data for the study. The averages of the results were 2.68, 5.30, and 8.12 pCi/L (9.92E-02, 1.96E-01, and 3.00E-01 Bq/L, respectively), respectively. The intra- and interlaboratory variations were acceptable. The method was adopted First Action by AOAC INTERNATIONAL.

[Adverse effects of corticosteroids].

Due to their anti-inflammatory and immunosuppressive properties, as well as their action on the hematopoietic system and the calcium phosphate turnover, corticosteroids are widely employed in the treatment of many diseases. However, the therapeutic use of these compounds is frequently associated with unwanted, mainly type A, effects that are directly dependent on the wide spectrum of their pharmacological activities. In this study these side effects are critically reviewed and classified on the basis of the organs and systems involved. Where possible, the incidence, pathogenesis, type of steroid, dose and schedule are reported, and the precautions that should prove useful in reducing the incidence and gravity of the side effects are given.
