Ranitidine vs metoclopramide in the medical treatment of reflux esophagitis.

45 patients with symptomatic reflux esophagitis were randomly treated with either Ranitidine (150 mg b.i.d.) or Metoclopramide (10 mg t.i.d.) for six weeks. The severity of dyspeptic symptoms and the grade of endoscopic and histological esophagitis were assessed before and after treatment. Both drugs proved significantly effective in inducing symptomatic and endoscopic improvement, but Ranitidine appeared significantly superior in promoting disappearance or improvement of endoscopic esophagitis. Moreover Ranitidine was found to significantly reduce the severity of histological changes, whereas Metoclopramide was unable to do so.

[Truncal vagotomy and cholelithiasis].

A group of 50 patients with total vagotomy plus pylorolasty are studied with pre and postoperative cholecystography in a four years follow-up. 50 patients with parietal cell vagotomy (selective vagotomy) served as control group. The AA. study the correlation of gallbladder dilatation with gallstones formation after total vagotomy. On the contrary parietal cell vagotomy prevents these dysfunctions.

Double-blind trial of pirenzepine versus sucralfate in the treatment of endoscopic duodenitis.

Thirty dyspeptic patients with endoscopic evidence of mild to moderate (nonerosive) duodenitis were randomly assigned to treatment with 50 mg of pirenzepine twice daily or 2 gm of sucralfate twice daily for four weeks. Posttreatment endoscopy revealed normalization of duodenal mucosa in half of the patients in each treatment group. The severity of dyspeptic symptoms was significantly reduced in both groups, with no significant between-group differences.

[Newborn infants of drug-addicted mothers. Review of the literature and report of personal cases].

The effects of drug addiction in perinatal period and its clinical, diagnostic and therapeutic problems have been considered. The authors reported five cases of newborns from addicted mothers. Neurologic symptoms were present in all cases, gastro-intestinal disturbances were present in two newborns. One presented with convulsions and died for severe hyaline membrane disease. One newborn was small for gestational age and one was premature. Treatment has been carried out successfully with diazepam and/or phenobarbital in four cases. The authors stress the importance for a careful methadone program during pregnancy in order to reduce prematurity and neurologic sequelae of the narcotic withdrawal syndrome.
