The recurrent laryngeal nerve: related vascular anatomy.

This study was based on 34 recurrent laryngeal nerve dissections after arterial casting with red-colored latex. The aim was to provide specific information about the perineural microvasculature. This study established the following points: 1. a great anatomic variability does exist; 2. the laryngeal nerve is usually in relation to the posterior branch of the inferior thyroid artery; and 3. this vascular branch is sometimes replaced with a vascular network. In all cases, this microvascularization must be preserved during thyroid surgery.

[Temporal fibrous dysplasia with labyrinthine involvement. Apropos of a case and review of the literature].

Craniofacial fibrous dysplasia manifests clinically with dysmorphic syndrome, ophtalmologic or otologic signs. We report a case of temporal polyostotic fibrous dysplasia with vertigo and sensorineural hearing loss. Sensorineural hearing loss, with no cholesteatoma associated, as described in our case is rarely described. We review the literature concerning main clinical, diagnostic and therapeutic aspects of this disease.

[A case of Kikuchi disease. Review of the literature].

We report a case of Kikuchi's disease in a young man. It is a rare entity, described for the first time in 1972. It manifests clinically with cervical adenopathy and fever, often associated with other non-specific clinical signs. Laboratory tests are often normal. The diagnosis is established on the basis of histologic of lymph node excisional biopsy. Kikuchi's disease is likely to be misinterpreted as malignant lymphoma or systemic lupus erythematosus. It generally needs no treatment because it runs a spontaneously benign course with complete resolution of the symptoms within 6 months. Secondary systemic lupus erythematosus may develop. For this reason, regular follow-up of patients is recommended. The etiopathogenesis of this disease is still unknown, perhaps due to an immunologic phenomenon. It could be a hyperimmune reaction induced by various antigenic agents (infectious, neoplasic), or an autoimmune process.

[Extemporaneous histologic examination of cutaneous carcinoma of the face].

One hundred thirty-eight skin cancers of the face were reviewed retrospectively in order to evaluate the extemporaneous histology examination. The results obtained were satisfactory and in agreement with those in the literature. This technique is perfectly adapted to cancers with a facial localization. In our opinion, the indication for extemporaneous examinations should be widened to all cases of epithelioma of the face.

Identification of a human delta opioid receptor: cloning and expression.

The delta opioid receptor is an important target for analgesic drug development. This report describes the identification of delta opioid receptor clones from human cDNA libraries and the preparation of a human delta receptor cDNA in the pcDNA3 expression vector for transfection studies. The cDNA encodes a 372 amino acid protein that has 93% amino acid identity to mouse and rat delta receptors. COS-7 cells transfected with this clone express over 1.0 pmol receptor/mg protein when measured by saturation binding with [3H]naltrindole. The delta receptor selective ligands NTB, BNTX, [4'-Cl-Phe4]DPDPE and [D-Ala2,Glu4]deltorphin all have Ki values under 10 nM while the affinities of the mu and kappa opioid receptor ligands CTAP and U-69593, respectively, are over 4.0 microM. Agonists show binding to multiple affinity states of the receptor consistent with the presence of G-protein coupled and uncoupled forms of the expressed receptor. The 8-fold higher affinity of NTB relative to BNTX suggests that the human delta receptor is of the delta 2 subtype.

[The use of cryotherapy in oral pathology. Our experience in 36 cases].

Cryotherapy is a simple to use, weakly invasive technique provoking only moderate complications in the treatment of certain mucosal lesions of the buccal cavity. Indications of choice for this type of treatment are angioma and papilloma. Long term follow up of patients treated for reactive keratosis or epithelioma in situ is at present insufficient to establish precise indications.

[Radiological aspects of cerebral and spinal cysticercosis. Apropos of 2 cases].

Two cases of neurocysticercosis are reported, one cerebral and the other spinal. Results of radiologic imaging of this affection, a rare parasitosis in occidental Europe, are discussed with emphasis on computed tomography findings. In fact, when perfect conditions can be established the CT scan provides data confirming the diagnosis of cerebral cysticercosis in most cases. However, this scan is insufficient for investigation of ventricular or cisternal forms when gas or positive contrast is sometimes necessary for visualization of parasitic cysts. Radiologic diagnosis of extra- and intra-medullary forms is based an myelography with water-soluble contrast completed by a scan.

[Congenital anomalies at the emergence of lumbosacral roots].

Thirteen of 45 cases of congenital anomalies of the lumbosacral nerve roots at their site of emergency (ARE) were treated surgically. The most frequently involved roots were L5 and S1, and the commonest anomaly observed was fusion of adjacent roots. The malformations were either bilateral, symmetric or asymmetric, their preoperative diagnosis being by radiculography with metrizamide. Diagnostic criteria and limitations relating to the different varieties of ARE are discussed, confirmation of the presence of the anomaly depending on two peroperative findings. Conventional operative techniques involve wide surgical exploration, but pediculectomy supplies a new approach to the problem, and the advantages of a microsurgical technique are discussed. Improved results can be expected in the future following preoperative recognition of the presence of ARE and advances in operative techniques.

Spinal subdural metastasis. A case report.

A case of metastasis to the thoracic spinal subdural space is reported. Metrizamide myelography showed narrowing of the upper thoracic subarachnoid space. A tumor in the spinal subdural space is unusual; only a few cases have been reported. The myelographic appearance is not specific; an epidural metastasis can produce the same myelographic picture.

Percutaneous puncture of spinal cysts in the diagnosis and therapy of syringomyelia and cystic tumors.

The experience with percutaneous puncture of intraspinal cystic lesions is reported in four cases: an extradural cyst demonstrated with air, a spinal cord tumor with lipiodol and two cases of syringomyelia with metrizamide. Percutaneous puncture of cystic lesions makes possible an accurate delineation of the lesion and a histological approach through biochemical and cytological analysis of the aspirated cystic fluid, while aspiration of the fluid of cystic tumors decompresses the spinal cord and thus plays a therapeutic role.

[Spondylocostal dysostosis and polydactylia].

The authors report the case of a 22-month-old boy who presented a spondylocostal dysostosis associated with preaxial polydactylia. This peculiar malformation syndrome seems to be the first reported in the literature till today.

Metrizamide demonstration of the subarachnoid space surrounding the Gasserian ganglion.

The chance opacification of the subarachnoid space surrounding the Gasserian ganglion, observed during metrizamide basal cisternography, is reported. This is compared to similar demonstrations of the optic subarachnoid space. Such infrequently observed images should be known because they may be related to the occurrence of trigeminal neuralgia.
