[Value of computer tomography in the diagnosis and development of rectal cancer].

The accuracy of CT in staging rectal carcinoma was examined on the basis of 172 preoperative CT-examinations of the pelvis. Especially in cases of advanced tumours CT was found to be superior to clinical staging as defined by Mason, except for low ventral tumour localisation. On the other hand, routine staging by CT is not justified because mesenteric lymph node metastases cannot sufficiently be predicted. The value of various diagnostic methods in detecting local recurrences was determined in 295 other patients after radical resection of the rectosigmoid due to carcinoma. Recommendations for effective follow-up studies of those patients are based on these results.

[Angiography of the upper abdomen using DSA. An alternative to conventional arteriography?].

DSA is not suitable for the investigation of hepatic and splanchnic vessels by intravenous or non-selective arterial injections. On the other hand, its use in selective arterial injection offers many advantages compared with conventional arteriography. It can be particularly recommended for routine use in the pre-operative diagnosis of abdominal masses. In some cases additional conventional angiography may also be required and it is therefore desirable to perform both examinations at one unit in order to profit from the advantages of each technique.

[Arterial digital subtraction angiography for demonstrating aortocoronary venous bypasses].

We examined the patency of aortocoronary venous bypasses in 51 patients during the early postoperative phase by using non-selective contrast injection into the aorta with digital subtraction angiography. In all patients it was possible to evaluate the degree of patency by this method. The accuracy and value of this examination, as compared with other invasive and non-invasive methods, is discussed.

[Diffuse infiltrating lipomatosis. A subentity of intrapelvic fatty tumor].

Nineteen patients with various types of lipoma in the pelvis are described. Clinical findings, radiological examinations, and follow-up studies constitute a particular type of lipoma, diffuse infiltrating lipomatosis; this can be distinguished from other fatty tumours because of its localisation, extent, therapeutic results and long course. Computed tomography enables us to differentiate simple lipomatosis pelvis from diffuse infiltrating lipomatosis. Histological examination is essential for excluding a liposarcoma. Growth or malignant change of diffuse infiltrating lipomatosis is most easily recognized by computed tomographic serial observation.

[Functional images of the left ventricle using digital venous angiography].

Imaging and functional evaluation of the left ventricle are of great interest in cardiological diagnosis. So far, the left ventricle has been imaged by laevocardiography or by a nuclear medicine method (gated blood pool, first pass). The limited spatial and temporal resolution of nuclear methods and the invasiveness of selective laevocardiography are well known disadvantages of these techniques. With special software and hardware, it is possible to obtain functional images of the left ventricle by means of intravenous contrast injection; these provide definitive information concerning local myocardial contraction patterns.

[Value of computed tomography for tumors of the epi- and oropharynx].

The results of computed tomography in 89 patients with tumours of the epipharynx and oropharynx involving the mouth are compared with the findings of other forms of clinical examination. In 26 patients, CT provided additional relevant information, in eight patients CT provided less information than clinical examination. The value of CT depended largely on the localisation of the tumour; the poorest results were found in tumours of the tonsils and the soft palate. CT provided additional information most commonly with large tumours, since these are frequently associated with bone destruction, local infiltration and occlusion of large vessels.

[Digital subtraction angiography (DSA) in acute pulmonary embolism].

The results of 49 DSAs (in 29 patients) are presented; these were performed for the diagnosis or follow-up of pulmonary emboli. The direct or indirect signs of pulmonary emboli, known to occur during conventional pulmonary angiography, were used as diagnostic criteria. In 47 examinations it was possible to make or to exclude the diagnosis unequivocally. The advantages of DSA make it desirable to use this method as the first form of examination in the diagnosis of acute, but not immediately life-threatening, pulmonary emboli.

[Left-side changes in thoracic follow-up studies after heart surgery].

In routine thoracic x-ray follow-ups after cardiosurgical procedures, 93.2% of our patients (n = 88) showed radiologically perceivable pathological changes, reduced ventilation being the most common phenomenon. 77.1% the the patients showed signs of reduced ventilation, mostly the left side. Limited motility of the diaphragm on the left side was visible in 69% of the cases studied. The defective motility of the left diaphragm is attributable to direct damage to the left nervus phrenicus caused by extracardial heart cooling during surgery.

[Serial computed tomography -- methods and early experience (author's transl)].

Serial computed tomography following intravenous bolus injection of contrast agent is an important development in computed tomographic diagnosis. The technical necessities and methods of examination are discussed. Our experience with 108 examinations is evaluated. The additional information in terms of morphology and function is described.

[Suprapubic prostatic sonography in the diagnosis of carcinomas and adenomas of the prostate (author's transl)].

Prostatic adenomas and carcinomas were examined by suprapubic transvesical prostatic sonography. The value of the method is comparable with transrectal prostatic sonography. Adenomas and carcinomas can be differentiated on the basis of their echo patterns and marginal contours. In the presence of carcinomas, sonography is a valuable addition to clinical staging and staging by CT. Compared with CT, the ability to perform longitudinal sections permits good evaluation of the bladder base. Sonography is also valuable for serial observations after treatment and for diagnosing recurrences.

[Bühler's anastomosis (author's transl)].

The arc of Bühler between the celiac and superior mesenteric arteries has to be considered as an embryological persistence of the portion of the ventral longitudinal anastomosis. The collateral development of the anastomosis refers to a different hemodynamic in the upper visceral arteries. We discovered the arc of Bühler in 14 cases among 340 selective celiac and superior mesenteric arteriographic studies. Its existence permits conclusions about further angiographic procedures and facilitates the interpretation of visceral collaterals.

[Post partum occlusion of the hepatic artery: sonographic, computer tomographic and angiographic findings (author's transl)].

A case of post partum acute thrombosis of the hepatic artery with infarction of the liver is reported. The prognosis depends on the areas of obstruction, on the early development of a collateral circulation and on the contribution to the blood supply from the portal vein. The diagnosis of liver infarction was made by computed tomography. This was characterised by sharply demarkated hypodense areas, which did not enhance with contrast. An important clue is the lack of increase in density in the liver after intravenous injection of contrast when compared with the increased contrast of the spleen. This finding provided the indication for angiography. This confirmed the diagnosis of acute occlusion and made appropriate treatment possible.
