Frequency in allergy to proteins of latex in health care workers.

OBJECTIVE: The objective of this study is to evaluate allergy prevalence to latex proteins in health care workers at the Laboratory and Surgery Room of Hospital CIMA Chihuahua. METHODOLOGY AND RESULTS: A thorough clinical chart was recorded for all health care workers studied: hematic biometry, total IgE by ELISA method, specific IgE to latex antigen by ELISA (pharmacy CAP system), cutaneous tests with latex antigen (Aphi de México, Hevea Brasiliensis Biopal Inc. Spoken WA), through scarification, together with histamine and Evans (positive-negative control). The number included is 99 individuals. Specific IgE to latex in 4 cases was positive (4%) and in the cutaneous tests to latex, 24 cases (24%) resulted positive. CONCLUSIONS: The study reports a prevalence of (4%) when performing the specific IgE to latex and (24%) to the cutaneous test with antigen to total latex. This data allows us to continue evaluating the personnel at risk at the hospital, with better results in the administration of Labor Medicine at this medical institution.

bcl-2 Expression and apoptosis in primary and metastatic breast carcinomas.

OBJECTIVES: To evaluate the role of bcl-2 and apoptotic index in the progression from primary to metastatic breast carcinoma and their influence on prognosis. METHODS: bcl-2 expression was examined by immunohistochemistry and apoptotic index by in situ end-labelling in 116 surgical breast carcinomas and lymph node metastases from 50 patients. RESULTS: bcl-2 was observed in 69 cases (59.4%) of primitive carcinomas and 26 cases (65%) of metastatic breast carcinomas and there was agreement of bcl-2 expression between primary and metastatic sites except in 3 cases. bcl-2 expression was significantly associated with several favourable prognostic features, such as small tumour size (p = 0.03) and oestrogen and progesterone-receptor positivity (p < 0.01 and p < 0.001, respectively). A high apoptotic index was significantly associated with a number of poor prognostic factors, including poorly differentiated carcinomas, large tumour size, high Ki67 expression and high mitotic count (p < 0.001 in all cases). The mean apoptotic index was higher in lymph node metastasis than in primary carcinomas (1.19 vs. 0.69, p < 0.01). A low bcl-2 expression and a high apoptotic index were significantly associated with short-relapse free survival rates (p = 0.02 and p < 0.01, respectively), but only apoptotic extent provided independent prognostic information by multivariate analysis. CONCLUSIONS: The evaluation of bcl-2 expression and extent of apoptosis may provide useful prognostic information on breast cancer patients; however while increased apoptosis is strongly associated with the progression from primary carcinomas to lymph node metastases, bcl-2 does not seem to play a significant role in this process.

Evaluation of two mycotoxin binders to reduce toxicity of broiler diets containing ochratoxin A and T-2 toxin contaminated grain.

In order to assess ochratoxin A (OA) and T-2 toxin (T-2) binding ability of two commercial sorbents, both in vitro and in vivo trials with broilers were performed. Crude OA and T-2 extracts from contaminated grain were used to assess in vitro binding ability of two sorbents (Zeotek [Zk] and Mycofix [Mx]), by quantifying free mycotoxin through an enzyme-linked immunosorbent assay (ELISA) test. For in vivo trial, a 3 x 2 x 2 factorial arrangement was used for this experiment, being the factors: adsorbents (none, Zk, and Mx), OA (0 and 567 parts per billion [ppb]) and T-2 (0 and 927 ppb). OA and T-2 contaminated wheat and corn, respectively, were added to sorghum-soybean meal diets to meet 567 ppb of OA and 927 ppb of T-2. Mycotoxins were fed alone or combined in treatments. After 21 days, blood chemistry, gross, and histological evaluations were performed. Relative weights of liver, kidney, and bursa of Fabricius were obtained. Zk had the highest OA and T-2 in vitro binding ability (100% and 8.67%, respectively). Chickens fed OA with or without sorbents had a lower body weight and feed intake reduction. However, those birds fed T-2 were partly protected by a sorbent. Birds fed both toxins showed toxic additive effects, and no protection of any adsorbent was observed. A significant reduction in plasma proteins, albumin, and globulins was a characteristic observed in all birds fed diets with OA both with or without adsorbents. Uric acid level in blood was increased in all chickens fed OA-contaminated diets. Histological findings observed in birds fed OA-contaminated diets were necrosis of kidney tubular cells, swollen and necrotic hepatocytes, bile ducts hyperplasia, and increased diameter of proventriculus glands. In birds that received T-2 alone, only the liver, with the same kind of lesions, was affected. According to these results, it can be concluded that there is not a relation between in vitro and in vivo trials. OA toxic effects could not be counteracted by any sorbent. T-2 toxicity could be partially counteracted by an adsorbent used in this research.

Susceptibility to retinopathy of prematurity in Alaskan Natives.

Threshold retinopathy of prematurity occurred in 11 of 34 Alaskan natives compared with 10 of 93 non-natives. Natives constitute 16% of the state population. This significant Alaskan native preponderance was not explained by differences in prenatal or intensive care unit morbidity except that the intervals from birth to extubation and birth to cryotherapy were shorter for natives.

[Hepatic adenoma rupture as a cause of bleeding in the third trimester of pregnancy: report of a case and review of the literature].

Anemia and gynecological hemorrhage are common during pregnancy. For this reason an unusual cause of bleeding can pass unnoticed, and a late diagnosis is an important cause of maternal and fetal morbidity and mortality. In this case report we describe the rupture of am hepatic adenoma as cause of abdominal hemorrhage in adolescent pregnant during the labor. The patient didn't have an antecedent of oral contraceptives use; but a has a history of previous spontaneous abortion.

A lymphoproliferative disease in Japanese Quail.

A naturally occurring lymphoproliferative disease was found in 3 flocks of Japanese quail in Mexico. The tumorlike lesions were detected mainly in livers and spleens. Histopathology revealed a pleomorphic lymphoproliferative infiltration with dark-staining large cells, lymphocytes of different sizes, and reticulumlike cells. Marek's disease (MD) herpesvirus was not isolated from 74 quail tested, nor were antibodies to MD detected in 84 sera. Antibodies to reticuloendotheliosis (3/24) and avain leukosis virus subgroup A (2/24) were present. The possible etiology of this disease is discussed.

[Epithelial tumors of the upper urinary tract].

The paper refers to thirteen patients with epithelial tumours of the upper urinary tract, diagnosed and treated at our unit during the last five years and emphasizes the need of early diagnosis, custumorized therapy and close follow-up. Evolution is clearly more favourable in well differentiated and low stage tumours, regardless the type of surgery performed, versus the poor results obtained in infiltrative and undifferentiated cases for which there is no effective treatment available. Results are analyzed based on a brief literature review and considering the short number of cases contributed.

[Urethral diverticulum in women. Echographic diagnosis].

Review of clinical, radiological and ecographical data observed in 11 cases of diverticulum of urethra in women. Mictional cystourethrography was performed to all patients and acography through suprapubic route to 7 of them. The results from the radiological study and ecography findings are discussed; also the differential diagnosis with other masses in the same location.

[Nephrogenic adenoma in urethral diverticulum in women].

Nephrogenic adenoma is a benign metaplasic lesion of uncertain aetiology. The tenth case of this curious entity in a diverticulum of urethra in women is presented here. The probable factors which intervene in its pathogenesis are discussed in this paper, including a short review of the literature.

[In situ tumor excision of a renal neoplasm].

The present work reviews the conservative surgical options to treat renal tumor formation in patients who still have both kidneys, and tries to clarify the diagnostic aspects, indications and therapeutical alternatives. The controversial items are analyzed versus treatments advocating a radical surgical approach, establishing a parallelism with current trends of surgical non-intervention in renal traumatism of moderate to severe condition (grade II). A case is presented where these two pathological circumstances are concurrent: Renal traumatism with complete parenchymatous rexhis and contralateral solid renal mass, both treated conservatively. Pre-surgical diagnosis was left renal traumatism grade II and righ renal neoformation and was based in findings with CAT and arteriographia. Histology of the piece from tumorectomy was renal angiomyolipoma. After a post-surgical evolution of twelve months, therapeutical results were assessed as excellent, with no evidence of either immediate not late complications. Considering data recently communicated by most authors, the concept of simple tumorectomy or renal tumoral enucleation is suggested as valid. However, arguments both clinical and surgical, in order to confirm its indication, require very rigorous criteria and a careful selection of patients.

[Syringocele of Cowper's gland].

The dilatation of the duct of Cowper's gland is known as Cowper's Syringocele, a rarely occurring pathology, usually of a congenital origin. We offer two new cases corresponding to a perforated Syringocele and a non-perforated Syringocele. We analyse the clinical, radiological and therapeutic aspects of this entity.

[Response of thyrotropin (TSH) to the stimulation with hypothalamic releasing factor (TRF) in hypothyroid children].

The normal human serum thyrotropin (TSH) measured by radioimmunoassay in 36 children was 3.3 +/- 1.13 muUI/ml. No differences dependent on age or sex were found. Stimulation by intravenous injection of 100 mug of TRF has been carried out in 7 euthyroid children and in 14 with hypothyroidism. TSH was determined prior to, and during TRF stimulation. Among other thyroid tests included in this study were: serum T3 uptake, total thyroxine, serum protein-bound iodine, 131INa uptake and thyroid scanning. In the euthyroid children, TSH at zero time was 4.4 muUI/ml.; a significant increase was reached at 30-40 minutes. In 13 children with a light primary hypothyroidism, TSH was little increased in correlation with other parameters assessed. In two children with true primary hypothyroidism, TSH at zero time was X 465 muUI/ml., showing an increase at 30 minutes of X 550 muUL/ml. which was in agreement with the absence of thyroid tissue, showed by the scanning image and with the low concentration of thyroid hormones. The synthetic thyrotropin-releasing factor (TRF) should be a useful agent for clinical assessment of pituitary reserve. TSH, T3, T4 and T4I serum concentration measurement in pediatric age, makes possible to evaluate the physiopathology of the pituitary - thyroid axis.
