Corneal melting in a chronic alcoholic contact lens wearer.

PURPOSE: We report a case of bilateral keratitis in a 43-year-old alcoholic patient who wore soft daily wear contact lenses. METHODS: After consecutive negative cultures of corneal and conjunctival scrapings and after observing no improvement following antibiotic therapy, the patient was clinically diagnosed with vitamin A deficiency. RESULTS: After 20 days of treatment with vitamin A, the patient's cornea completely healed. Laboratory results confirmed the clinical diagnosis. CONCLUSIONS: When the ophthalmologist is confronted with corneal melting in a known alcoholic or drug-user, it is imperative to consider the possibility of vitamin A deficiency as a possible cause, especially in the contact lens wearer.

[Tuberculous choroiditis].

Tuberculosis is a rare cause of choroiditis. In most cases there is only a presumptive diagnosis based on a history of TB, its clinical picture, and a positive skin test. A 79-year-o;d man complained of acute, right visual loss. On examination, choroiditis was found. During his 20s he had had pulmonary tuberculosis; his Mantoux test was positive.

Pseudotumor cerebri induced by vitamin A combined with minocycline.

A 16-year-old girl complained about a headache of one-month's duration, accompanied by vertical diplopia that had appeared ten days earlier. The girl reported receiving vitamin A and minocycline to treat acne vulgaris for the previous six weeks. An examination revealed bilateral optic disc edema. Normal computed tomographic and magnetic resonance imaging examinations enabled a diagnosis of pseudotumor cerebri to be made. Soon after discontinuation of those medications, the headaches and diplopia diminished. We suggest a periodic ophthalmologic examination during systemic therapy with vitamin A combined with minocycline to detect the early occurrence of pseudotumor cerebri.

Primary lymphoplasmacytoma of the conjunctiva.

We report the case of an 80-year-old patient with isolated lymphoplasmacytoma of the conjunctiva. Only five other such cases have been previously reported in the literature. In all six patients, the disease remained limited and no sign of systemic disease could be found after prolonged follow-up. As isolated plasmacytoma of the conjunctiva seems to be benign, treatment of such tumours should be conservative.

Central retinal-artery occlusion in a patient with circulating lupus anticoagulant.

The circulating lupus anticoagulant is a spontaneously acquired immunoglobulin that has been associated in vivo with an increased risk of thrombotic episodes. We describe the case of a young woman with central retinal-artery occlusion associated with circulating lupus anticoagulant in the absence of systemic lupus erythematosus or any other autoimmune or collagen vascular disease. In cases of central retinal-artery occlusion without apparent cause, the presence of the lupus anticoagulant should be excluded especially in cases that have been exposed to the human immunodeficiency virus.
