[Cardiogenic ischemic strokes: pathogenetic aspects].

Eighty-three patients with acute ischemic stroke (56 men and 27 women, mean age 90.1 +/- 10.8 years) were examined in order to elucidate the significance of paroxysmal heart rhythm disorders and silent myocardial ischemia and determine the pattern of hemorheological changes. Traditional clinical instrumental examinations were supplemented by Holter monitoring and measurements of a wide spectrum of hemostatic and hemorheological values. Cardiogenic ischemic strokes were pathogenetically heterogeneous. Holter monitoring helped detect the significance of paroxysmal atrial fibrillation in the pathogenesis of embolic cardiogenic stroke. One of the key factors in development of hemodynamic cardiogenic stroke was transitory bradyarrhythmia and deterioration of left-ventricular contractility. Cardiogenic strokes are associated with hemostatic activation with predominant changes in the plasma hemostasis, which dictates purposeful hemocorrection.

[Prevention of postoperative infectious complications with suture materials containing gentamicin].

Sutural materials containing antibiotics such as gentamicin and tetracycline were studied on dogs with respect to the action on development of inflammation reactions after operations on the large intestine. It was shown that the antibacterial surgical threads had a prophylactic action and inhibited infection development. There was noted a significant advantage of the threads over the control ones not containing the antibiotics. The inflammatory reactions were less intensive and of shorter duration. The terms of onset and completion of the reparation also markedly shortened.

[The content of cholylglycine in the blood serum in cholelithiasis during treatment with litholytic preparations of bile acids].

Liver function has been assessed on the basis of radioimmunoassays of the primary conjugated cholic acid, cholylglycine, in the blood sera of cholelithiasis patients treated with cheno- and ursodeoxycholic acid drugs for a long time. The findings indicate that the blood serum cholylglycine levels may be regarded, among other things, as a prognostic criterion of the efficacy of cholelithiasis therapy with bile acid preparations for litholysis.

[Bile excretion system in endogenous hypercorticism syndrome].

Bile excretion and biochemical properties of bile were investigated in 86 patients with the endogenic hypercorticism syndrome, 78 patients with the Itsenko-Cushing disease and in 8 patients with corticosteroma. A considerable incidence of functional and organic biliary disorders was demonstrated. Changes in cholecyst contractility were recorded in 54.6% of the patients, with hypermotor dyskinesia (36.4%) prevailing. An excess of endogenic corticosteroids was frequently accompanied by a hypotonus of Oddi's sphincter (65.6%) or Lutkens' sphincter (67.2%). Chronic (stoneless) cholecystitis was diagnosed in 70.9% of the patients, cholelithiasis, in 16.3%, and cholecyst cholesterosis, in 3.5%. Adrenocortical hyperfunction was associated with natural changes in biochemical composition of the bile, such as excessive cholesterol and a relative deficiency of cholic acid and phospholipids which might be conducive to cholelithiasis. High incidence of biliary disturbances and their pattern should be taken into account in the treatment of patients with hypercorticism.

[Experimental research on the effect of ultraviolet irradiation on the phosphorus-calcium metabolic status].

The influence of vitamin D and ultraviolet irradiation (UVI), used for prevention of vitamin D deficiency, on the state of phosphoric-calcium metabolism was studied in experiments on rats. It has been shown that daily injections of 1-5 IU of vitamin D, and UVI in biodoses from 1/8 to 1/4 promote the maintainance of the normal level of phosphoric-calcium metabolism. When UVI was applied according to the widely used "basic scheme" (in biodoses from 1/4 to 2 1/2) the following anomalies were recorded: a three-fold increase of 25-ON-D3 concentration in the blood plasma, hyperphosphatemia, a tendency to hypercalcemia. The use of higher doses of UVI led to a further increase of 25-OH-D3 level, hypophosphatemia, hypercalcemia, kidney and heart calcification. The data obtained have evidenced a possibility of vitamin D intoxication during UVI.
