Diverticular disease in an indigenous African community.

Sixteen cases of diverticular disease of the colon occurring in an urban hospital in Ghana are described. Ten of the patients had diffuse diverticulosis affecting the greater part of the colon. Apart from a high incidence of presentation with rectal bleeding the disease shows no particular differences in manifestation from what is encountered in the advanced countries. Sixty per cent of the patients belonged to the higher social classes, but all had lived on traditional African high-residue food. Other factors as yet unknown may contribute to the pathogenesis of this disorder.

Gastrointestinal replacement solution.

The mean sodium concentration of 61 gastrointestinal aspirations in peritonitis was 103 mEq/l. (S.D. 16.66), of 16 aspirations in vagotomy 88 mEq/l. (S.D. 19.49), of 12 aspirations in perforated duodenal ulcer 81 mEq/l. (S.D. 19.49), and of 15 aspirations in intestinal obstruction 89 mEq/l. (S.D. 19.49). The mean potassium concentrations were 9, 9.9, 13, and 8.8 mEq/l. respectively, and the mean chloride concentrations 122, 131, 125, and 112 mEq/l. respectively. It is suggested that a gastrointestinal replacement solution should contain 100 mEq of sodium, 12 mEq of potassium, and 122 mEq of chloride per litre; 50 g. of dextrose or 100 g. of fructose may be added to provide energy.

Trial of Badoe's maintenance solution.

The adult West African on intravenous fluid therapy requires 130 mmol of sodium, 50 mmol of potassium and 3L of water daily. At least 100g of glucose is usually provided. Three liters of Badoe's Maintenance Solution which contains 43.3 mmol of sodium of sodium, 16 mmol of potassium, 0.65 mmol of calcium, 9 mmol of bicarbonate, 51.7 mmol of chloride and 100g of sorbitol per litre, provides all the daily requirements of sodium, potassium and water. It also provides about 5000j of energy, nearly two-thirds of the resting energy expenditure. The trial in seven ill patients from the first day after operation has shown that the serum concentration of sodium and potassium and the urinary output of patients on 3L of Badoe's Maintenance Solution alone daily remain normal. Badoe's Maintenance solution is, therefore, recommended as the standard maintenance solution for patients in balance who need intravenous fluid therapy. It simplifies fluid therapy for medical and nursing staff as only one solution is used.
