Synovial hemangioma--a rare benign synovial tumor. Report of four cases.

We report our clinical experiences in the diagnosis and treatment of four patients with synovial hemangiomata. Synovial hemangiomata are rare causes of recurrent, nonspecific joint complaints and occur most often at the knee joint. Nontraumatic joint swelling combined with recurrent, possibly hemorrhagic joint effusions must be considered signs of a synovial hemangioma. Although no preoperative diagnostic tool enables confirmation of the diagnosis, and MRI seems to be the diagnostic procedure of first choice, it is most important fact for the clinician to be aware of the existence of this disease. Early surgical treatment with excision of the tumor within wide margins of noninvolved normal synovial tissue as partial or total synovectomy is the therapy of choice and avoids degenerative changes as demonstrated with the four cases reported here.

[Chronic impingement syndrome of the shoulder].

The most common manifestation of periarthritis of the shoulder is the chronic impingement syndrome, which represents an "entrapment situation". When the arm is raised, the supraspinatus tendon that forms part of the rotator cuff, comes into contact with the coracoacromial arch. The complains can often be improved by conservative treatment. If, however, the problem persists--in about 10 to 20% of the cases--surgical decompression of the coracoacromial arch by resecting the coracoacromial ligament and the subacromial bursa is indicated. This limited intervention carries only a low risk and, as studies have shown, has such a high rate of success that, in the individual case, the indication for surgery can be established earlier than has so far been assumed.

[Therapeutic concept of the flexion corset in a chronic lumbar syndrome and sciatica].

Lumbar flexion and reducing lumbar lordosis or hyperlordosis are basic principles in the treatment of chronic or recurrent low back syndrome, lumbar insufficiency and sciatica. This can be achieved by the Hamburger Flexion Corset, which is built by moduls. The front part of the brace exerts, compression on the abdomen by a deep imprint, so that the lumbar spine will be gradually transformed.

[Osteomyelitis after BCG-vaccination (author's transl)].

BCG-osteomyelitis is a rare complication of BCG-vaccination. The diagnosis is difficult and undiagnosed cases can be assumed. After adequate treatment a benign course is typical. There is quite a difference of morbidity in various European countries. Possible reasons are discussed.

[Radiographic signs of acetabular dysplasia of the adult hip (author's transl)].

Many papers have been published about the different types of measurements used in the evaluation of hip dysplasia in the pediatric age group. However, there are no publications that deal with this topic regarding adults. The authors present an initial series of measurements of the angle of the roof of the acetabulum in 100 randomized asymptomatic adult males. The normal value ranged from -10 degrees to +10 degrees. These measurements were correlated with those described by Wiberg. The combination of both methods yields highly reliable information in the evaluation of hip dysplasia.

[Hip dysplasia and congenital hip dislocation - a roentgenometric study in 110 families (author's transl)].

In human genetics and orthopedics quite different answers have been given to the question of hereditary transmission and frequency of hip dysplasia in families of children with congenital hip dislocation. We therefore have made roentgenometric measurements of 110 parents of children with congenital hip dislocation. In 25% we found abnormal flat acetabulae, whereas 12% had pathologic deep hips. This may propose a new concept of morphology of congenital hip dysplasia.

[Osteonecrosis morbus Friedrich].

This report presents in detail the aseptic bone necrosis of the sternal end of the clavicle, the so-called M. Friedrich. When pain persists this relatively rare illness can be operated successfully.

[Aseptic necrosis of head of femur and disturbance of fat-metabolism type IIa after Fredrickson (author's transl)].

The history of aseptic necrosis of the head of femur is given. Following publication of osteonecrosis in hyperlipoproteinemia type IV after Fredrickson, now for the first time the relation between aseptic necrosis of the femoral head and the primary disturbance of fat metabolism type II is revealed. There exists a hypercholesterinemia, more exactly a hyperbeta-cholesterinemia. The subgroups type IIa and type IIb are explained. Type II is of particular importance since it produces early arteriosclerosis which demands early diagnosis, since this condition can be treated by drugs and diet. In the writers' opinion further differentiation of dys-and hyperlipidemiae in aseptic necrosis of the femoral head will enable us to define signs of disturbances of lipid metabolism. The group of "idiopathic" necroses of the femoral head will become steadily smaller. According to this basic concept the hypothesis might be put forward that this type of aseptic osteonecrosis is a late complication of a differentiated fat-metabolic disturbance.

[Clinic of the upper cervical spine and review of the medical history of the regional osteology (author's transl)].

After a brief review of the systematic clinical work up of this region within our century, the medical history of the osteology of the occipito-cervical junction is given special attention. It is looked upon the illustrations of the first printed anatomical textbooks that are linked with such famous names as Berengario, Dryander, Estienne, Vesal and Eustachius. Leonardo da Vinci's concept of the first two vertebrae is also mentioned.
