[Evaluation of the usefulness of palpation and Daniel's biopsy for detection of metastasis to supraclavicular and scalene lymph nodes in patients with lung neoplasms].

In 169 patients with lung cancer the usefulness of physical examination and Daniel's biopsy in detection of metastases to supraclavicular and scalene lymph nodes were evaluated. In 35 patients with palpable lymph nodes and in 25 patients with nonpalpable lymph nodes metastases were confirmed by histological examination. The sensitivity, the specificity and the likelihood ratio of physical examination in detection lymph nodes metastases were respectively: 58%, 86% and 4.1. In 6 patients (5%) among 119 with nonpalpable lymph nodes the positive Daniel's biopsy was the diagnostic procedure. In 5 patients with non-small carcinoma and in 3 patients with malignant mesothelioma positive results of Daniel's biopsy enabled a change of cancer staging and giving up the eventual surgical operation.

[Lung lipoproteinosis--diagnostic problems].

Pulmonary alveolar proteinosis is a very rare disease, first described by Rosen in 1958. It is characterized by the accumulation of lipoproteins in the pulmonary alveoli. Clinical and radiologic presentations are typical for the interstitial lung disease. Only few cases of this disease have been described in the Polish literature. A patient with disseminated lesions to the lungs is presented. A diagnosis was possible after an analysis of BAL fluid and histological examination of the lung tissue collected by the transbronchial biopsy.

[Pulmonary function tests and bronchoalveolar lavage in sarcoidosis patients with 1changes and without changes in high resolution computed tomography].

In 20 sarcoidosis patients with I0 of radiological changes the high resolution computed tomography (HRCT), pulmonary function tests (spirometry, diffusion capacity, static compliance) and broncho-alveolar lavage (BAL) were performed. In 8 patients with pathological changes detected in HRCT and in 12 patients without HRCT changes the similar pulmonary function disturbances and BAL-cell count were observed.

[Effect of salbutamol inhalation on small airways function in patients with sarcoidosis].

The effect of Salbutamol (Polfa) inhalation on small airway function was studied in 20 patients with stage II sarcoidosis and 8 healthy volunteers. MEFV values and dynamic compliance were assessed. A significant statistical increase of FEV1, MEF50, Cdyn20 and Cdyn60 were seen only in healthy volunteers. In patients with sarcoidosis the assessed parameters were not effected by the Salbutamol inhalation.

[Mediastinal seminoma--diagnostic difficulties].

A case report of a seminoma of extragonadal localization--of the mediastinum--is presented. In specimens obtained during mediastinoscopy sarcoid lesions were detected. Parasternal mediastinotomy allowed to sample larger fragment of the tumor and the final diagnosis was made.

Evaluation of upper airway obstruction after partial laryngectomies by radiological method and flow-volume loop analysis.

Anatomical and functional estimations of the upper airways in patients after partial laryngectomies (cordectomy, hemilaryngectomy, enlarged hemilaryngectomy) carried out due to cancer are discussed in this paper. The post-operative lumen of the larynx and the trachea were estimated by radiological examination. The coefficient larynx/trachea (L/T) was proposed to describe fixed obstruction. At the same time, all patients underwent spirometric examinations. Inspiratory and expiratory parameters of the flow-volume loop were evaluated. In 39 patients the L/T coefficient was lower than in a group of patients with chronic bronchitis (p < 0.05). Also inspiratory and some expiratory parameters of the flow-volume loop decreased in contrast to the group with chronic bronchitis. All results showed the usefulness of radiological and spirometric methods in detecting upper airway obstructions and confirmed their fixed character. The influence of the area of operation on the degree of upper airway obstruction was emphasized.

[Lung fibrosis after bleomycin therapy].

A case of lung fibrosis is presented in a 44 year old male in whom interstitial pulmonary changes were observed in the course of bleomycin therapy of malignant granulomatosis. Following introduction of corticosteroids a radiological, clinical and functional improvement was observed. The functional changes were analyzed during the 7 month period of treatment. This case suggests that post bleomycin induced injury can be reversed with appropriate high doses of corticosteroids.

[Effect of bronchoalveolar lavage (BAL) on gasometric and spirometric parameters in patients with sarcoidosis].

The effect of BAL and fiberoptic bronchoscopy on gasometric and spirometric parameters was studied in 22 patients with sarcoidosis and 15 volunteers. In 12 patients with sarcoidosis during the repeated BAL procedure the effect of oxygen supplementation (by nasal prongs 3 L/min) was analysed. Significant decrease of oxygen pressure (PaO2) and vital capacity was seen in patients undergoing BAL and fiberoptic bronchoscopy. The administration of oxygen during the BAL procedure prevented the fall in oxygen pressured.

[Levels of endothelin-1 in bronchoalveolar lavage fluid of patients with bronchial asthma].

UNLABELLED: To evaluate the role of endothelin in the pathogenesis of bronchial asthma endothelin-1 levels were studied in the bronchoalveolar lavage fluid of symptomatic (n = 7) and asymptomatic (n = 10) asthmatic patients and healthy volunteers (n = 6). In all patients BAL fluid levels of endothelin-1 was assessed by RIA, total protein concentration by Lowry method. Patients with symptomatic asthma had increased amount of ET-1 in BAL fluid than asymptomatic asthmatics and control group. CONCLUSIONS: 1. Presence of endothelin in BAL fluid indicate that this peptide is involved in the pathogenesis of bronchial asthma. 2. Endothelin-1 BAL fluid levels may correlate with the severity of bronchial asthma.

[Usefulness of peak expiratory flow measurements in assessing degree of bronchial obstruction in patients with chronic obstructive lung diseases].

An analysis was carried of relations between PEF measurements and FEV1 values and between PEF and FEF25-75 in 130 patients with COLD (48 females and 82 males). PEF was assessed using the Wright peak flow meter, while FEV1 and FEF25-75 from the spirographic curve. A positive linear correlation was found between PEF and FEV1 in absolute values, % predicted and standardised residuals (r = 0.91, r = 0.87, p < 0.001). Diagnostic sensitivity of PEF in detecting bronchial obstruction was 82%, while the specificity was 85%. The weakest correlation was found between PEF and FEV1 in standardised residuals of 41 patients with PEF between 50% and 79% predicted (r = 0.41, p < 0.05). In this subgroup of patients, twenty one (51%) had normal FEV1, and twenty (49%) low FEV1 values. Measurements of PEF despite a strong correlation with FEF25-75 did not allow to single out cases without small airway involvement, because in 28% of the patients with PEF 100% predicted lowered FEF25-75 were noted. In 97% of patients with PEF values 80% predicted abnormal low FEF25-75 were demonstrable.

[Bronchoalveolar lavage (BAL) in patients with pulmonary sarcoidosis. I. Correlations between cellular morphology of BAL with radiologic and respiratory function tests].

BAL, spirometric evaluation and compliance values were analysed in 51 patients (21 females, 30 males; age range 19-51 years). Basing on BAL criteria, 19 had active sarcoidosis, 32 non-active sarcoidosis. Radiological stages (I, II, III) were found in similar percentages in both forms of the disease. Mean respiratory function parameters were similar in the active and non-active stage of the disease. The most common abnormality found in 33% of the patients was decreased DLCO.

[Bronchoalveolar lavage (BAL) in pulmonary sarcoidosis. II. Changes in BAL cellular morphology and respiratory function in patients with sarcoidosis treated with prednisone].

BAL and respiratory function were analysed in 26 patients with sarcoidosis (age range 22-48 years) before and after a 12 month steroid therapy trial. 11 patients were in the active stage according to the BAL criteria. After treatment a significant decrease in BAL lymphocytosis was found (a decrease from 31.4% to 22.8%). A significant increase in TLC, DLCOSB and D/VA mean values was found. The effect of steroid therapy was similar in the active and non-active stage of the disease. The authors found BAL useful in monitoring of therapy.

[Pulmonary function changes as a late diabetic complications].

During diabetic process angiopathy of lung vessels may be expected. In the study authors evaluated values of spirometric and gasometric parameters in patients with diabetes mellitus in two groups--with and without diabetic complications. It was documented that diabetic process caused the decrease of pulmonary diffusion volume and some gasometric disturbances. This above mentioned spirometric and gasometric changes indicated some coexistence with a late diabetic complications.

[Chronic bronchitis and efficiency of lung ventilation in workers of the "Zawadzkie" foundry].

The aim of this study was to determine the influence of smoking habit and occupational exposure to air pollution on the prevalence of chronic bronchitis and lung function efficiency in foundry workers. MRC survey and spirographic measurements were performed in 917 men aged 18-62 years (mean age 38.6). In 455 men (49.6%), occupational exposure to air pollution significantly exceeded the permissible values. In 122 men (13.3%) the exposure was low. Chronic bronchitis was more than twice as frequent in smokers (27.3%) as in nonsmokers (11.0). Cessation of smoking led to an improvement in the symptoms of chronic bronchitis. Chronic bronchitis was slightly more frequent in highly exposed (25.5%) as compared with low-exposed workers (15.6%). Age-related lung function decline was significantly greater in smokers than in nonsmokers. Unlike chronic bronchitis, lung function was still lower after smoking cessation. Among highly exposed workers, age-related FVC and FEV1 declines were significantly larger in smokers than in non-smokers.

Pulmonary complications of type 1 (insulin-dependent) diabetic patients.

We have investigated the influence of diabetes mellitus including the presence of late complications of the pulmonary system. To check this relationship 31 Type 1 (insulin-dependent) diabetic patients (mean age 30.6 +/- 5.32 years, mean duration of diabetes 12.9 +/- 5.05 years) were admitted into the trial and compared with 18 control subjects. Pulmonary function tests were measured including spirometric parameters, diffusing capacity, specific diffusing capacity and dynamic compliance measured at 20 and 60 breaths per min. No disturbance of the spirometric parameters was observed in the diabetic patients. Diffusing capacity in the diabetic patients with complications was significantly lower than in both the diabetic patients without complications and the control group (81.2 +/- 16.2%, 104 +/- 13.7%, 99.3 +/- 2.8%; p < 0.001, p < 0.005 respectively). Specific diffusing capacity was significantly lower in the diabetic patients than in the control subjects (80.3 +/- 13.1% vs 89.4 +/- 12.9%; p < 0.05). In the group with late complications specific diffusing capacity was lower than in the group without complications (69.7 +/- 9.17%; 87.2 +/- 10.7%, respectively; p < 0.001). Dynamic compliance at 20 breaths per min in diabetic patients was 84.06 +/- 17.08% vs 95.2 +/- 11.59% in the control subjects (p < 0.05). It was particularly low in the group with late complications 80.6 +/- 13.2% and patients with metabolic poor control, 80.3 +/- 12.02% (both p < 0.005 vs the control group). Dynamic compliance at 60 breaths per min was 60.1 +/- 15.0% as compared to 83.2 +/- 13.3% in the control group (p < 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Changes in the parameters of respiratory function tests in Raynaud's disease].

Patients with Raynaud's phenomenon can demonstrate arterial spasm not only in hands but also in other organs. The aim of this study was to investigate pulmonary function in 15 patients with Raynaud's disease and in 8 healthy controls before and after cold pressor test. Before the cold pressor test no significant differences in pulmonary parameters between the patients and the controls were observed. After cold pressor test, a statistically significant decrease in diffusing capacity (DLCO) was observed in the study group, but not in the controls. Total lung capacity (TLC) was also slightly decreased. No significant changes of specific diffusing capacity (D/VA) and flow-volume curve parameters (FEF50, FEF25) were noticed.

[Cellular bronchoalveolar lavage (BAL) components and pulmonary function in chronic extrinsic allergic alveolitis].

Bronchoalveolar lavage (BAL) and pulmonary function tests were carried out in 22 patients with chronic, extrinsic allergic alveolitis. In all a marked decrease of diffusion capacity was observed. The mean DLCO values was 34% predicted value. In 94% of the patients compliance was lowered. More pronounced abnormalities of pulmonary function (decrease of TLC and DLCO) were found with low BAL lymphocyte counts and increased macrophages count. The duration of the disease correlated with the absolute lymphocyte BAL count (r = -0.43). The BAL differential can be an additional measure of severity, duration of the disease process in chronic, extrinsic allergic alveolitis.
