The prognostic significance of metastatic cervical lymph nodes.

The ability to predict accurately the clinical course of a patient with a malignancy is critically important to the patient's subsequent management. It has been well documented that the presence of metastatic nodal disease is associated with decreased patient survival. Survival data from a group of 242 head and neck cancer patients from the University of Iowa were analyzed to determine the significance of specific characteristics of metastatic lymph nodes. Evaluation of absolute numbers and percentages of positive nodes or node size generally was not useful. However, involvement of the posterior triangle nodes, noncontiguous nodal sites, or multiple sites was associated with a worse prognosis. When that was accurate enough to be of help to the clinician in prognostication. The single most important feature seems to be documentation of the presence of metastatic nodal disease, rather than particular features (i.e., number, size) of the metastatic nodes.

Tissue distribution of copper-labeled 3-ethoxy-2-oxobutyraldehyde bis (thiosemicarbazone) (Cu-64 KTS) in mice and rats: concise communication.

The antitumor activity of, 3-ethoxy-2-oxobutyraldehyde bis (thiosemicarbazone) (KTS), is related to the presence of copper(II) ion. We have studied the tissue distribution of Cu-64-labeled KTS in rats and mice carrying transplanted tumors to evaluate whether the uptake of the radioactivity in the tumor is adequate to warrant further investigation of the tracer as a tumor-seeking agent in patients. Four groups of three of four animals each were studied: (a) mice with fibrosarcoma; (b) mice with mammary adenocarcinoma; (c) rats with fibrosarcoma; and (d) rats with squamous cell carcinoma of the lung. The animals were killed at intervals of 0.25, 1, 4, 24, and 48 hr after i.v. injection of 1.6 X 10(-3) M Cu.KTS containing 3 to 18 muCi Cu-64. Blood, tumor, and six to ten additional tissues were counted for radioactivity. The mouse fibrosarcoma concentrated Cu-64, reaching 15% of the administered dose/g at 48 hr after injection. This suggests that for tumor scanning, the 61.7-hr Cu-67 might be more suitable as a label for KTS than the 12.7-hr Cu-64.

HLA polymorphism in Israel. 8. The Armenian community in Jerusalem.

Gene and phenotype frequencies for the three HLA loci were determined for 90 random Armenians from the Old City of Jerusalem. Relatively high frequencies were found for HLA-A2, A3 and Aw24, while the lowest frequencies were those for Aw23 and Aw25. The major distinguishing antigen, when compared with European Caucasoids is Bw35 with a frequency of 22%. Two unusual associations between the B and C loci were observed: Bw 17, Cw3 and B7, Cw3. Another interesting association in Armenians, Aw32 Cw4, has previously been found only among Cochini Jews and Malay populations.

A new syndrome involving cleft palate, cardiac anomalies, typical facies, and learning disabilities: velo-cardio-facial syndrome.

This report describes a pattern of similarities among 12 patients which are felt to represent a newly recognized congenital malformation syndrome. The symptoms shown most consistently by the 12 patients were overt or submuscous clefts of the secondary palate, ventricular septal defects, typical facies, and learning disabilities. Other symptoms were noted with varying frequency. The occurrence of velopharyngeal insufficiency in all twelve patients reflected poor motion in the lateral pharyngeal walls, thus necessitating specific forms of treatment. Treatment was often dependent on the extent of cardiac lesions.

Oral cytembena absorption and phase I--II studies.

Cytembena is a nonmyelosuppressive drug that has received phase I--II trials in this country and is presently being used as a chemotherapeutic agent in Europe. Sterile abscesses at the site of im injection and "autonomic storm" after iv bolus represent the most frequent dose-limiting toxic effects, and intermittent schedules reflect reports of prolonged plasma levels of drug. This report of excellent oral absorption and a short half-life suggests that alternative routes and schedules of administration should be evaluated. The dose-limiting toxic effects observed after frequent oral administration were transient proteinuria and increased creatinine levels.

Cardiac pacing in children and adolescents.

Nineteen patients aged 1 month to 18 years underwent implantation of a cardiac pacemaker and were followed up for up to 9 years (average duration of pacing 54 months). Complete heart block was present in 16 patients and sinus nodal dysfunction in 3. Heart block was presumably of congenital orgin in eight, secendary to cardiac surgery in seven and subsequent to cardiac catheterization in one. Sinus nodal dysfunction was of presumed congenital origin in one and occurred after cardiac surgery in two. Pacing was required because of syncopal attacks in eight patients, three of whom had congestive heart failure or low cardiac output on physiologic studies. It was required in four because of congestive heart failure, in two because of low cardiac output (one with a wide QRS complex), and in five for postoperative rhythm control. With return of sinus rhythm after 2 and 3 months, respectively, pacing was discontinued in two patients. One child was partially corrected disease died within 3 months, one died of wound breakdown and sepsis after 10 months of pacing and one died suddenly 4 years after implantation. All others have returned to normal activity; only one requires cardiac medication. The degree of emotional stability has been striking. Asynchronous and atrial synchronous pacing are of equal therapeutic value. The very small radiofrequency implanted receiver has been useful in younger children. The major problems have been caused by the large size and short longevity of the generators and the child's growth stressing the lead system. Transvenously implanted pacemakers have presented no greater management problems than those placed during thoracotomy.

Symptomatic sinus arrest in a young girl.

Sinus node dysfunction, previously unreported in girls, occurred in a 13-year-old girl who required permanent pacemaker implantation because of recurrent syncopal attacks. In addition to periodic sinus arrest, the presence of significant atrioventricular conduction disturbance was also documented. Although she had had a persistent ductus arteriosus divided at an earlier age, the disturbance of rhythm and conduction cannot be ascribed to a congenitial or haemodynamic abnormality. An acquired origin can only be postulated.
