Short-latency vergence eye movements induced by radial optic flow in humans: dependence on ambient vergence level.

Radial patterns of optic flow, such as those experienced by moving observers who look in the direction of heading, evoke vergence eye movements at short latency. We have investigated the dependence of these responses on the ambient vergence level. Human subjects faced a large tangent screen onto which two identical random-dot patterns were back-projected. A system of crossed polarizers ensured that each eye saw only one of the patterns, with mirror galvanometers to control the horizontal positions of the images and hence the vergence angle between the two eyes. After converging the subject's eyes at one of several distances ranging from 16.7 cm to infinity, both patterns were replaced with new ones (using a system of shutters and two additional projectors) so as to simulate the radial flow associated with a sudden 4% change in viewing distance with the focus of expansion/contraction imaged in or very near both foveas. Radial-flow steps induced transient vergence at latencies of 80-100 ms, expansions causing increases in convergence and contractions the converse. Based on the change in vergence 90-140 ms after the onset of the steps, responses were proportional to the preexisting vergence angle (and hence would be expected to be inversely proportional to viewing distance under normal conditions). We suggest that this property assists the observer who wants to fixate ahead while passing through a visually cluttered area (e.g., a forest) and so wants to avoid making vergence responses to the optic flow created by the nearby objects in the periphery.

Functional Fas ligand expression in thyrocytes from patients with Graves' disease.

Fas/Fas ligand (FasL) interaction has been suggested to play a role in the pathogenesis of Hashimoto's thyroiditis. This manuscript addressed a role for Fas/FasL interaction in the pathogenesis of Graves' disease (GD). Apoptosis was detected in 0.5-5.0% of GD thyrocytes, but not in normal thyrocytes from patients with adenoma (N). Fas was constitutively expressed on the basement membrane of both GD and N thyrocytes. Thyrocytes expressed Bcl-2 constitutively in both GD and N thyrocytes. FasL was detected at the messenger ribonucleic acid level in thyroid tissue and cultured thyroid cells by Northern blotting and RT-PCR. FasL protein was detected in the cytoplasm and basolateral surface of thyrocytes from GD, but not in N. Cell surface expression of FasL on cultured thyrocytes disappeared within 48 h after their isolation. However, it was retained by culturing the cells with a matrix metalloproteinase inhibitor. Coculture with thyrocytes induced apoptosis of Fas transfectants, which was blocked by an anti-FasL antibody. Although cultured thyrocytes expressed Fas on the surface, they were not killed by an agonistic anti-Fas antibody. Interferon-gamma-induced Fas up-regulation was suppressed by TSH. These results suggest that the increased expression of FasL in GD thyrocytes, the down-regulation of Fas expression by TSH or possibly by TSH receptor autoantibody, and the overexpression of Bcl-2, which could render thyrocytes resistant to FasL-mediated elimination, may thus be involved in the pathogenesis of GD.

Reconstruction of the proximal interphalangeal joint collateral ligaments using the Mitek Micro Arc Anchor: an in vitro biomechanical assessment.

Proximal interphalangeal joint (PIP) collateral ligament injuries are common and sometimes result in painful, stiff fingers. A number of techniques have been utilized in the past to reconstruct complete collateral ligament injuries. Recently, bone anchor systems have been described that allow collateral ligament reconstruction without the necessity of pull-out wires and other more cumbersome methods to reconstruct these ligaments. The Mitek bone anchor system has been used successfully in a variety of anatomic locations throughout the body to reattach soft tissues to bone. The current study was conducted to evaluate the biomechanical effects of the use of the Mitek Micro Arc Anchor in collateral ligament injuries of the PIP joint. In cadaveric PIP joints, the collateral ligament was isolated and then transsected in its mid substance. The authors compared suturing the ligament alone in standard fashion to repair of the ligament using a bone anchor inserted in the base of the middle phalanx. The joints were stressed to failure, with a lateral load applied at the distal end of the middle phalanx on a materials testing machine. The mean tensile failure loads of the two groups were the following: Mitek anchor, 16.4 +/- 3.7 N; suture repair, 19.3 +/- 7.6 N. The authors found no significant difference in the force required for failure of the repair or in the pattern of failure in the two groups. The Mitek Micro Arc Anchor appears to be adequate biomechanically to reconstruct the collateral ligaments of the finger PIP joint.

Fluorescence spectral properties of the anticancer drug topotecan by steady-state and frequency domain fluorometry with one-photon and multi-photon excitation.

Topotecan is an antitumor agent with activity against a variety of cancers. We examined the steady-state and time-resolved fluorescence spectral properties of topotecan with one- and two-photon excitation. Topotecan was found to display a high two-photon cross section near 20 GM for wavelengths within the fundamental output of a Ti:sapphire laser, 800-880 nm. In frozen solution the anisotropies of topotecan are near the theoretical maxima for one-photon and two-photon excitation with colinear electronic transitions. The intensity and anisotropy decays of topotecan fluorescence were found to be homogeneous (single exponentials) in phosphate-buffered saline and propylene glycol. The steady-state and time-resolved data indicate that topotecan binds to a double-helical DNA oligomer d(AT)10 resulting in increased anisotropies and multiexponential intensity and anisotropy decays. Subnanosecond components in the anisotropy decay of the DNA-topotecan complex suggest loose binding of the drug to DNA. Loose binding of topotecan to DNA is also revealed by accessibility of topotecan to collisional quenching by iodide.

[The clinical and pathology study of HBV precore mutant infection in chronic HBV patients].

OBJECTIVE: To elucidate the clinical feature, pathological change and IFN therapy of HBV pre-c mutant(1,896 G-->A point mutation). METHODS: Mutation specific PCR was employed for detecting precore mutation from 108 serum and/or liver samples of patients with chronic hepatitis B infection. Liver damage was evaluated with Knodell's histology activity index(HAI) in 46 patients. RESULTS: HBV precore mutant existed popularly in different e systems status, but the detective rate of HBV precore mutant or mutant prevailing over wild type in HBeAb+ group(14.29%, 46.23%) was significantly higher than that in HBeAg+(0%, 6.45%)group. Mutant infection was associated with the severity of liver diseases, the detective rate in severe chronic hepatitis group was significantly higher(13.64%, 40.91%) than that in chronic asympotamatic carriers groupe(0%, 15.62%). 5 precore mutant positive patients with IFN treatment showed either response or nonresponse to IFN therapy. There was no significant difference of liver pathological change between mutant type and wild type infection in patients with mild chronic hepatitis. CONCLUSION: HBV precore mutant infection existed commonly in patients with chronic hepatitis B, despite of the e system status, though it was more prevalent in patients with HBeAb positive. Further study is needed in the pathogenicity and the treatment of HBV precore mutation.

[Inhibitory effect of IGF-II antisense RNA on malignant phenotype of hepatocellular carcinoma].

OBJECTIVE: Inhibitory effect of insulin like growth factor II (IGF-II) antisense RNA on malignant phenotype of hepatic cancer cells was studied. METHODS: A 0.1 kb cDNA of human IGF-II was reversely inserted into a eukaryotic expression vector of pcDNA3 and an IGF-II antisense RNA expression vector of pIGF-II As was obtained. The pIGF-II As was then introduced into human hepatic cancer cell line SMMC-7721. The effect of IGF-II antisense RNA which was expressed by pIGF-II As on SMMC-7721 cell cycles progression was measured with FCM. RESULTS: The coloning formation in anchorage-independent assay of SMMC-7721 cells with pIGF-II As was significantly decreased by comparison with control groups of SMMC-7721 cells and SMMC-7721 cells transfected with pcDNA3 vector alone. FCM analysis showed that the S phase of cell cycles for SMMC-7721 cells with pIGF-II As was increased, but there were no obvious changes in the control groups of SMMC-7721 cells and SMMC-7721 cells with pcDNA3. CONCLUSION: The pIGF-II As of IGF-II antisense RNA acts as an inhibitor on carcinogenesis of the SMMC-7721 cells in vitro.

Neurovascular anatomy of the rectus femoris muscle related to functioning muscle transfer.

To describe the intramuscular neurovascular anatomy of the rectus femoris muscle and to evaluate whether the muscle can be split into two functional units, 40 rectus femoris muscle specimens were studied. Ten fresh human cadavers were injected with a mixture of lead oxide, gelatin, and water through the femoral arteries. The rectus femoris muscle with its neurovascular pedicles was dissected out and then radiographed. Computer wire was sutured to each nerve branch in the muscle, and the muscle was radiographed again. Radiographs with and without radiopaque wire were then analyzed. In 10 preserved cadavers, the rectus femoris muscle was dissected out. Note was made of the vessel and nerve to the muscle. All muscles were cut serially into 2-cm cross-sections, and the position and course of the intramuscular tendon were then grossly examined. Three different vascular patterns in 40 rectus femoris muscles were found, based on the number of vascular pedicles and their relative dominance within the muscle. The rectus femoris muscle received either a single vascular pedicle (12.5 percent), a dominant vascular pedicle and one or two minor pedicles (80 percent), or two dominant vascular pedicles (7.5 percent). The rectus femoris was innervated by a large nerve branch from the posterior division of the femoral nerve, and the branch generally divided into two sub-branches before it reached the muscle. Both branches were respectively accompanied by arterial branches to form neurovascular hila. Furthermore, this present study has provided a detailed description of the intramuscular neurovascular territories. Also, the pattern of neurovascular supply of the muscle makes it possible to subdivide the muscle into two functional units for segmental muscle transfer.

[Evaluation of safety in Chinese women with amenorrhea following injection of depot-medroxyprogesterone for contraception].

OBJECTIVE: To determine whether serum estradiol (E2) level was low after Depot-medroxyprogesterone (DMPA) induced amenorrhea for contraception, thus to evaluate the safety of using DMPA. METHODS: Forty four Chinese women with amenorrhea after injecting DMPA were investigated, and the median period of amenorrhea was 18 months. Symptoms, pelvic examination, serum E2 and follicular stimulating hormone (FSH) levels, and vaginal cytology were examined. RESULTS: Women with amenorrhea had neither symptoms nor genital atrophy that similar to menopausal syndrome. The median level of serum E2 was 150.5 pmol/L (equal to the level of early follicular phase), and the median level of serum FSH was 14.0 IU/L. There were no correlation between E2 and FSH levels, and no correlation between amenorrhea period and E2 or FSH levels (P > 0.05) as well. No significant relationship between estrogen effects by vaginal cytology and the time period of amenorrhea was found. However, the estrogen effect was significantly correlated to the interval from last injection to vaginal smear tested. CONCLUSION: It could be concluded that though DMPA may induce amenorrhea, it will not lower serum E2 levels, and the inhibition of ovarian function is reversible.

[The effects of IL-1 on synthesis of collagen in scar-derived fibroblasts].

OBJECTIVE: To investigate modulation of cytokines on biological activities of scar-derived fibroblasts. METHODS: This study focused on the effects of interleukin-1 (IL-1) on collagen biosynthesis, nucleic acid, and cell organs of fibroblasts. RESULTS: The results showed the IL-1 could reduce not only the collagen synthesis of fibroblasts, but also quantity of nucleic acid. CONCLUSION: The authors confirm that IL-1 is a cytokine with down-regulation effect on scar formation, and suggest that IL-1 can play an important role in prevention and treatment of scar.

[The effect of hysterectomy on sexual behavior in women].

OBJECTIVES: To explore the effect of total hysterectomy on sexual behavior in women and to improve the quality of life in posthysterectomic women. METHODS: We observed 107 of patients who had received surgery for more than six months. They were divided into 3 groups: 1st group (19 pationts), hysterectomy; 2nd group (18), unilateral salpingo-oophorectomy and hysterectomy; and 3rd group (70), bilateral salpingo-oophorectomy and hysterectomy. The patients were also divided into groups(< 45 years, 45-50 years, > 50 years) according to age. The difference of sexual behavior before and after hysterectomy was compared and the effect of FSH, E2, T on sexual behavior was also analysed. RESULTS: Sexual function after hysterectomy was normal in 43.9%, sexual disturbance 26.2%, fear or misunderstanding 17.8%, and no partner or ill partner 12.2%. Sexual function and sexual desire in the < 45 years group was significantly different from other groups (chi 2 = 11.2633, P = 0.0036). The effect of E2 on sexual function and desire was significant (t = 2.18, P = 0.032). CONCLUSIONS: Total hysterectomy affects sexual behavior. It is highly important to advise patients to get rid of psychosomatic disturbances before and after hysterectomy. Since overian function is very important for maintenance of normal sexual function, it is not advisable to do preventive overiotomy. The estrogen level has a great influence on the sexual behavior of women who had hysterectomy, but the role of androgen in women's sexual behavior needs to be studied.

[A study of the maximal mouth opening movement of normal subjects].

OBJECTIVE: The aim of this paper was to study condyle movement and the relation between it and incisor movement when the subjects performed maximal open-closed movement. METHODS: 38 normal students were tested by a six degrees of freedom mandibular movement recorder MT-1602. The traces and velocity curves of the condyle movement were analized on the horizontal plane. RESULTS: (1) The horizontal traces of condyle movements included straight line and smooth curve. (2) The velocity curve was like triangle. Acceleration was constant before and behind the peak respectively. (3) Unequal velocity of the bilateral condyles was common. (4) The appearances of traces correlated with the beginning relative horizontal velocity of bilateral condyles. The tracing vector was inclined to the rapid side. (5) The incisor pathway was affected by both the relative horizontal velocity and the corresponding horizontal trace of the bilateral condyles. CONCLUSION: The movement of condyle were adaptable to the stomatognathic system.

[Effects of Astragaloside in treating myocardial injury and myocardial Sarco/Endoplasmic Ca(2+)-ATPase of viral myocarditis mice].

OBJECTIVE: To investigate the effects on myocardial injury and sarcoplasmic reticulum (SR) Ca(2+)-ATPase of viral myocarditis mice treated with Astragaloside (AS) and Astragalus Injection (AI). METHODS: Viral myocarditis model was created by intraperitoneal inoculation with coxsackievirus B3m (CVB3m) solution and were divided into model, AS, AI and normal control groups. The mortality, myocardial pathological changes, serum cardiac troponin I (cTnI) and the activity of myocardial Sarco/Endoplasmic Ca(2+)-ATPase (SERCA) were observed. RESULTS: The mortality of model was higher than that of the normal control (P = 0.0042), AS and AI (P < 0.05). The serum level of cTnI of model was significantly higher than that of the normal control (P < 0.001), AS (P < 0.025) and AI (P < 0.05). The myocardial necrosis and inflammatory changes of AS and AI groups were alleviated than that of model (P < 0.01). The activity of myocardial SERCA of model were significantly lower than that of normal control (P < 0.001), AS (P < 0.01) and AI (P < 0.05). CONCLUSIONS: AS and AI have some protecting effects on myocardial injury of viral myocarditis mice. AS is the effective component of Astragalus membranaceus in treating viral myocarditis. One of the mechanisms of Astragalus membranaceus and AS for viral myocarditis mice depriving of the myocardial injury may be due to improve the activity of myocardial SERCA in the mice.

[Malignant pheochromocytoma: diagnosis and treatment].

OBJECTIVE: To review the experience with the diagnosis and treatment of malignant pheochromocytoma. METHODS: Between 1986 and 1996, 7 patients with malignant pheochromocytoma were analysed. RESULTS: Compared with benign pheochromocytoma of adrenal gland, the malignant one usually exceeded 7.0 cm in diameter, irregular in shape, invading the surrounding tissue and the normal structure of the effected adrenal gland disappeared. Bleeding and necrotic area were seen in tumor mass, even seemed as a cystic lesion with thick wall. In some patients, blood catecholamine and urine VMA significantly elevated without hypertension. The separating phenomenon and progressive weight loss, accelerated ESR were characteristics of malignant pheochromocytoma. CONCLUSIONS: In some patients with malignant pheochromocytoma, histological findings are not consistent with their biological behaviors. Follow-up studies is mandatory in malignant pheochromocytoma as well as in benign one. Recurrence and/or metastasis after operation often occur in patients with malignant pheochromocytoma. In addition to early diagnosis and surgery, radiotherapy of tumor bed and systematic chemotherapy should be emphasized.

[Prevention of postoperative recurrence of bladder cancer: a clinical study].

OBJECTIVE: To lower postoperative recurrence rate of bladder cancer, the prophylactic effects of five kinds of method on bladder cancer were evaluated. METHODS: Between 1982 and 1997, 313 patients after TURBT or partial cystectomy were followed up for 2 to 15 years (average 7.6 years). These patients were divided into six groups: BCG, mitomycin C (MMC), thiotepa, Chinese herb medicine Zhuling (Grifola umbellata pilat), afterloading brachytherapy and control group. The prophylactic effects of them on postoperative recurrence of bladder cancer was evaluated. RESULTS: During the follow-up, the recurrence rates were 35.1% in BCG group, 34.9% in Zhuling group, 41.7% in MMC group, 52.6% in thiotepa group, 64.7% in control group, respectively. 25 high-risk patients with invading or multiple bladder cancer were treated by afterloading brachytherapy. They were followed up from 12 to 42 months, with a recurrence rate being 24.0%. CONCLUSIONS: The prophylactic effect of Zhuling and BCG on bladder cancer recurrence was better than MMC. The vale of thiotepa was not significant. The afterloading brachytherapy was of great vale to invading or recurrent, multiple bladder cancer.

[A clinicopathologic study on eight cases of cystic and solid tumors of pancreas].

OBJECTIVE: To study the clinicopathologic characteristics, differentiation patterns and histogenesis of cystic and solid tumors of the pancreas (CSTP). METHODS: 8 cases of CSTP were studied using histologic (HE and PAS), immunohistochemical (S-P method) and electron microscopic techniques. RESULTS: All the patients were adolescent and young adult females, 14-33 years in age (mean 25.3 years) without recurrence after tumor resection. The mean diameter of tumors was 9.6 cm, all encapsulated. Histological examination showed presence of solid sheets, pseudopapillary, in all of the cases. Hemorrhage, foam cells, and cholesterol crystals were often found. Immunohistochemically, 8 cases were positive for alpha(1)-AT and lysozyme; 6 cases expressed vimentin, 2 cases expressed actin, and CgA-positive cells found in the tumor cell nests in one case. All of the cases showed PR, and 4 cases showed ER positive immunoreactivity in the majority of tumor cells, but negative for CK AE1, CK AE3, EMA, Synaptophysin, ACTH, gastrin, somatostatin, insulin, and glucogan in all the cases. Electron microscopy of 3 cases showed evidences of polymorphism in differentiation of the tumor cells, including the transitional appearance into ducts, acinus, and endocrine cells. Weibel-Palade body found in tumor cells in one out of 8 cases. CONCLUSIONS: (1) CSTP is a distinct clinicopathologic entity in young female patients with a benign clinical course. (2) CSTP develops from primitive pancreatic cells, with the potentiality of developing into ducts, acinus, and endocrine cells.

[Significance of detection of HBV-DNA and CMV-DNA by polymerase chain reaction in screening mothers' milk].

Hepatitis B virus(HBV) DNA and cytomegalovirus DNA in the colostrum of lying-in women were detected by polymerase chain reaction for the safety of mothers' milk in the milk bank. The results showed that: 1. HBV-DNA was not found in the milk of women with negative serological markers of the virus, while in the milk of women with positive marker of HBsAg, HBeAg and HBcAb, the positive rate of HBV-DNA was 81.25%, in the milk of mothers with positive marker of HBsAg, HBcAb and HBeAb, the positive rate was 45.24%, in the milk of women only with HBcAb and HBeAb positive, the rate was 23.08%. 2. The specific amplified products of CMV DNA in the milk were obtained in 42.80%-70.00% women whose CMV IgM antibody was positive. These findings suggest that the excretion of viruses would increase in mothers with active virus replication, and the viruses would be transmitted to newborns by milk. The authors think that only the milk from the mothers with both negative serological markers and negative PCR of the viruses can be collected and stored in the milk bank. PCR is a specific and sensitive technique which can be used to monitor the safety of mothers' milk. In this paper, the authors have presented some suggestions about how to give breast-feeding and protect newborns whose mothers are various status of virus secretion in the milk.

[Determination of alkaloids in Coptis-Cinnamomum compatibility by capillary electrophoresis].

OBJECTIVE: To establish a HPCE method to determine the contents of alkalods in Rhizoma Coptidis. METHOD: HPCE. RESULT: The constituents thus determined have good linearity and separation. The average recovery of coptisine, berberine, palmatine and jatrorrhizine was 98.28%, 98.38%, 97.78% and 98.89% respectively and RSD 0.86%, 1.12%, 1.06% and 1.11% respectively. CONCLUSION: The method is convenient, rapid, accurate and practicable. The compatibility reduces the contents of alkaloids in Rhizoma Coptidis in different degrees.

[Expression and significance of neuropeptide and neurotensin in the extract of platelet of chronic renal failure patients during hemodialysis].

The aim of this study was to investigate the change of neuropeptide Y(NPY) content and neurotensin (NT) content of the platelet from chronic renal failure (CRF) patients during hemodialysis (HD) and explore its mechanism of action. Platelet was separated from the patients' plasma and the NPY and and NT contents of the platelet and plasma were dynamically observed pre-HD and post-HD. 30 healthy people were chosen as the controls. The results showed the NPY and NT contents of the platelet extract were (58.18 +/- 21.29) ng/10(9) and (25.38 +/- 13.43) ng/10(9) respectively. The NPY content of the platelet extract of the CRF patients was obviously decreased and the NT content of the plasma was obviously increased however, the NT contents of the platelet extract and the plasma were both higher than those in the control group. The NPY content of the platelet extract and NT content of the plasma at the post-HD obviously increased as compared with pre-HD, but the NT content of the platelet extract and the NPY content of the plasma obviously decreased. There was an obvious correlation between NPY and NT. The authors conclude that during the period of CRF, the immunoreactions which mainly involve bio-active substances such as NPY, NT and 5-hydroxytryptamine released by the platelet to interfer the vasoconstrictive effect are the main pathologic factors to induce renal hypertensin and renal vasospasm.
