Atrioventricular block in the atypical form of junctional reciprocating tachycardia: evidence supporting the atrioventricular node as the site of reentry.

Serial electrophysiologic studies were performed in 19 patients with the atypical form of supraventricular tachycardia having a long RP and short PR interval. In all 19 patients, supraventricular tachycardia was found to have a 1:1 P-QRS relation during initial control electrophysiologic studies, and in all 19 patients, electrophysiologic studies suggested that junctional reentry was the mechanism of supraventricular tachycardia. Seven of the 19 patients developed atrioventricular (AV) block during initiation of supraventricular tachycardia or after induction of supraventricular tachycardia following various drug administrations in subsequent studies. In three patients, second degree block within the His bundle or block distal to the His bundle recording site occurred after administration of quinidine. In one patient it occurred after procainamide, and in another patient it occurred after atropine. In one patient, 2:1 block proximal to the His deflection occurred after verapamil. In the remaining patient, a transient Wenckebach block proximal to the His deflection was noted after adenosine triphosphate. In this latter patient, 2:1 AV block was also noted after propranolol and digoxin. The site of reentry in these seven patients with AV block during supraventricular tachycardia was confined to the AV node area. Their supraventricular tachycardia did not involve a slowly conducting paraseptal accessory pathway because the distal AV node, His bundle and ventricle were not found to be necessary links in the tachycardia circuit.

Myo-inositol does not modulate PI turnover in MDCK cells under hyperosmolar conditions.

The relationship between free cellular myo-inositol concentration and phosphatidylinositol turnover was evaluated in Madin-Darby canine kidney (MDCK) cells under isosmolar and hyperosmolar conditions. MDCK cells exposed to high extracellular sodium chloride were documented to increase their free myo-inositol content as measured by gas-liquid chromatography in both a time- and concentration-dependent manner. Measurement of phosphatidylinositol, phosphatidylinositol 4-monophosphate, and phosphatidylinositol 4,5-bisphosphate mass failed to reveal changes under conditions where the myo-inositol concentration was more than threefold higher compared with control conditions. CDP diacylglycerol:myo-inositol 3-phosphatidyltransferase activity, measured in plasma membranes from MDCK cells grown under control and hyperosmolar conditions, was kinetically similar with comparable observed Michaelis constant (Km) and maximal rate of enzyme reaction. Moreover, the apparent Km was significantly below the estimated intracellular myo-inositol concentration consistent with exposure of the transferase to saturating concentrations of myo-inositol under both conditions. Finally, bradykinin-stimulated myo-inositol trisphosphate mass was measured by use of a competitive binding assay. Both basal and hormone-stimulated myo-inositol 1,4,5-trisphosphate levels were not significantly different under control and hyperosmolar conditions. These data indicate that bulk free myo-inositol content is unlikely to regulate phosphatidylinositol turnover and myo-inositol trisphosphate formation under hyperosmolar conditions.

Hemodynamic and clinical efficacies of catheter balloon percutaneous transvenous mitral commissurotomy: experience of 100 patients with rheumatic mitral stenosis.

From January 1987 to December 1988, 100 patients with symptomatic severe rheumatic mitral stenosis underwent percutaneous transvenous mitral commissurotomy (PTMC). The patients included 32 males and 68 females, aged 19-71 years (mean of 41). Mild mitral regurgitation (grade 1 or 2) was present in 23 patients and a history of thromboembolism in 12. One patient had had mitral restenosis after surgical open mitral commissurotomy 9 years earlier. The mitral valve was successfully dilated in 97 patients. PTMC resulted in immediate improvements in hemodynamic measurements. The left atrial pressure decreased from 24.5 +/- 5.3 to 14.8 +/- 5.2 mmHg (p less than 0.001), the mean mitral transvalvular gradient from 13.8 +/- 4.8 to 5.0 +/- 2.8 mmHg (p less than 0.001), and the mean pulmonary artery pressure from 38.8 +/- 12.0 to 30.6 +/- 10.3 mmHg (p less than 0.001). The mitral valve area increased from 1.1 +/- 0.3 to 2.2 +/- 0.8 cm2 (p less than 0.001). The cardiac output increased from 4.5 +/- 1.2 to 4.84 +/- 1.2 L/min (p less than 0.05). The right atrial pressure did not change significantly after PTMC (6.5 +/- 3.8 vs 6.4 +/- 4.0 mmHg). The mitral valve area measured by 2-D echocardiograms increased from 1.04 +/- 0.48 to 1.88 +/- 0.66 cm2 after PTMC (p less than 0.001). All 97 patients were followed for 6-24 months (median of 13) after the PTMC. After an initial recovery period of 1-2 weeks, all patients reported improvements in symptoms and in New York Heart Association (NYHA) functional class by at least one class. A comparison between treadmill exercise test durations before, and 3 months after PTMC, showed an increase from 9.1 +/- 4.3 to 15.4 +/- 3.8 minutes (n = 60; p less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

Clinical application of microcomputer to visual function examination.

Microcomputer technique is put into use in ophthalmological clinical visual function examination. In visual EOG, ENG, visual tracking, contrast sensitivity and VEP examination, objectifying and standardization of experiment and data analysis are achieved by computer technique. In addition, a lot of manpower and time is saved, test results are obtained faster and more accurately. The basic rational of computerization in clinical visual function examination is presented.

ERG characteristics of congenital stationary night blindness.

The ERGs of 9 cases (18 eyes) of congenital stationary night blindness with normal fundi or myopia were tested. All eyes showed nonrecordable rod ERG and cone ERG with normal a-wave. Scotopic mixed ERGs were of the negative type in 7 eyes and of the subnormal type in 11 eyes. The b/a ratio was reduced in all eyes. The ERG characteristics are useful for the classification, estimation of the probable location of the lesion, and differential diagnosis of the disease.

The proportion of various types of congenital color vision defects.

One hundred and three color vision defective subjects were screened from 3456 middle school students with pseudoisochromatic plate test. One hundred subjects out of them were further examined with a test battery including Panel D-15 and FM 100-hue test and Neitz anomaloscope test. It was found that there were 21 protanopes (P), 3 extremely protanomalous (EPA), 13 protanomalous (PA), 25 deutenopes (D), 5 extremely deuteranomalous (EDA), 28 deuteranomalous (DA), 6 unclassified subjects in our investigation. The proportion of various types of congenital color vision defects was P:EPA:PA:D:EDA:DA = 1.00:0.14:0.62:1.19:0.24:1.33.

[Visual pigment genes for color vision defects].

Applying recombinant DNA techniques, the structures of red pigment gene (RPG) and green pigment gene (GPG) were analyzed for 43 patients with protan or deutan (including 3 females), 4 normal relatives and 3 carriers out of 3 families, as well as 11 normal controls. Abnormality of RPG was detected in all 19 protan and that of GPG was found in 14 out of 24 deutan. In about 80% (32/40) of protan and deutan the changing of exon 5 for RPG or GPG was discovered. In protan the normal RPG was replaced by a 5' red -3' green hybrid gene. Some of the deutan had no GPG, some had 5' green -3' red hybrid gene with or without GPG. Furthermore, the exon 5 of RPG and GPG was amplified by polymerase chain reaction (PCR) and further analyzed by Rsa I digestion. The results for PCR are identical to that of Southern blot hybridization.

[The visual-vestibular interaction in normal subjects].

Full-field optokinetic nystagmus (OKN) at constant speed of 40 0/s and 60 0/s, vestibular ocular reflex (VOR) (sinusoidal 0.2 Hz, 60 0/s peak velocity) with different background (light and dark) and VOR-fix were observed in 72 normal subjects (144 normal eyes). The gain, FCV and DP of OKN, VVOR, VOR were also analysed. The gain of OKN is reduced accompanied with stimulation increasing, while the FCV is increased. The gain of VVOR is 1.08 +/- 0.09 (X +/- SD), while the gain of VOR is 0.64 +/- 0.09. The FCV of VOR is much reduced than that of VVOR. The FCV in the groups under 30 years of age are much faster than the groups above 30 years of age. The VOR--fix gain is 0.05 +/- 0.04. The DP of OKN, VVOR, and VOR are 0.05 +/- 0.04. The results showed an co-operation between visual and vestibular systems and the results also suggested that the series methods of visual-vestibular interaction (OKN, VVOR, VOR, VOR-fix) might be useful in eye movement examination.

[The effect of pesticides-CDM and AMZ on inhibition of the binding of 3H-clonidine to alpha 2-adrenoreceptor in rat forebrain tissue].

Chlordimeform (CDM) and Amitraz (AMZ) are two formamidine pesticides. CDM and AMZ inhibited the binding of 3H-Clonidine, 3H-Yohimbine to Alpha 2-adrenoreceptor of rat forebrain tissue in vitro with IC50 values of 62-68 mumol/L (CDM) and 95-110 mumol/L (AMZ). In vivo the administration of CDM and AMZ by ip showed the effect of inhibition on the binding and significant correlation of dose-effect r = 0.995 (CDM), 0.884 (AMZ). Time course test indicated that the lasting inhibition effect of CDM following exposure was shorter than AMZ. Similar results were observed with plasma in vitro. Comparing inhibition effects on rat forebrain tissue, directly induced by CDM and AMZ at different doses, with the results from the plasma of treated rats in vitro, there was a significant correlation r = 0.973 (CDM), 0.909 (AMZ). It showed that the inhibition effect of both CDM and AMZ was linked to the concentration of compounds chemical and their metabolites in blood. Time course and washing test indicated that the inhibition effect by CDM and AMZ was reversible.

Deletion mapping of Aland Island eye disease to Xp21 between DXS67 (B24) and Duchenne muscular dystrophy.

Aland Island Eye Disease (AIED) is an X-linked form of ocular hypopigmentation--also known as Forsius-Eriksson, or type 2, ocular albinism--in which affected males demonstrate subnormal visual acuity, protanomalous red-green colorblindness, axial myopia, astigmatism, hypoplasia of the fovea, and hypopigmentation of the fundus. A patient has previously been described who, in addition to AIED, manifested a contiguous gene syndrome which included congenital adrenal hypoplasia (AHC), glycerol kinase deficiency (GKD), and Duchenne muscular dystrophy (DMD). In the present paper report we report the molecular genetic analysis of his deletion. Initially, multiplex polymerase-chain-reaction amplification was used to screen for a DMD-locus deletion which was then further characterized, using DMD cDNA and genomic probes, via Southern blot analysis. The deletion includes the region encompassed by probes C7 (DXS28) and DMD cDNA 8. Probes B24 (DXS67) and DMD cDNA 5b-7 show normal hybridization patterns and appear to flank the deletion, while the DMD cDNA 8 detects a junction fragment. Molecular genetic techniques have mapped the deletion in this patient to the subbands Xp21.3-21.2, between DXS67 and DMD.

[The relationship of Sindbis virus assembly and the viral protein 6K with intermediate filaments].

The relationship of Sindbis virus (Sbv) assembly with intermediate filaments was studied by means of whole-mount and DGD embedment-free technique of EM together with the procedure of gentle extraction. In the early stage of Sbv infection, the "virus assembly center" was suspended in the intermediate filament network. In the late stage, the assembling and assembled virus nucleocapsids were associated with intermediate filaments. It is presumable that the virus nucleocapsids may move from the "virus assembly center" to cytoplasmic membrane along the intermediate filaments. Further study using immunolabelling technique indicated that the nonstructural protein 6K of Sbv was probably associated with intermediate filaments as well.

[Pharmacological studies of huomaisan].

After per os Huomaisan twice a day for 5 days, HMS significantly prolonged thrombus formation time and inhibited platelet aggregation induced by ADP and collagen. Meanwhile, HMS also decreased blood viscosity, but did not significantly affect hematocrit. The result shows that HMS has multiple actions on blood.

[The visual evoked potentials in ischemic optic neuropathy].

In this study the pattern-reversal stimulation was used for analysing the clinical VEP in 27 cases (37 eyes) of ischemic optic neuropathy (ION), and combined uses of visual field (VF) and contrast sensitivity (CS) examinations were compared and observed with 3 kinds of spatial frequencies (60', 30', 15' check size), VEP in the eyes of ION displayed decrease in amplitude as a dominant manifestation accompanied by the slight increase of latency period. The abnormal latency and amplitude were 83.5% and 81.3% respectively. The decreased CS in the ION eyes was appeared mainly in the stimulation of middle and high frequencies or all frequency bands. There was a significant correlation between the VEP amplitude and CS in the patients with ION. The quadrant VF defect and bow-shaped scotoma were displayed in majority of the cases. Abnormal VEP was shown to be related to the central VF impaired. Among these patients, 7 cases (8 eyes) were followed up with VEP, however the VEP amplitude was shown decreasing somewhat compared with first exam. The results indicated that the ION was responsible for many visual function damages.

Evaluation of automated urinary iodine methods: problems of interfering substances identified.

We evaluated automated methods for measurement of urinary iodine (UI) over a range expected in iodine-replete and iodine-deficient populations. Results obtained with Technicon AutoAnalyzer II systems, based on either dialysis or acid digestion, were compared with those obtained by a manual alkaline ashing technique. Results of automated dialysis were consistently higher than those obtained by the other methods. The apparently higher concentrations of UI we measured were due to interfering substances crossing the dialysis membrane and participating in the catalytic reaction. Thiocyanate (SCN) was one endogenous substance contributing to the increased measurement of UI. For urinary SCN concentrations of 5 to 15 mg/L, the amount of overestimation in the UI measurement attributable to SCN ranged from 21.8 to 61 micrograms/L. However, SCN may account for only 40-50% of the apparent increase in UI. In samples with lower UI (less than 50 micrograms/L), interfering substances produced a 100% error in results. We conclude that the automated dialysis system should not be used to assess iodine-deficient populations. This leaves a major dilemma for researchers wanting to assess the iodine status of populations, because the automated digestion method is no longer commercially available.

Topical iodine-containing antiseptics and neonatal hypothyroidism in very-low-birthweight infants.

The thyroid function of very-low-birthweight (VLBW; below 1500 g) infants admitted to neonatal intensive-care units was studied at two hospitals; one routinely used topical iodinated antiseptic agents and the other used chlorhexidine-containing antiseptics. Serial monitoring of urinary iodine excretion and serum thyrotropin and thyroxine levels was undertaken from birth for the first 4 weeks of life. Urinary iodine excretion rose dramatically in the 54 iodine-exposed infants and was up to fifty times greater than in the 29 non-exposed infants. Within 14 days, 25% (9 of 36) of the infants exposed to iodine had serum thyrotropin levels above 20 mIU/l, compared with none of the control group. The mean serum thyroxine level in these 9 infants (44.1 nmol/l) was significantly lower than that in exposed infants with normal thyrotropin levels (83.1 nmol/l) and in the non-exposed control group (83.0 nmol/l); thyroxine levels fell before serum thyrotropin rose. These disturbances in thyroid function correlated positively with urinary iodine excretion and hence iodine absorption. Thyroid function had returned to normal by the time of discharge from hospital. It is concluded that iodine absorption, from topical iodine-containing antiseptics, may cause hypothyroidism during a critical period of neurological development in the newborn infant. The routine use of iodine antisepsis in VLBW infants should be avoided because of this effect.

Combined segregation and linkage analysis for IDDM and HLA-DR under several ascertainment assumptions.

Combined segregation and linkage analysis of the Genetic Analysis Workshop 5 (GAW5) data suggests a complex basis for susceptibility to insulin-dependent diabetes mellitus (IDDM). One susceptibility gene, linked to the HLA-DR region, is additive on the liability scale (d = .49 +/- .15, t = 2.9 +/- .6) with a gene frequency q = .30 +/- .03. A second locus, with a gene frequency of q = .07 +/- .02, which is recessive and unlinked to HLA, is also suggested by the analysis. The ascertainment correction used has little effect on the results, presumably because most of the information comes from the cosegregation of HLA alleles and disease status. The results are consistent with a direct involvement of the HLA-DR region in susceptibility, but are not a proof of it.
