A bioelectric inverse imaging technique based on surface Laplacians.

A new approach is proposed to solve bioelectric inverse problems by employing the surface Laplacian of the bioelectrical potential. A theoretical investigation was conducted to test the feasibility of epicardial inverse imaging of cardiac electrical activity. A two-sphere homogeneous volume conductor model, where the inner sphere represents the epicardium and the outer sphere the body surface, was used. Radial and tangential current dipoles were used to approximate localized wavefronts propagating from the endocardium to the epicardium, and ectopic myocardial activities. The epicardial potential distribution was reconstructed from the body surface Laplacians with the aid of the Tikhonov zero-order regularization technique, which then was compared with the results obtained from the body surface potentials using the same regularization scheme. The two inverse solutions were compared qualitatively via visual inspection of the reconstructed epicardial potential maps, and quantitatively by examining relative errors and correlation coefficients between the "true" and the reconstructed epicardial potentials. Both qualitative and quantitative results indicate that the surface Laplacians play a positive role in improving the ill-posed nature of the bioelectric inverse problem, which would enhance our capability of reconstructing important epicardial events such as extrema in the epicardial potential distribution. The present theoretical study suggests that the Laplacian-based inverse imaging technique may have important applications to epicardial inverse imaging and other bioelectric inverse imaging.

Atypical atrioventricular nodal reentry tachycardia with atrioventricular block mimicking atrial tachycardia: electrophysiologic properties and radiofrequency ablation therapy.

INTRODUCTION: Fast-intermediate form AV nodal reentry tachycardia (AVNRT) sometimes may mimic atrial tachycardia or atrial flutter and render the diagnosis difficult when the tachycardia rate is fast and AV block occurs during tachycardia. METHODS AND RESULTS: A 45-year-old woman with paroxysmal supraventricular tachycardia was referred to this institution. Initially, the tachycardia was thought to be an atrial tachycardia because of: (1) a short cycle length of the tachycardia with 2:1 and Wenckebach AV block; (2) a difference in the atrial activation sequence during tachycardia and during ventricular pacing; and (3) failure of burst ventricular pacing to affect the atrial rate and the atrial activation sequence during tachycardia. An accurate diagnosis of fast-intermediate form AVNRT was subsequently made based on the finding that the tachycardia was induced following delivery of a third ventricular extrastimulus, which showed a sequence of V-A-H and a change on atrial activation sequence of the induced beat. Successful radiofrequency ablation was achieved only after accurate diagnosis of the tachycardia was made. CONCLUSION: Fast-intermediate form AVNRT sometimes may masquerade as atrial tachycardia. Accurate diagnosis is mandatory for successful ablation therapy.

Value of molecular monitoring during the treatment of chronic myeloid leukemia: a Cancer and Leukemia Group B study.

PURPOSE: Disappearance of the Philadelphia chromosome during treatment for chronic myeloid leukemia (CML) has become an important therapeutic end point. To determine the additional value of molecular monitoring during treatment for CML, we performed a prospective, sequential analysis using quantitative Southern blot monitoring of BCR gene rearrangements of blood and marrow samples from Cancer and Leukemia Group B (CALGB) study 8761. PATIENTS AND METHODS: Sixty-four previously untreated adults with chronic-phase CML who were enrolled onto CALGB 8761, a molecular-monitoring companion study to a treatment study for adults with chronic-phase CML (CALGB 9013). Treatment consisted of repetitive cycles of interferon alfa and low-dose subcutaneous cytarabine. Blood and marrow Southern blot quantitation of BCR gene rearrangements was compared with marrow cytogenetic analysis before the initiation of treatment and of specified points during therapy. Reverse-transcriptase polymerase chain reaction (RT-PCR) analysis was performed to detect residual disease in patients who achieved a complete response by Southern blot or cytogenetic analysis. RESULTS: Quantitative molecular monitoring by Southern blot analysis of blood samples was found to be equivalent to marrow monitoring at all time points. Twelve of 62 (19%) follow-up samples studied by Southern blot analysis had a complete loss of BCR gene rearrangement in matched marrow and blood specimens. Southern blot monitoring of blood samples was also found to be highly correlated to marrow cytogenetic evaluation at all points, although there were four discordant cases in which Southern blot analysis of blood showed no BCR gene rearrangement, yet demonstrated from 12% to 20% Philadelphia chromosome-positive metaphase cells in the marrow. RT-PCR analysis detected residual disease in five of six patients in whom no malignant cells were detected using Southern blot or cytogenetic analyses. CONCLUSION: Quantitative Southern blot analysis of blood samples may be substituted for bone marrow to monitor the response to therapy in CML and results in the need for fewer bone marrow examinations. To avoid overestimating the degree of response, marrow cytogenetic analysis should be performed when patients achieve a complete response by Southern blot monitoring. This approach provides a rational, cost-effective strategy to monitor the effect of treatment of individual patients, as well as to analyze large clinical trials in CML.

Morphine stimulates adrenocorticotropin and cortisol release in the late-term ovine fetus.

Opiates are widely used as obstetrical analgesics during pregnancy and, as such, their interactions with the fetal endocrine system may have important consequences. In this study, the effects of morphine administration to fetal sheep in utero on fetal plasma immunoreactive (ir)-ACTH and ir-cortisol were examined. At the lowest dose administered (0.6 mg/h, i.v.) morphine reduced, although not significantly, plasma ir-cortisol levels. A dose-dependent stimulation of cortisol release was observed with higher doses of morphine. Doses of 2.5 and 5.0 mg/h morphine resulted in a significant increase in ir-cortisol with a change from control levels equal to 9.6 +/- 1.1 ng/mL (p = 0.03) and 17.6 +/- 5.1 ng/mL (p = 0.03), respectively. This increase in plasma ir-cortisol was associated with a significant increase in ir-ACTH (111.8 +/- 23.2 pg/mL versus 42.8 +/- 5.1 pg/mL; p = 0.02) that was naloxone-reversible. These effects of morphine were observed in fetal lambs only > 125 d of gestation, suggesting a maturation of functional opioid receptors in the ovine fetal hypothalamic-pituitary-adrenal axis after this time.

The role of protein kinase C in GH secretion induced by GH-releasing factor and GH-releasing peptides in cultured ovine somatotrophs.

The involvement of protein kinase C (PKC) in the action of GH-releasing factor (GRF) and synthetic GH-releasing peptides (GHRP-2 and GHRP-6) was investigated in ovine somatotrophs in primary culture. In partially purified sheep somatotrophs, GRF and GHRP-2 caused translocation of PKC activity from the cytosol to the cell membranes and caused GH release in a dose- and time-dependent manner. GHRP-6 did not cause PKC translocation. The PKC inhibitors, calphostin C, staurosporine and chelerythrine, partially reduced GH release in response to GRF and GHRP-2 at doses which selectively inhibit PKC activity. These inhibitors totally abolished GH release caused by phorbol 12-myristate 13-acetate (PMA). Down-regulation of PKC by the treatment of cells with phorbol 12,13-dibutyrate for 16 h caused a significant (P < 0.001) reduction in total PKC activity and totally abolished PKC translocation in response to a challenge with GRF, GHRP-2 or PMA. In addition, down-regulation abolished GH release in response to GRF, GHRP-2 or GHRP-6. Treatment of cells with H89, a selective PKA inhibitor, totally blocked GH release caused by either GRF or GHRP-2 and partially reduced PMA-induced GH release. H89 had no effect on PKC translocation caused by GRF, GHRP-2 or PMA and did not affect GH release caused by GHRP-6. These data suggest that GHRP-2 and GRF activate PKC in addition to stimulating adenylyl cyclase activity. Although the cAMP-protein kinase A (PKA) pathway is the major signalling pathway employed by GRF and GHRP-2, the activation of PKC may potentiate signalling via the cAMP-PKA pathway in ovine GH secretion. Importantly, the effect of PMA in increasing the secretion of GH from ovine somatotrophs is effected, in part, by up-regulation of the cAMP-PKA pathway. We conclude that there is cross-talk between the PKC pathway and the cAMP-PKA pathway in ovine somatotrophs during the action of GRF or GHRP.

[Expression of p53, c-erbB1, c-myc and p16 gene proteins in human glioma].

OBJECTIVE: To realize the significance of c-erbB1 and c-myc protooncogenes, p53 and p16 tumor suppressor genes in the development and pathologic diagnosis of human glioma. METHODS: The expression of p53, c-erbB1, c-myc and p16 gene proteins was detected immunohistochemically in 65 cases of glioma. The relationship between these results and the pathological grades was determined. RESULTS: The overexpression of mutant p53, c-erbB1 and c-myc was in accordance with the increasing grade of glioma malignancy, showing significant difference (P < 0.05) between the benign group and highly malignant group in p53 and c-erbB1, while the deletion of p16 protein was more frequent in the highly malignant group than in any other group. These 4 genes showed co-expression in some of the gliomas. CONCLUSIONS: The 4 genes, especially p53 and c-erbB1, play an important role in the development of glioma; the detection of these gene proteins has a positive significance on malignancy determination for glioma.

[The test of motion perception in normal subjects].

OBJECTIVE: To study the properties of motion perception (MP) of normal Chinese subjects. METHOD: MPs were tested for 56 cases (112 eyes) of normal subjects, their ages ranging from 11 years to 68 years, by using vertical moving bars generated on VGA monitor in a PC compatible computer. RESULTS: MPs were affected by age when the bars moved horizontally for 2 pixels, the ratios of MP were increased with age growing from 10 year group to 30 year group, then the ratios of MP were gradually decreased from 40 year group. MPs were not affected by age under the horizontally moving conditions of 4 pixels and 6 pixels and > 40 Hz flicker movement. There were no relationships between either MP and sex or MP and eye side difference. CONCLUSIONS: The characteristics of MP are determined and the normal reference values are provided in normal Chinese subjects.

[Local electroretinogram and pattern visual evoked potential in maculopathies].

OBJECTIVES: To observe the changes of local electroretinogram (LERG) and pattern visual evoked potential (PVEP) in maculopathies and to evaluate the value of both LERG and PVEP in clinical application. METHODS: LERGs at 5 degrees, 10 degrees and 15 degrees macular region were recorded from 27 normal subjects (54 eyes). The LERG and PVEP were recorded from 25 patients (35 eyes) with maculopathies. RESULTS: In the patients with maculopathies, the amplitudes of LERG a wave and b wave at 5 degrees, 10 degrees and 15 degrees macular region were significantly lowered (P < 0.01), the mean value of P1 latency was prolonged and that of N1-P1 amplitude of VEP was lowered at 36.5', 73' and 146' in the comparison with the control group. In approximately the same stimulated area (15 degrees LERG and 14.8 degrees x 19.0 degrees PVEP), the abnormal rate of LERG was 62.2%, and the abnormal rates of PVEPs were 48.5% (146'), 54.5% (73') and 48.5% (36.5') respectively, no significant difference in the abnormal rate between LERG and each value of PVEP (P > 0.05) being found. CONCLUSION: The abnormal rates of LERG and PVEP in maculopathies are close. Because LERG is not influenced by visual pathway diseases and visual acuity, LERG is more effective than PVEP and is a direct way in measuring macular function.

[The significance of preoperative visual electrophysiology for vitrectomy].

OBJECTIVE: To investigate the prognostic value of preoperative visual electrophysiology in patients with vitreoretinopathy (VRP) who underwent vitrectomy. METHODS: Electroretinogram (ERG) and visual evoked potential (VEP) were performed prior to vitrectomy in 82 patients with vitreous hemorrhage or opacity due to ocular trauma, retinal detachment or retinal vascular diseases (Eales' disease, diabetic retinopathy, etc.). A comparative analysis was made of the effectiveness and visual acuities 2 weeks postoperatively. RESULTS: Among the 82 eyes, the preoperative ERG was normal in 6 eyes, slightly or moderately reduced in 37 eyes and severely reduced or had no response in 39 eyes. Of these 39 eyes, no improvement or reduction in postoperative visual acuity was seen in 23 and 6 eyes respectively, a sharp contrast to the postoperative results of the 6 normal ERG eyes. The preoperative ERGs differed due to different causes. A further analysis regarding the VEP of the patients who had poor postoperative results was made. CONCLUSIONS: The data showed that the preoperative visual electrophysiology was significantly correlated with the effectiveness of the operation. It is helpful in prognostic assessment of the postoperative outcome and visual acuity changes in VRP patients treated mainly by vitrectomy.

[Regeneration of functionally active rat brain muscarinic receptor in vitro after inhibition with methylmercury chloride].

The effect of methylmercury on muscarinic receptors and the regeneration of functionally active muscarinic receptor in vitro by antagonists were investigated. The result showed that methylmercury chloride (MMC) inhibited the binding of [3H] QNB to muscarinic receptor of rat brain-lysed synaptosomes, with IC50 values of 4.18 mmol/L. Regeneration of functionally active rat brain muscarinic receptors after inhibition with methylmercury was achieved by 2,3-dimercapto-1-propanesulfonc acid, Na salt (DMPS) dithiothreitol (DTT), glutathione (GSH) and Cysteine. Blocking the sulfhydryl groups is suggested to be the molecular mechanism of inhibition of brain muscarinic receptors by methylmercury. Our results provide evidence that thiols chelate out mercuric cations that tightly bound to sulfhydryl groups in muscarinic receptor binding sites and regenerate [3H] QNB binding activity.

[Studies on programmed cell death induced by amsacrine and expression of bcl-2 in leukemia cell lines].

OBJECTIVE: To study programmed cell death (PCD) in two human acute myelocytic leukemia cell lines in different differentiated phases, NB4(AML3) and HL-60(AML2). METHODS: PCD cells were detected microscopically, DNA electrophoretically and flow cytometrically. Expression of bcl-2 gene was examined by immunohistochemistry staining. RESULTS: Exposed to 1-20 micrograms/ml m-AMSA for 6-12 h, morphological changes in NB4 cells were more evident than in HL-60 cells. Internucleosomal fragmentation(DNA ladders) was more marked in NB4 than in HL-60 cells. Flow cytometric analysis indicated that the number of PCD cells in NB4 cells was more than that in HL-60 cells. HL-60 cells had significantly higher mean bcl-2 level than NB4 cells. CONCLUSION: The high bcl-2 level in HL-60 cells may be related to their comparatively low sensitivity to m-AMSA-induced PCD.

[Study of the relationship between the expression of Ki-67 antigen and the pathobiological behaviours of stomach cancer].

OBJECTIVE: To evaluate the relationship between the expression of Ki-67 antigen and the pathobiological behaviours of stomach cancers especially their distant metastases. METHODS: 56 stomach cancer specimens routinely fixed in formalin and embeded in paraffin (FFEP) were studied by immunohistochemical method. RESULTS: Expression of Ki-67 antigen was significantly related to the distant metastases to liver, ovary and adrenal gland (P < 0.005); but not related to the histological type, growth pattern, depth of invasion, histological differentiation and the metastases to local lymph nodes (P > 0.05). Furthermore, the Ki-67 expression was significantly related to the DNA anueploid pattern and the expression of p53 protein, both of which are closely related to poor prognosis. CONCLUSION: Overexpression of Ki-67 can be used as an objective marker of the proliferative activity for predicting prognosis of stomach cancer and to foretell the high-risk of metastasis of distant organs.

[Automatic discriminatory analysis of visually evoked potentials waveforms in normals and amblyopes].

We presented a method of discriminatory analysis for the discrimination of P-VEP waveforms in frequency domain, and used this analysis in the discrimination of the P-VEP waveforms of normal eyes and amblyopic eyes. In this analysis, the amplitudes and the phases of the first to the sixth harmonics of P-VEP waveforms were calculated, thus 12 parameters of the P-VEP waveform were obtained for the discriminatory analysis. The P-VEP waveforms of thirty-one normal eyes and thirty-one amblyopic eyes were used to establish the discriminatory system. The result of clinical application showed that this method of Discriminatory Analysis was effective for the discrimination between normal and amblyopic P-VEP.

Molecular alteration of alpha-induced transformed Syrian hamster embryo cells.

Syrian hamster embryo (SHE) cells are irradiated by alpha particles emitted from Plutonium-238 in vitro. Transfection frequencies of donor DNA derived from passaged cells, including monocloned cells from 20th passage cells, after irradiation are parallel with their tumorigenicity of donor cells. After transformation, expressions of H-ras and c-myc genes are in the state of activation, which might be resulted from occurrence of mutation in H-ras gene and hypomethylation of c-myc gene respectively, even though no alteration was observed on their restriction fragment length polymorphisms (RELP) patterns of both genes. On the other hand, expression of p53 gene is also found to be in the state of activation in transformant, but no heavy staining of p53 protein appeared in the transformant-derived tumor with p53 specific McAb HD 200. This might be result from the partial deletion of p53 cDNA in transformant by reverse transcriptional polymerase chain reaction (RT-PCR). These results imply that activation of H-ras and c-myc genes coupled with inactivation of p53 tumor suppressor gene play important role in alpha particle-induced cell transformation.

Neoplastic transformation and cytogenetic aberrations of rat tracheal epithelial cells induced by alpha-particle irradiation.

Primary rat tracheal epithelium (RTE) cells can be transformed in vitro by alpha-particles irradiation. One out of 3 isolated morphologically transformed colonies gave rise to an immortal cell line and ultimately became neoplastic by successive passaging. Cytogenetic analysis was performed on the cell line in order to understand how the specific chromosome alterations participate in the process of neoplastic transformation of RTE cells. The 5th, 18th, 39th passage cells and the colony cells isolated from soft agar (SA) culturing of the 40th passage cells were analyzed. The results showed that the 20th passage cells were nontumorigenic, but the 40th passage cells were tumorigenic. The increase of the length of the long arm of chromosome 15 (15q+) was frequent at the 5th passage (67.8%), especially in SA cells in which it occurred in a clonal fashion (100%). Therefore, 15q+ might be the critical gene lesion after alpha-particles irradiation. Monosomy of chromosome 8 that occurred frequently at passage 5 (54%) and 40 (53%) might be correlated with earlier transformation of RTE cells. In addition, three marker chromosomes (M1, M2 and M3) showed a passage-dependent increase. In particular M1 and M2 that were highly frequent at passage 39 (90%) suggested that they might play an important role in the process of cell transformation.

Changes in the divorce rate and age distribution in China since the 1980s.

"The divorce rate in China during the 1980s was much higher than before for a number of reasons. In order to understand this phenomenon, the authors will make a comparison between the divorce rates, and the age and duration of the marriage at the time of divorce in the early 1980s and in the late 1980s and early 1990s."
