Albumin isolated by the Hagen/Münster heat-ethanol method is a remarkably stable solution of high purity and clinical compatibility. The simplicity of this method makes it a model fractionation procedure for the small transfusion service. Immunization studies carried out by us are compared with previous accounts of a "new" antigen occuring in plasma protein solution produced by partial heat denaturation. Clinical compatibility of cold-ethanol and heat-ethanol fractionated albumin solutions are also shortly discussed.
During a routine clinical examination of a 7-year-old girl and a 17-year-old man a large tumor was discovered in the left upper quadrant of the abdomen. Large splenic cyst were found in the spleen at splenectomy. The traumatic etiology of the cysts can be assumed on the basis of histologic results, as well as of a significant history of previous trauma. The etiology, differential diagnosis, therapy, and classification of splenic cysts are discussed.
The change of physical properties of plastics (polyoxymethylene-copolymer, polyethyleneterephthalate, polyethylene, polytetrafluorethylene) and the bio-compatibility of these materials were examined by implantation in the backmuscle of 6-month-old male rabbits for 4 and 12 weeks and after autoclavation. We have found out, that after implantation and autoclavation polyethylene-terephthalate demonstrates a strong diminution of the visco-elastic qualities. Polyethylene and polytetrafluorethylene were not changed by these treatments. After an implantation of 12 weeks and an autoclavation of four times the visco-elastic properties of polyoxymethylene-copolymer were only slightly diminished by 10 to 15%. The histological investigation of the surrounding tissue demonstrated a very good bio-compatibility of polyethylene. After an implantation of 4 weeks polyoxymethylene-copolymer, polyethyleneterephthalate and polytetrafluorethylene produced a comparable foreign body reaction, which, however, was evidently diminished after an implantation of 12 weeks.
Although the heat-ethanol method of fractionation for the isolation of normal serum albumin (NSA) is technically easier and more economic than cold-ethanol methods, globulin removal and albumin concentration remain basic hinderances to a truly simplified procedure. Continuous flow centrifugation for the separation of precipitated proteins has been effectively replaced on the one hand by alluvial filtration (globulins), and onthe other hand by diafiltration (albumin). Through these changes, investive and running costs are further reduced, noise is all but eliminated, albumin yield is somewhat increased, and personnel required for a 600 1 plasma batch is reduced from four workers to one.
At the present time there is an enormously increasing demand for albumin. The most common procedure for the isolation of this plasma component is the cold ethanol technique developed by Cohn. Because this process necessarily isolates other blood components for which there is less demand in relation to albumin, albumin production is expensive. Therefore, we have developed a two-step fractionation for the isolation of albumin. It is basically a heat precipitation method with the albumin yield being about 90% of the original plasma albumin. In comparison to cold ethanol methods, it is considerably less expensive. Other blood components, e.g., clotting factors, immunoglobulins, may also be isolated. A nonmodified gamma-globulin for intravenous use is obtained by removing anti-complementary activity with hydroxyethyl starch. Additional fractionation steps are required to isolate these other components, but unlike in established methods, these are not necessary for the isolation of solely albumin.
The bio-compatibility of different plastic materials were examined after their implantation in the back-muscle of 6-months-old male rabbits. Twelve weeks after the implantation chemical measurements (DNS-concentration of the tissue) and histological investigations of the surrounding tissue demonstrated for polyethylene the best bio-compatibility.
The availibility of a non-modified gamma globulin for intravenous use should abolish the intramuscular administration of Cohn standard gamma globulin and replace modified products to the greatest possible extent. The product developed in our institute is purified from Cohn fraction II using HES and aluminum silicate for the elimination of aggregates. The product shows excellent purity, stability, and clinical compatibility.

Mathematical and experimental results demonstrate that the customary Redon drain as well as the improved Neo-Redovac drain develop suction only over a short initial distance. The above mentioned innovation of three additional oval holes does not increase efficiency. The disadvantage of rapidly diminished suction is overcome in the "Ulmer Drain", in which equidistant openings of continually increasing aperture are aligned towards the drain end.

Different types of autogenous cancellous grafts were inserted into both femora of 20 sheep subsequent to special preparation or lethal radiation. The results were indicative of the primary role of the amount of grafted cancellous substance rather than the vitality of the transplanted cells. As compared to the outstanding effectiveness of the muscle-periosteum-flap with its revascularizing ability the significance of the compact receptor bed is minimal. These facts enhance the importance of early formation of vessels and the quantity of the osseous fraction of the substance grafted.
