Cosmetic extensile exposure of the facial skeleton.
Professor A. K. Henry coined the term 'extensile exposure'. Three incisions are described to expose the facial skeleton. The incisions are placed to best cosmetic advantage and provide extensile exposure to fit many of the routine procedures undertaken by the oral and maxillofacial surgeon.
Experience with ITI osseointegrated implants at five centres in the UK.
Experience with the two-part ITI titanium implant system at five centres in the UK is reported. A total of 461 implants were inserted in 176 patients, who received 189 prostheses. Criteria for the assessment of success following implantation are listed. Results over a 3-year period showed a success rate of 94% when implants were used for mandibular overdentures and crown and bridge prosthetics. A lower success rate in the edentulous maxilla was recorded.
Practical mandibular implantation.
The author's experience with an implantation system for use in the mandible, mainly for the support of lower dentures, is described and the results, over the last four years are analysed.
Some advances in endosseous implants.
A major advance in endosseous implantology hinges on the coating of titanium implants by porous biodegradable, bioreactive tricalcium phosphate ceramic. When implanted into alveolar bone by a careful technique, the ceramic is resorbed to be replaced by bone. As a result, the endosseous implant becomes ankylosed to the bone. This represents a stable implant/bone interface as opposed to the unstable connective tissue interface that encapsulates many other implant devices.
A new implant philosophy.
This form of tooth implant appears to exhibit great potential since it eliminates the disorganized connective tissue layer present at the implant/bone interface of most other devices. Also, as established for other implants, as long as a tight cuff of relatively immovable tissue exists around the polished cervical collar of an implant, an effective seal against the ingress of oral fluid and toxic irritants appears to be maintained, although the nature of this seal has yet to be determined. In addition, the epimobile superstructure serves to dissipate occlusal stress on the "ankylosed" implant. The prognosis of an implant is determined in part by its associated superstructure. There is a need for research into the loading that can be accommodated by an implant. Further study is required to ascertain the full potential of this implant system.
Effect of internal irrigation on frictional heat generated from bone drilling.
Frictional heat generated during bone drilling was reduced by internal irrigation compared with external or no irrigation. This was based on an in vitro study using two bur designs at low rotational speeds to prepare experimental cavities to varying depths.
Gingival metastasis from primary hepatocellular carcinoma. Report of a case.
A case of primary hepatocellular carcinoma metastatic to the gingiva is described. Hepatocellular carcinoma is an uncommon malignancy, generally occurring in a cirrhotic liver, which rarely metastasizes to the maxillofacial area. Of eight such cases in the English-language literature, the present case is the fourth involving metastasis to the gingiva. Hepatocellular carcinoma would seem to metastasize with equal frequency to the gingiva and to the mandibular bone. In the case described, histologic examination of the gingival lesion definitively established the diagnosis following somewhat equivocal results of needle biopsy of the liver.
