[The palliative intubation of malignant esophageal stenoses by means of a spiral tube. Indications--technic--results].

This is a 15-year-report on experience in the use of a special designed esophageal tube for the treatment of inoperable carcinoma of the esophagus and gastric cardia in 59 patients. Indications and operative procedures are described in detail. The operative mortality was 23.7%. The result was good in 66%, satisfactory in 27%, and poor in 7% of all patients. The longest survival time was 15 months. Complications of the endoesophageal intubation were dislocation, obstruction, penetration, perforation, and bleeding.

[Experience in treating hepatic coma by extracorporal liver perfusion (author's transl)].

19 extracorporeal liver perfusions have been performed for treating hepatic coma in 11 patients. No improvement was found in 3 patients, improvement in 8.5 of these 8 patients recovered full consciousness. All 7 patients with pre-existent liver disease died. Of 4 patients without pre-existent liver disease, 2 were discharged and are still alive. At present, extracorporeal liver perfusion seems to be the most promising treatment for hepatic coma. It should be undertaken at Stage IV and at Stage III with increasing prothrombin time. Pre-existent liver disease should be excluded before by laparoscopy.

The optimization of the pulse delivered by the pacemaker.

The results of stimulation threshold analysis carried out in 230 patients after the initial electrode implantation, and in 188 patients during pacemaker replacement, are presented. The electrodes investigated were the IE-60K-10 (279 cases), the IE-60-K (96 cases) and the ME-50 (26 cases). The chronic stimulation threshold voltage for the electrode IE-60K-10 (electrode surface area: 10 mm2) at a pulse width of 0.5 ms, after an electrode function time of 25 months, is 2.05 +/- 0.18 V, representing 256% of the acute threshold, and is equally as high as the chronic figure for the IE-60-K electrode (surface area: 27 mm2) measured at 2.08 +/- 0.20 V. The current threshold for the IE-60K-10 increased from 0.78 +/- 0.07 mA by 299% to 2.33 +/- 0.30 mA and was thus, as expected, lower than that of the large area electrode IE-60-K, which increased from 1.11 +/- 0.22 mA to 3.23 +/- 0.43 mA (291%). On the basis of the computation of the stimulation threshold energy and the stimulation threshold charge, a reduction of pulse duration to below 0.5 ms would not appear to make such energy-saving sense.

Renal graft evaluation with pertechnetate and I-131 Hippuran. A comparative clinical study.

This retrospective study compared standard clinical and biochemical data from 50 graft recipients against 533 I-131 Hippuran sequential scintigrams and 515 [99mTc]pertechnetate serial scintigrams. All grafts included in this study are cadaver kidneys. The majority of the studies were made during the early posttransplantation period. Anuria or oliguria of at least 4 days duration was seen in 18 patients. The study spans 574 days of oliguria during which 136 dual-tracer studies were made. I-131 Hippuran renography of functioning grafts was carried out 397 times, and the Tc-99m sequential scintigraphy 379 times. In all, 47 espisodes of actue rejection were registered clinically in functioning gfafrs, 36 of which were recognized during Hippuran renography and 38 with the pertechnetate study. False-positive errors were seen 12 times during renography. The study also demonstrated that furosemide will significantly and predictably influence renography and the pertechnetate study. This seems noteworthy since furosemide is extensively used in posttransplant management.

Histamine content and mast cells in human gastric and duodenal mucosa.

A high histamine content was found in all areas of human gastric and duodenal mucosa. In single subjects there was no general preference of histamine being located predominantly in fundus and corpus. In biopsy specimens of human corpus mucosa many histamine-containing O-PD reactive cells could be identified. In the sections treated previously with O-PD also many toluidine blue staining cells (mast cells) could be visualized. As shown by a transcription technique their place and number was the same as those of O-PD reacting cells. It is concluded that histamine in human corpus mucosa is localized exclusively in mast cells which seem to participate in acid secretion and in duodenal ulcer disease in man.

The frequency of Y chromatin-positive spermatozoa during in vitro penetration tests under various experimental conditions.

Complete analysis were carried out on the semen of 55 subjects. The semen was then investigated by using the fluorescent microscopic technique to determine the percentage proportion of Y chromatin-bearing spermatozoa. After staining with quinacrine mustard, an average Y chromatin frequency of 48.2% +/- 2.9% was recorded. With this spermatozoal material, 139 in vitro penetration tests were carried out with cervical mucus. After penetration there was a small, but statistically significant, increase in the percentage of Y spermatozoa (to 52.7% +/- 3.8%). The figures thus recorded were then correlated under variable experimental conditions in addition to variable semen parameters. The ratio of X:Y spermatozoa was related to the duration of the penetration tests, varying temperatures during the penetration, varying acid values of semen and cervical secretion, and also to the value of sperm motility in the semen analysis and to cervical factors. None of the above variables had any measurable influence on the percentage distribution of X and Y spermatozoa during in vitro penetration.

Hormone control in regeneration: effects of somatostatin on appendage regeneration, blood glucose and liver glycogen in Diemictylus viridescens.

In the present communication, synthetic somatostatin, a hypothalamic factor which has a known inhibitory effect on the release of growth hormone, thyroid-stimulating hormone, prolactin, insulin and glucagon in man and other mammals, was found to have an inhibitory effect on limb and tail regeneration in adult Diemictylus viridescens, when the newts were treated with a daily dose of 3-5 or 15 microgram/animal for a period of 34 days post-amputation. At the higher dose, the animals exhibited total inhibition of appendage regeneration in a few cases and the remainder showed a considerable delay compared to the controls; none of the experimental animals reached the advanced four-digit stage achieved by the controls. Furthermore, the blood glucose and liver glycogen values in the somatostatin-treated animals were significantly lower than the control values. Mechanisms in the storage, mobilization and utilization of glucose (involving hormones) are discussed in relation to appendage regeneration in the newt and possible controls of regeneration at the level of the hypothalamus are suggested.

[Prajmalium bitartrat-induced liver damage (author's transl)].

Liver damage was found in five patients (three men, two women) who were receiving prajmalium bitartrate. The cause was proven by re-exposure which was positive in all instances. The illness tended to start with acute fever and severe upper abdominal pains. When the drug was continued in one case, laparotomy was performed for suspected obstructive jaundice. Another patient had been admitted for operation because of jaundice. On re-exposure the most striking feature was a definite rise in glutamate-dehydrogenase. Prajmalium bitartrate is thus no better tolerated by the liver than ajmalin: both clinically, biochemically and morphologically the two drugs affect the liver similarly.

[Electrotherapy of life-threatening arrhythmia in acute myocardial infarct].

As a result of an adjustment of the experiences which were made in four clinics with the electrotherapy of life-threatening disturbances of rhythm in acute myocardial infarction recommendations for the use of this therapy are given. The value of the various methods of electrotherapy compared with medicamentous treatment are elaborated.
