Scaling of disfigurement and dysfunction in postoperative head and neck patients.

The primary purpose of this investigation was to construct a quantitative scale to measure the perception of severity of visible disfigurement and dysfunction following ablative head and neck cancer surgery. Facial disfigurement and disruption of physical function induced by head and neck surgery pose formidable obstacles to the medical and psychosocial adaptation of cancer patients. Although this problem is widely acknowledged, virtually no research has attempted reliable and valid measurement of such concepts. Consequently, the present study investigated the psychological perceptions of 100 registered nurses regarding 11 disfiguring surgical procedures and loss of associated functions. Results indicated that the nurses' judgments of disfigurement and dysfunction were highly consistent and not a function of patient gender. Judgments of the severity of disfigurement, moreover, were independent of the perceived severity of associated dysfunction. It was concluded that this is a reliable method for measuring the relative degree of disfigurement and dysfunction and represents a critical precursor to prediction of a patient's psychological rehabilitation and compliance with medical care after surgery.

Symmetrical lupoid onychodystrophy in dogs: a retrospective analysis of 18 cases (1989-1993).

A unique, symmetrical onychodystrophy is described in 18 dogs. A rather sudden onset of onychomadesis is followed by chronic onychodystrophy affecting all claws. Pain and lameness are recognized in half of the patients, but the dogs are healthy otherwise. Histopathologically, this disorder is characterized by hydropic and lichenoid interface dermatitis. Nine dogs were treated with a commercial, fatty-acid supplement and had good-to-excellent responses. Due to the clinicopathological characteristics of this disorder, the authors propose the name "symmetrical lupoid onychodystrophy."

Tetracycline and niacinamide for the treatment of sterile pyogranuloma/granuloma syndrome in a dog.

A sterile pyogranuloma/granuloma syndrome in a dog is described. Diagnosis was based on cytological examinations of the skin and lymph nodes and histopathological examinations of the skin and nictitans. Although the condition initially was responsive to large doses of glucocorticoids, it subsequently was treated successfully with tetracycline and niacinamide. The excellent responses of this dog suggest that this drug combination may be a viable therapeutic option for dogs in which glucocorticoids cannot be used.

Anxiety, critical thinking and information processing during and after breast biopsy.

State anxiety (STAI), critical thinking ability (CTA), and judged duration were tested for coping relatedness and change over time in 85 women, aged 18 to 60, who were experiencing breast biopsy. Participants were tested after hospital admission but before diagnostic results were known. Six to eight weeks later, women whose results were benign were tested again--when an acute crisis is considered concluded. Findings revealed extremely high state anxiety levels prior to biopsy and compromised reasoning ability at a critical time when demands on cognitive functioning were high.

Stress-coping response to genitourinary carcinoma in men.

In this descriptive study of 30 men with chronic genitourinary cancer, subjects were found to have minimal emotional distress in response to a periodic diagnostic procedure (cystoscopy). State anxiety levels and critical thinking ability measured preprocedure were not significantly different from levels obtained six to eight weeks postprocedure in subjects with negative biopsy results. Routine hospitalization for diagnostic monitoring was found, during COPE interview (Weisman & Worden, 1976), to be less stressful than regular work or financial and family concerns. Subjects with higher concurrent life stress had higher state anxiety and achieved less resolution of problems. Although anxiety levels for the group were within normal limits, 33% of the sample were categorized as having high anxiety on at least one testing occasion. This high-anxiety group had more severe behavioral responses, including feelings of depression, helplessness, and inability to set priorities.
