Use of atracurium during major abdominal surgery in infants with hepatic dysfunction from biliary atresia.

The use of atracurium during major abdominal surgery was assessed in 20 infants with hepatic dysfunction caused by biliary atresia. An initial dose of 0.6 mg kg-1 provided excellent intubating conditions in all patients. Subsequent neuromuscular blockade was monitored with a peripheral nerve stimulator and incremental doses of atracurium were given on reappearance of a single twitch. Neuromuscular conduction was allowed to return at the end of surgery and by careful timing of increments it was necessary to antagonize the neuromuscular blockade in only two patients.

The crystal structure of beta-lactamase from Staphylococcus aureus at 0.5 nm resolution.

The preparation, crystallization and low-resolution structure determination of beta-lactamase (EC 3.5.2.6, 'penicillinase') from Staphylococcus aureus is described. The enzyme crystallizes in space group I222 with 1 molecule per asymmetric unit and cell dimensions a = 5.45(1), b = 9.39(1) and c = 13.87(2) nm. The structure was determined at 0.5 nm resolution by using phases calculated from (NH4)2Pt(CN)4 and KAu(CN)2 derivatives. The mean figure of merit mean value of m, for the 1106 reflexions used was 0.70. Difference Fourier syntheses for data collected from crystals soaked in platinum D-methionine and in 6-(4-hydroxy-3,5-di-iodobenzamido)penicilloic acid revealed the likely position of the active site of the enzyme.

Comparison of i.v. glycopyrrolate and atropine in the prevention of bradycardia and arrhythmias following repeated doses of suxamethonium in children.

The effectiveness of administration of glycopyrrolate 5 and 10 micrograms kg-1 and atropine 10 and 20 micrograms kg-1 i.v. immediately before the induction of anaesthesia, to prevent arrhythmia and bradycardia following repeated doses of suxamethonium in children, was studied. A control group was included for comparison with the lower dose range of glycopyrrolate and atropine. A frequency of bradycardia of 50% was noted in the control group, but this was not significantly different from the frequency with the active drugs. Bradycardia (defined as a decrease in heart rate to less than 50 beat min-1) was prevented when the larger dose of either active drug was used. It is recommended that either glycopyrrolate 10 micrograms kg-1 or atropine 20 micrograms kg-1 i.v. should immediately precede induction of anaesthesia, in children, if the repeated administration of suxamethonium is anticipated.

Letters and shapes: developmental changes in search strategies.

In search tasks that are otherwise identical, adults process strings of letters differently from strings of other shapes (Hammond & Green, 1982). This result has implications for word recognition and reading, and it is important to establish its developmental sequence. Two groups of primary school children and an adult control group completed a similar visual search task: determining whether a predesignated target character occurred in a subsequently displayed character string. The mean search latency decreased with age, and the results showed a qualitative difference in the processing of letters and shapes for even the youngest group. However, the left to right processing of letter strings does become more established with age.

How to make a good cup of tea: exploring the scripts of thought-disordered and non-thought-disordered patients.

Thought-disordered and non-thought-disordered schizophrenic patients, psychometrically defined, were compared with control patients on a task requiring them to describe how to make a good cup of tea. Schizophrenic patients produced less detail on how to prepare tea and thought-disordered patients, in particular, omitted optional actions involved in serving tea. A number of alternative hypotheses for incoherence are considered in terms of the data and the cognitive complexity of a mental representation is advanced as one crucial factor.

The significance of western equine encephalomyelitis viral infections in Aedes trivittatus (Diptera: Culicidae) in Iowa. I. Variation in susceptibility of Aedes trivittatus to experimental infection with three strains of western equine encephalomyelitis virus.

F1 pregnancy obtained from field-collected Aedes trivittatus were evaluated for susceptibility to infection with western equine encephalomyelitis (WEE) virus by intrathoracic inoculation and by oral imbibition of virus-blood suspensions through a membrane. Mosquitoes were uniformly susceptible to infection by intrathoracic inoculation of three strains of WEE virus, but minimum infective doses varied as much as 2,000 to 12,000-fold between strains by membrane feeding. Dose-response data obtained by membrane feeding also indicated that field strains of A. trivittatus were quite heterogeneous in their susceptibility to WEE virus since some individual mosquitoes could be infected by ingestion of low virus concentrations while others could not be infected by a 20,000-fold increase in virus concentration. Moreover, A. trivittatus showed a greater affinity for a WEE viral strain isolated from this species than for a WEE viral strain isolated from Culex tarsalis, even though the site, date of collection, and passage history of these isolates were identical. Field strains of A. trivittatus were relatively refractory to oral infection with WEE virus.

The reaction of cow beta-lactoglobulin with tetracyanoaurate(III).

The reaction of cow beta-lactoglobulin with Au(CN)-4 is shown to label the free sulphydryl group on the protein by a reductive mechanism yielding a stable complex. High salt concentrations are shown to affect the pH-dependent conformational transition (N in equilibrium R yields S). It is hoped that Au(CN)-4 may be of more widespread use as a specific heavy-atom label for the isomorphous replacement method.
