[The current concept of tumor markers in oncological urology--cell surface markers].

Basic problem of the diagnosis and treatment of superficial noninvasive bladder cancer is the absence of criteria for its potential invasiveness. In recent years it became clear that cell differentiation in some cancer forms essentially depends on the presence or absence of some antigens on the cell surface. These may be demonstrated by means of appropriate methods and may thus serve as marker in determining the prognosis and the stage of the disease and for monitoring the therapy in patients with bladder carcinoma. Of major value for the clinical practice are the clinical practice are the T-antigen, the ABO(H) antigens and the tumor-assisted antigens. Demonstration of T-antigen is effectuated by means of a modified erythrocyte-binding test and of the ABO(H) antigens by the Davidsohn's test. Correlation of these antigens with the pathological and clinical stage in a series of studies has demonstrated their practical significance in choosing an optimal therapeutic plan. There also exists relationship between the blood group antigens and the degree of malignancy in urothelial tumors. This allows a more precise evaluation of the indications for organ-preserving operation in some cases.

[The diagnostic value of testicular biopsy in andrologic practice].

Results are reported of 41 testicular biopsies in patients with azoospermia. The results are classified in two basic groups: gr. I--17 patients (41.6 per cent) with azoospermia had preserved spermatogenesis, but the seminal ducts were obstructed as a result of chronic inflammatory process; gr. II--24 patients (58.5 per cent) with incretory azoospermia and absence of spermatogenesis, caused by the following testicular pathology: Sertoli cell syndrome following orchidopexy for cryptorchidism, Klinefelter's syndrome, postparotitis orchitis and a group of patients with primary hypogonadism. The technique of open testicular biopsy as a possible routine diagnostic method in andrology is described.

[A new approach in the diagnosis of obstructive azoospermia].

The author describes a new approach to the diagnosis of obstructive azoospermia. The method consists of administration of Panetblau in the seminal duct by an operative way and blue staining is anticipated in urine. This method gives a possibility to examine the patency of seminal pathways at proximal direction. This method was used in 78 patients with mean age of 29 years, suffering from chronic nonspecific epididymitis and obstructive azoospermia and fertile problems. Allergic reaction, caused by Patenblau occurred only in one patient. The proposed method has the following advantages: it is an objective method for establishment of the patency of the seminal pathways, the drug used is water solution, which does not irritate and does not obstruct the lumen of seminal duct, and this method avoids roentgenological irradiation of patients with disturbed spermatogenesis and has a definite economic effect as well.

[The conservative treatment of Peyronie's disease with orgotein].

Results are analyzed of treatment of patients with induratio penis plastica by local application of orgotein. Fifteen patients with clinically manifest disease were the subject of this study; 4-8 mg Orgotein bulk was applied once a week in a total course of 8 to 10 injections. Criteria for the effect of treatment were: regression of the plaque consistency and size; decrease in the erectile deformity; change or disappearance of pain. Seven patients (46.7 per cent) experienced considerable improvement. Absence of effect was observed in one third of the patients. Quickest to be affected was pain and the penetration possibility, while the erectile deformity regressed at a slower rate. No side reactions or intolerance to the preparation were observed. Essential for the occurrence of therapeutic effect was the use of appliance for injection under high pressure. It this treatment fails, surgical management should be considered.

[The surgical treatment of Peyronie's disease by Nesbit's technic].

In 1965 Nesbit described an operative technique for correction of congenital penis distortions. Twenty eight patients with induratio penis plastica and marked erectile deformity were operated and followed up at the Department of Urology. All had previously received conservative therapy without effect. Indication for surgical intervention was the inability to perform coitus. The original method was applied: an elliptical opening was excised in tunica albuginea, contralateral to the distortion. In other cases compensation was performed by plication of tunica albuginea without opening the cavernous space. The result was satisfactory in 75 per cent of the patients and failure in 25 per cent. The latter was attributed to loosening of the correcting sutures or great rigidity of the plaque. The encouraging functional results and the rather uneventful postoperative period allow the authors to recommend Nesbit's operation for treatment of some forms of Peyronie's disease, which are not affected by conservative treatment.

[Retrograde ejaculation following transurethral surgery for bladder neck sclerosis].

A brief review of the literature on the physiology of ejaculation and the causes and treatment of retrograde ejaculation is made. Retrograde ejaculation following transurethral resection for bladder neck sclerosis is discussed in detail. Seventy four patients were under control for a period of 5 years--69 after transurethral resection and 5 after transurethral incision of the bladder neck. Retrograde ejaculation was observed in 8.6 per cent of the patients. The possibility to solve the fertility problems in young men, as well as to avoid this complication is discussed. Inferences are made and recommendations given for establishing the diagnosis, treatment and prophylaxis of this complication.

[Physiological, psychological and ergonomic research on jobs in the manufacture of chocolate products].

The investigations include job description and analysis of the activity, functional examinations of basic dynamometric indices (muscle strength and endurance of static effort), the pulse rate and the time of carrying out of one operation and for manufacturing of unit product. Psychological tests for subjective evaluation of the stress and monotony, the type and rate of vertigo, of the psychic status in the presence of monotony, are applied. The ergonomic analysis comprises evaluation of the work place work posture and equipment. The results of the investigations point out that the activity of the basic professions in this production is expressed monotonous, with significant static load of the muscle system and total hypokinesia and hypodynamia. Changes in the working capacity and fatigue are registered in the middle of the working day as well as disturbances in the neuro-vegetative and adaptation mechanisms of the organism and appearance of unfavourable psychic conditions (decreased vigilance, boredom, satiating). Ergonomic incongruities were found in the dimensions and the order of the working place and equipment.

[The noise factor in the manufacture of chocolate and pastry products].

Studies are performed on the noise and vibration condition and hearing in women workers in enterprises for confectionery. It is established that the noise is leading occupational hazard in the working environment and in half of the working places significantly surpasses the sanitary norms and the peak values reach sometimes to 102-105 db/A. Especially intensive sources of noise are some machines and industrial operations-vibratory tables, mixers, cocoa-rollers, grinder for sugar and cocoa, the operation "hammering of forms". The vibrations (general) are rarely met hazard in industry. A considerable decrease of hearing in the examined women workers is established.
