Release of nickel into fluid stored in the reservoir of Dermo-Jets.

Release of nickel into triamcinolone acetonide suspensions stored in the reservoir of a Dermo-Jet was demonstrated by electrothermal atomic absorption spectrometry. Increased urinary nickel excretion was found for three days following intradermal injections (with the Dermo-Jet) of triamcinolone acetonide in patients suffering from alopecia areata. The dimethylglyoxime test showed that the internal rod of the reservoir as well as metallic internal parts of the nozzle leached appreciable amounts of nickel, even before any clinical use.

Alopecia Areata↗

An anatomo-clinical study of delayed skin allergic reactions to nickel following intradermal injections of lidocaïne with a Dermo-jet.

10 women allergic to nickel developed skin reactions at injection sites with a Dermo-Jet (Krantz model) of (a) 2% lidocaine solution and (b) 0.9% saline solution. 10 women (age-matched) not allergic to nickel were selected as controls and submitted to the same injections; they had no positive reactions. Various controls (including injections with a needle and patch tests) were made in both groups. Nickel was leached into fluids from the metallic internal parts of the Dermo-Jet. The positive reactions were allergic and due to nickel. Histology showed changes of an allergic contact dermatitis with particular features, probably due to the intradermal injection of nickel. Some practical implications regarding the quality of medical instruments are discussed.

Polyvinylpyrrolidone (povidone) storage in RX-damaged skin following procaine-povidone intramuscular injections.

A man, aged 67, has been treated in 1968 by X-rays for a basal-cell carcinoma of the back. The treatment was followed by an ulcerative radiodermatitis. 3 years later, the patient was treated by ten intramuscular injections of procaine-polyvinylpyrrolidone (or procaine-povidone) for sciatic neuralgia. An orange infiltration appeared progressively around the radiodermatitis. The area was excised in 1979 and polyvinylpyrrolidone (povidone) storage, responsible for the ring-shaped orange hue, could be demonstrated in the dermis and subcutaneous fat. Special stainings confirmed the presumed chemical nature of the deposits. 3 cases of povidone pseudotumours following procaine-povidone injections had been described previously. The preferential deposit of povidone in RX-damaged skin is discussed.

Does mast cell protection plus mediator antagonism surpass the effect of a classic antihistaminic in the treatment of chronic urticaria? A double-blind comparison of oxatomide and mequitazine.

A new type of antiallergic drug, oxatomide (60 mg daily), was compared with the antihistaminic, mequitazine (10 mg daily), in a 5-week double-blind randomized study involving 26 patients with chronic urticaria. In the 13 oxatomide-treated patients, papules, erythema and pruritus were significantly suppressed after 2 weeks, which was markedly more rapid than with mequitazine. Slight somnolence was reported by 3 patients on oxatomide and 1 on mequitazine; in the latter case, severe urine retention was also reported. Another patient on mequitazine had to discontinue treatment because of severe drowsiness.

[Permanent tattoos following injections with Dermo-Jet: anatomoclinical study. Apropos of 5 cases].

Blue or black tattoos were observed in five patients who had received several intradermal injections (of a lidocaine solution or of a triamcinolone acetonide suspension) with the Dermo-Jet. A histological examination revealed the presence of black masses, rounded or elongated, different in size, distributed throughout dermal tissue. Additionally, conglomerates of black grains in the cytoplasm of histiocytes and of pericytes are precisely observed in semi-thin sections. It has been proved by several investigations that these foreign particles are not of metallic nature. It can be concluded from electron microscopic studies that the particles are fragments of black rubber, from the upper joint of the reservoir. Indeed, some rubber fragments are leached into the reservoir. Some of these are passing through the wire-mesh filter of the nozzle (with the solution or the suspension) when injections are made. Therefore, they are injected intradermally and are permanently tattooing the skin.

Eau de Javel and prevention of chromate allergy in France.

"Eau de Javel" (liquid bleach) is a common cause of chromate contact allergy. A historical review explains the reasons for the addition of sodium dichromate after the First World War. The chromium content in different brands of eau de Javel has been determined by flameless atomic absorption spectrophotometry. The results indicate that the chromium content is very low (0.03--3.65 mg/l) in French brands, whereas it is still important (7.06--82.9 mg/l) in Belgian brands, such an observation indicates that the almost complete removal of chromate in most brands of French eaux de Javel is a good example of prevention in the field of contact dermato-alllergology.

[Multiple thoraco-abdominal paraffinomas (author's transl)].

We report the case of a man aged 39 who received several injections of mineral oil in his pectoral muscles 20 years ago. Supposedly, the purpose of the injections was to improve the aesthetic appearance as well as the strength of the muscles. The final result was the development of impressive symmetrical paraffinomas. A short review of the recent literature on paraffinomas is presented.

Epidemiological survey of occupational contact dermatitis of the hands in Belgium.

A total of 400 patients suffering from hand eczema were patch tested to 25 chemicals (20 from the standard series of the ICDRG plus five from the Tri-Contact of Belgium); 197 (49.2%) had a positive patch test to one or more substances, and there was no significant difference in the prevalence between males and females. Potassium dichromate and PPD mix were statistically more frequently involved as skin allergens in males than in females, whereas nickel sulphate, thiuram mix and neomycin sulphate gave a significantly higher number of positive responses in women than in men. Results of this epidemiological survey are discussed in the light of similar studies from the literature.

Dermatoses of the neck affecting violin and viola players ("fiddler's neck", and contact dermatitis).

Skin lesions occurring on the neck of violin or viola players are described, in the light of two personal observations. The lesions can be divided into two types: 1. Lesions described as "fiddler's neck", i.e. an area of erythematous, sometimes pigmented or scaly lichenification on the left side of the neck, just below the angle of the jaw, where the chin rest of the instrument is in contact with the skin. Histopathologic features of the lesions are described. 2. Allergic contact dermatitis to wooden or metallic parts of the violin. The recent literature is reviewed.

Low allergenicity of triclosan. Predictive testing in guinea pigs and in humans.

Allergic predictive testing with triclosan (Irgasan DP 300) has been carried out in guinea pigs and in humans in order to evaluate its potential allergenic properties. Triclosan, an antimicrobial agent increasingly used in the cosmetic industry (concentration 0.5%) has recently appeared on the market at a higher concentration (3%) in association with flumethasone pivalate as an antimicrobial and antimycotic cream (Logamel). The results of predictive tests for allergy in guinea pigs (1 of 20 positive) and in humans (none of 20 positive) indicate that the sensitization index of triclosan is very low, which is in good accord with some reports from the literature. These underline the rarity of cases of allergic contact dermatitis to triclosan considering its widespread use in the cosmetic industry.

[Spectacle-frame acanthoma. Review of the literature and histopathological diagnosis versus chondrodermatitis nodularis helicis (author's transl)].

Five cases of spectacle-frame acanthoma are reported; four were retro-auricular (three unilateral; one bilateral) and one was present on the left side of the nose. Clinical aspects of the lesions are described by the light of all the other cases that have been previously reported in the literature. Histopathological features are compared to those encountered in chondrodermatitis nodularis helicis for which a frictional origin may also be suspected. In the vast majority of cases, spontaneous resolution is obtained when responsible spectacle frames are removed whereas a few lesions persist indefinitely despite of the fact that corrective measures are applied, on the contrary to what has been reported in previous papers. Apart from removing spectacles, these few cases needed surgical excision for a complete cure. Anatomo-clinical confrontations seem to indicate that those persistent lesions are completely fissured acanthomas.

Occupational allergic contact dermatitis to isophorone diamine (IPD) used as an epoxy resin hardener.

Three patients employed in the manufacture of plastic tennis rackets developed allergic contact dermatitis to isophorone diamine (IPD), an epoxy resin hardener, and concomitantly to epoxy resin. Patch tests were positive to IPD at 1, 2 and 5% in olive oil and to IPD at 1, 2 and 5% in ethanol. Investigations were conducted in control subjects to confirm the allergic nature of reactions. A review of the literature about IPD and related compounds is presented.

A modified technique (using polyester tape) of skin surface biopsy. Its interest for the investigation of athlete's foot.

A technical modification of skin surface biopsy has been introduced by using plastic tape instead of glass as holder, mainly to investigate mycological infections of skin folds. Among various brands of plastic sheets, a polyester film (Melinex O UCB-SIDAC) has been demonstrated as the most suitable. A direct microscopic comparison has been made between our modified technique and conventional scraping as procedures for collecting material from interdigital spaces in 30 patients with bilateral athlete's foot. It has been shown that the skin surface biopsy gives a slightly greater number of positive results (presence of dermatophytes or Candida species) than the conventional scraping technique, although the difference between both techniques is not statistically significant at the 0-05 level (0-05 less than P less than 0-10). Some advantages of the modified skin surface biopsy are emphasized.
