Laparoscopic surgery of focal lesions of the liver.

In this work the laparoscopic surgical approach of two benignant hepatic lesions (focal nodular hyperplasia [1st case], and hepatic cyst with simultaneous cholecystolithiasis [2nd case]) will be discussed. The authors present the steps of the laparoscopic procedures: non-anatomical liver resection in the first, resection and cholecystectomy in the latter case. During the operations no surgical complications occurred, the postoperative course was also uneventful. The patients were discharged in the fifth day after the surgery, without any complains. During a short follow-up period, our patients were asymptomatic and free of recurrences. In our opinion, the laparoscopic hepatic surgery might be a feasible technique in the hands of surgeons with hepatic and laparoscopic experience, but careful selection criteria should be followed in the selection of such cases.

[Hysteroscopic resection of submucosal myomas in abnormal uterine bleeding: results of a 4-year prospective study].

OBJECTIVE: The evaluation of the effect of transcervical resection of submucous myoma/s in patients with abnormal uterine bleeding. DESIGN: Prospective clinical study. SETTING: Department of Obstetrics and Gynaecology, 1st Medical Faculty, Charles University and General Faculty Hospital in Prague, Czech Republic. METHODS: 45 patients with resected submucous myoma/s within the period 1995-1998 were selected. Patients in whom resection of myoma was combined with endometrial ablation were excluded. The average age of the cohort was 43 years (29-53). In 37 patients (82%) therapeutical curettage was performed for severe bleeding in the past (3 procedures on average, interval 1-7). In 16 patients (36%) transfusion was administered in the treatment of anaemia secondary to abnormal uterine bleeding in the past. Hysteroscopy, ultrasound scan or both techniques were used to diagnose and classify myomas. In 39 patients 1 myoma was resected, in 6 patients 2 myomas. In 12 (27%) patients myoma of 0., in 29 (64%) of I. and in 4 (9%) of II. has been diagnosed according to the ESH classification. In case of several myomas, the classification was based on the one of the highest degree. Surgery was performed in the early proliferative phase of the menstrual cycle. Purisol (sorbitol and mannitol) was used as a distension medium. Myomas were resected using loop high frequency electroendoresection technique. In 3 (7%) patients myomas of ESH II. were resected in more steps--in 2 cases (5%) in 2 steps and in 1 case (2%) in 3 steps. In patients with myomas classified as a 0. and I., only the character of menstrual cycle has been monitored after surgery. Ultrasound investigation was performed in patients with myomas of II. 3 months after surgery. RESULTS: The bleeding was controlled in terms of eumenorrhoea or hypomenorrhoea in all 45 patients (100%). Ultrasound investigation showed no residual myoma in any of 4 patients after resection of II. myoma. One case fluid overload syndrome of minor degree was documented as the only complication. CONCLUSION: Hysteroscopic high frequency endorsection is a safe and effective method and method of choice in the treatment of submucous myomas in patients with abnormal uterine bleeding. The high success rate in our cohort is certainly influenced by the number of patients included and by the length of follow-up.

Protein expression in the stressed Vibrio strains.

In a conjunction process using Escherichia coli SM10 (pLOF) KmR APR as donor and Vibrio S141 SmR as recipient, several mutants were constructed: Vibrio PH 101, V. PH 106, and V. PH 109 with lowered ability to synthesize poly-beta-hydroxybutyrate. The survival and metabolic activities of parent and mutant strains were estimated when they were subjected to stress conditions (starvation of carbon and energy sources and/or cadmium treatment). Using two-dimensional electrophoresis, the synthesis of stress proteins was demonstrated. Vibrio cultures consecutively exposed to CdCl2 and then to starvation or vice versa responded similarly metabolically. These results show increased proteosynthetic activity of the stressed Vibrio cells, indicating that the primary cadmium treatment induced the expression and synthesis of the protective proteins, enabling the cells to cope with the secondary stress.

Metabolic characteristics of bacterial cells entrapped in beaded calcium alginate and/or pectate gels.

A mixture of heterotrophic bacteria and collection strains of Escherichia coli and Pseudomonas fluorescens were immobilized in calcium alginate or pectate gels. Comparison of respiratory activity, substrate uptake and biosynthetic capacity of immobilized cells showed that both types of carriers permit a prolonged preservation of metabolic activity but the transfer of substances through the gel is faster in the pectate. Morphological changes include some intracellular structures, partial shrinkage of the plasma membrane of immobilized cells, and transformation of a rod-like cell shape to an oval one.

Effect of chemical water pollution on the physiological activity of microorganisms.

Anionactive detergents occurring in water in concentrations up to 100 micrograms . ml-1 do not exert inhibitory effects on the present microflora, either in case of heterotrophic bacteria, indicators of fecal pollution, or pathogenic and facultative pathogenic bacteria, including viruses. On the contrary, the studied bacterial species are able to decompose surfactants and utilize them as carbon source. Though, the rate of degradation is different with respect to respective substances, as well as with regard to the species of organisms, and their physiological activity. Only at higher detergent concentrations inhibition of microbial cell growth may be observed.

[To the multiplication ability of intestinal bacteria in water (author's transl)].

In three series of laboratory tests, the influence exercised on intestinal bacteria by waste water from dairies, breweries, starch factories and the meat-processing industry, as well as by the individual potential nutritive substances such as casein, yeast autolysate, starch and bovine serum was examined in native and sterilised surface water. This was the continuation of the experiments with waste water from sugar factories. All substances tested so far can being about a growth of potentially and obligate pathogenic bacteria. However, the influence exercised by the different types of waste water and their potential nutritive substances varies. Yeast autolysate has the most striking effect, e.g. E. coli grow in the order of 4-5, P. aeruginosa in the order of 2-3, and in the order of 1-2 in the case of the 2 serotypes examined, S. enteritidis and S. schottmuelleri. A lesser effect was produced by casein, while the influence exercised by starch and bovine serum was minimal or nil. When nutritive substances are again added, the growth of all test organisms examined is less marked and it is mainly attributable to the action of yeast autolysate and casein. However, the possibility of a secondary increase of the potential infectiousness of the water under the influence of specific trophic conditions of the outfall, was clearly demonstrated.

Peritoneal washing cytology on fluid hysteroscopy and after curettage in women with endometrial carcinoma.

OBJECTIVE: To assess the influence of fluid hysteroscopy with target biopsy of the endometrium and the influence of added curettage on the results of peritoneal washing cytology (PWC) in endometrial carcinoma. STUDY DESIGN: In 42 women at risk of endometrial carcinoma, we performed fluid hysteroscopy with target biopsy of the endometrium and curettage. Evaluation of PWC of the pouch of Douglas was performed three times during the procedure: prior to hysteroscopy, after fluid hysteroscopy with target biopsy and after curettage. RESULTS: On cytologic slides from peritoneal washings in 11 patients with carcinoma of the endometrium, malignant endometrial cells were found after curettage in 72.7%. There was no statistically significant difference in PWC prior to hysteroscopy (two women, 20%) or after hysteroscopy with target biopsy (three women, 30%). There was a statistically significant difference (.05 level) in positive PWC after hysteroscopy with target biopsy (three women, 33.3%) and after curettage (eight women, 88.9%). CONCLUSION: Slides from carcinoma of the endometrium in PWC do not deteriorate after hysteroscopy with target biopsy of the endometrium, but tumor cells will appear in the pouch of Douglas after curettage.
