[Total ethmoidectomy using the mixed rhinoneurosurgical approach for malignant tumors of the anterior skull base. 14 years' experience. 62 cases].

Based on 62 cases, the authors report on 14 years of experience of total ethmoidectomies through a mixed rhinological and neurosurgical approach. Following a survey of the main points of the surgical technique, they emphasize the major evolution undergone, on the one hand, by the preoperative topographic diagnostic possibilities thanks to progress in imaging, and, on the other hand, the operative technique itself. The optimization of the reconstruction time for the base of the skull seems to provide a substantial advantage by reducing morbidity and operative mortality. The use of the epicranial flaps and to a lesser degree, cryo-stored bone allografts are the major stages. Prognostically speaking, the mixed approach offers a clear improvement in the 5-year survival rate of adenocarcinomas (80%), and olfactory placode tumors. However, the prognosis for epidermoid carcinomas continues to be poor (35% at 5 years) due to the frequency of local recurrences and visceral metastases.

[Treatment of subglottic stenoses using the Pearson's operation: apropos of 3 cases].

Surgery of stenoses of the laryngo-tracheal junction is governed by the necessity of maintaining the support of the cricoid-arytenoid articulations and of preserving the recurrent nerves. Sub-total cricoid resection with conservation of the recurrent nerves, known as Pearson's operation, enables the treatment of fixed sub-glottic stenoses with mobile larynx. On the basis of three recent observations, the authors define the indications and the performance of this surgery. The respiratory and vocal results are considered satisfactory in these three cases.

[Progressive facial palsy. 2 recent cases].

Following 2 recent observations of progressive facial paralysis, the authors discuss the diagnostic problems encountered and the compressive etiology observed frequently. Extrinsic nerve damage calls for rapid care but should not cause the existence of neurinomas of the 7th nerve, more frequent than suspected, to be overlooked. Facial paralysis is the most frequent inaugural symptom, however the neurinomas of the 7th nerve may be preceeded by a pure cochleovestibular syndrome or a tumoral syndrome of the middle ear. They can appear at any place on the nerve, however the tympanomastoidal locations are most frequent. The approach of these tumors depends on their position, size and the patient's hearing. Nerve rehabilitation can be obtained using modern derouting techniques, grafts of sometimes hypoglossalfacial anastomosis.

[Contribution of immunohistochemistry to the diagnosis of cervicofacial cancers].

In spite of classical morphological studies, a good number of cervico-facial cancers remain classified as undifferentiated. Thanks to the use of intracellular antigens, immunohistochemistry (IHC) allows for a differentiation of tumoral growths. 33 patients were used in this study within the last three years. After studying the different markers used, the authors consider their specificity and their reactivity, than their respective usefulness in the three main diagnostic cancer of the face. In 22 patients, IHC alone allowed a diagnosis and a specific therapeutic orientation. Material, technical and human demands must be considered along with the efficacity to the technique.

[Primary malignant melanoma of ORL mucosa. Apropos of 5 cases].

Based on five observations of primary mucosal melanomas recently treated in the department, the authors describe the main features of this very rare condition, and particularly the evolutive aspects in diagnosis and prognosis. Histological diagnosis is sometimes difficult, and must involve specific argentic staining and a search for the presence of the protein S100. Tumor thickness was systematically examined. No prognostic interpretation, however, is possible, given the small number of cases in the series. Evolution is unpredictable; there is very commonly a series of local recurrences which may be well controlled, followed by remote metastases remaining long quiescent, bypassing the cervical ganglion relay. Treatment is surgical whenever possible, with palliative radiotherapy for local recurrences and chemotherapy for metastatic forms. Other therapies have yet to prove their effectiveness.

[Florid oral papillomatosis: evaluation apropos of 2 cases].

Oral florid papillomatosis is a rare, so-called pre-neoplastic non-obligatory complaint. The authors, on the basis of two cases, attempt to define the concept of malignity associated with this complaint and the resulting surgical principles.

Trends in seniors' financial aid counseling and career counseling.

The responses to questions regarding financial aid counseling show a small but significant increase in the quality of counseling when compared to 1983 responses. The percent of students who received "enough" information increased, while the percent who received "no" information on financial aid decreased. In addition, seniors have a greater awareness of the broad range of topics that concern securing and managing loans. Improvement can also be seen in the schools' effectiveness in providing assistance in career and placement counseling. The percentage of seniors who said the school was "very helpful" in career choice counseling increased from 11.2 percent to 18.6 percent. In placement counseling, the percent who responded in the "very helpful" category almost doubled. Despite the improvement in counseling during the past five years, most seniors say they have not received enough information concerning financial aid, and over 80 percent of the seniors rated their career choice counseling and placement counseling as "limited" or non-existent. Clearly, from the students' perspective, the schools could be doing a better job of providing counseling services to students.
