Immunohistochemical demonstration of alpha-fetoprotein in testicular germ cell tumors.

Immunohistochemically AFP was demonstrated in testicular germ cell tumors using the PAP method after Sternberger. AFP immunoreactivity was found in the yolk sac components of combined germ cell tumors. But embryonal carcinomas showed positive immunoperoxidase reaction for AFP in the cytoplasm of single cells in the solid masses and in areas forming tubular patterns. Also embryoid bodies in a polyembryoma and in combined germ cell tumors showed an AFP reactivity. Furthermore in immature teratomas positive staining was seen in epithelial cells of primitive cysts, adenoid and gut-like structures of probably entodermal origin. The positively reacting structures of combined germ cell tumors could be interpreted as early stages of embryonal development. Thus the results support the theory of totipotency of cells of embryonal carcinoma.

[Ultrastructural cytology of testicular seminomas].

The aim of this electron microscopic study was to analyze and compare aspects of various histologically distinct seminoma variants including the classical form (11 cases), anaplastic seminoma (1 case) and seminoma with syncytiotrophoblastic giant cells. Sub-microscopic features of these seminoma types are essentially the same; these various seminoma forms can not be electron microscopically distinguished. Although the cytoplasmic structures of the tumor cells varied, the most striking feature was a high glycogen content. Three types of seminoma cells could be distinguished on the basis of the relation between glycogen accumulation and cytoplasmic organelles: 1. tumor cells with plentiful glycogen and scant organelles, 2. tumor cells packed with organelles including RER, mitochondria and Golgi-apparatus but with finely dispersed glycogen, 3. tumor cells with numerous free ribosomes but few organelles and little glycogen. Glycogenosomes are described here for the first time as distinct organelles in seminoma cells, suggesting a disturbed carbohydrate metabolism. Another typical finding was a network of fine filaments measuring about 4 to 6 nm which probably correspond to actin. By interweaving in all directions they produce a felt-like meshwork on the inner-side of the cell membrane. Based on the composition of the seminoma cell cytoplasm with its collection of special organelles including lamellae annulatae, chromatoid bodies and crystalline structures resembling Lubarsch crystals, the tumor cells may be interpreted as neoplastic germ cells related to the so-called atypical germ cells of the testis.

Transmission of Anaplasma marginale Theiler by males of Dermacentor andersoni Stiles fed on an Idaho field-infected, chronic carrier cow.

The role of ticks and carrier cattle in epizootics of bovine anaplasmosis was further clarified by demonstrating unequivocally, for the first time, that male ticks fed on a chronic carrier cow naturally infected with Anaplasma marginale can transmit this parasite intrastadially and biologically when subsequently fed on susceptible cattle. These data indicate that field epizootics of acute anaplasmosis may be initiated by males of tick vector species that feed on carrier cattle and subsequently transfer to susceptible cattle.

Epidemiologic aspects of bovine anaplasmosis in semiarid range conditions of south central Idaho.

The prevalance of Anaplasma marginale-infected cows, as determined by use of the modified rapid card agglutination (MRCA) test, was measured during a 4-year period (1980-1983). The prevalence of A marginale-infected cows, defined as positive reactors on the MRCA test, remained constant (31%-37%). The apparent incidence of A marginale transmission to susceptible cows was approximately 7% from 1980 to 1981, 8% from 1981 to 1982, and no transmission from 1982 to 1983. The occasional MRCA-positive cow became negative on the MRCA test, and 1 cow was determined to be free of A marginale infection by subinoculation of 100 ml of the cow's blood into a susceptible, splenectomized calf. Dermacentor andersoni, a known vector of A marginale, was often found on the cattle and in their environment. However, A marginale was not transmitted to susceptible, splenectomized calves, using collected ticks. Of 56 calves born to MRCA-positive cows, 82% were MRCA-positive within the first 3 months of life. These calves converted to MRCA-negative status and were determined to be free of A marginale infection by subinoculation of their blood into susceptible, splenectomized calves, indicating the passive transfer of colostral antibodies.

[Early diagnosis of primary chronic glomerulonephritis in the latency stage].

In 74 of 195 patients with isolated proteinuria and/or haematuria investigated by means of renal biopsy laser-nephelometric determinations of the concentrations of albumin, alpha2-macroglobulin, transferrin, IgG, IgA and IgM in the urine were performed. Their result was that patients with chronic interstitial nephritis or chronic glomerulonephritis by increased concentration of albumin, transferrin, IgG and IgA statistically ascertained differ from patients with normal renal tissue or slight glomerular abnormalities.

[IgM/kappa-immunoblastoma of the brain following Hodgkin's disease. Report on a clinical case and review of literature].

Report of a 41-year-old man who died from an intracerebrally localized pleomorphic IgM/kappa immunoblastoma 7 1/4 years after the diagnosis of nodular sclerosis in mediastinal lymph nodes and treatment by surgery, radiation and cytostatics. There was no evidence of residual Hodgkin's disease at autopsy. The cases of malignant non-Hodgkin's lymphomas after treated Hodgkin's disease reported in the literature are tabulated. The relationship of these secondary neoplasms to the primary disease and its treatment is discussed.

Effect of dexamethasone on babesiasis in hamsters.

Three subcutaneous injections of 0.20 mg of dexamethasone/kg of body weight caused a substantial increase in Babesia microti-parasitized RBC of hamsters, indicating that this was a useful method for revealing the presence of latent infections. A relative neutrophilia, lymphocytopenia, and eosinopenia were also seen in the long-term B microti-infected carrier hamsters after 0.20 mg or 0.02 mg of dexamethasone/kg. Noninfected hamsters treated with dexamethasone had a neutrophilic leukocytosis and a transient lymphocytopenia. Spleen to body weight ratios of noninfected hamsters decreased significantly (P less than 0.02) after 4 injections with either dosage level. These ratios did not significantly (P greater than 0.05) decrease in dexamethasone-treated infected hamsters.

Immune responses of calves antigenically stimulated and challenge exposed with Anaplasma marginale during tick infestation or treatment with dexamethasone.

Similar anamnestic antibody responses to a 2nd injection of a Anaplasma marginale vaccinal antigen were observed in calves infested with the tick Dermacentor albipictus and in tick-free calves. When challenge exposure of these calves to virulent A marginale was done, infestation with the tick Boophilus microplus increased anemia (P less than 0.01), but did not suppress antibody production to A marginale or increase parasitemia. None of the vaccinated calves, regardless of infestation, experienced clinical anaplasmosis. Mitogenic responses of lymphocytes from infested animals were unaltered by either infestation. Tick infestations did not cause immune suppression in the calves, whereas use of dexamethasone resulted in a significantly lower antibody response (P less than 0.05) after a 2nd injection of vaccinal antigen. After challenge with virulent A marginale, dexamethasone-treated calves showed more pronounced parasitemia (P less than 0.01) and anemia (P less than 0.01) than did control calves. Anaplasmosis did not prevent the calves from developing resistance to reinfestation, which was accompanied by immediate, but not delayed, hypersensitivity reactions against homologous tick extracts. Dermacentor albipictus did not seem to share common antigenic determinants with B microplus, since extracts from the latter did not elicit immediate hypersensitivity responses in calves sensitive to D albipictus extracts. Calves were somewhat resistant to reinfestation as evidenced by reduced numbers of adult fed ticks, decreased weights of ticks after feeding, and smaller egg masses.

[Ampicillin associated acute interstitial nephritis with renal insufficiency--case report].

It is reported on a 20-year-old male patient, in whom during the treatment of a streptococcal infection with ampicillin an exanthem and an acute interstitial nephritis with acute renal insufficiency appeared. The temporary connection between the application of the medicament and the occurrence of the clinical symptoms and the sensitization to ampicillin proved by the lymphocyte transformation test speak for an ampicillin-induced interstitial nephritis. The discontinuation of the treatment with ampicillin and the application of prednisolone led to a complete involution of the clinical picture. Since ampicillin is frequently used in the clinic one should take into consideration in the indication that possibly an acute renal insufficiency may appear due to an acute interstitial nephritis.

Ultrastructural and ultrahistochemical investigations of lead-induced intranuclear inclusion bodies in rat kidney.

The morphogenesis of intranuclear inclusion bodies in the proximal convoluted tubules of rat kidney is studied in chronic lead poisoning. In early stages electron dense particles and fibrillar structures in connection with interchromatin granules were observed. The fully developed inclusion bodies showed an electron dense core from which fibrils project outwards forming an irregular coronal framework. By the sulphide silver method for the demonstration of heavy metals an intense reaction was observed in the outer areas of the fibrillar corona after digestion using DNase.

[Malignant fibrous histiocytoma of bone. Clinical pathology and histological diagnosis].

Malignant fibrous histiocytoma of bone is a histologically well-defined tumor. Our aim is to describe five own cases and to analyze the published cases in order to demonstrate, the controversial aspects of clinical pathology. The essential histological criteria are the storiform tissue pattern and the presence of fibroblastic and histiocytic cells and giant cells. Inclusive of our cases, the total number reported stands at 196. There are features of malignant fibrous histiocytoma of bone about which there is almost general agreement: 1. The tumor occurs at all ages with an average onset from the age of 10 to the age of 50. 2. The tumor occurs in both the long and flat bones, but the main sites are the distal femur and the proximal tibia. 3. The tumor lacks any initial distinctive features in its clinical phase, but with respect to its biological behaviour, malignant fibrous histiocytoma of bone can be distinguished from osteosarcoma.

[Melanotic ectodermal tumor of infancy (melanotic progonoma)--an unusual soft tissue tumor].

In rare cases an utmost uncommonly pigmented lesion is found in young infants which is mostly located in the anterior maxilla. The histogenesis of this unusual soft tissue tumor has provoked a long-lasting debate, which is reflected in many synonyms. There is no anatomical precursor and the possibility of a phylogenetic ancestral form is discussed. Therefore, the term melanotic progonoma was proposed. Because of the derivation from neural crest cells the designation melanotic neuroectodermal tumor of infancy was introduced. This name is now generally accepted. In this study, two typical cases of this rare tumor are described. The tumors are composed of large epithelial-like melanin-producing cells and small nonpigmented cells, so-called lymphocyte- like cells resembling neuroblasts. The diagnostic relevant histological pattern is characterized by intensely pigmented cells arranged either in strands or clusters often forming the lining of small cleft-like or tubular spaces, or by alveolar structures surrounded by a fibrovascular stromal component. At ultrastructural level, the pigment corresponds to the cutaneous type of neural crest type of melanin. The histogenesis of these lesions and the classification of pigmented benign and malignant neuroectodermal tumors of the soft tissues are discussed especially taking into consideration the concept of the soft tissue variant of melanomas. The melanotic neuroectodermal tumor of infancy is a benign growth Only in very few cases a fatal outcome is reported in the literature. The melanotic neuroectodermal tumor of infancy must be distinguished from other types of benign and malignant neuroectodermal tumors. From histological point of view and with regard to its biological behaviour this lesion is a particular entity of pigmented neuroectodermal tumors of the soft tissues, and for subclassification the term melanotic progonoma should be maintained, too.

[Congenital hepatoblastoma of mixed type. Light and electron microscopy studies].

Malignant liver tumors of infancy are uncommon. In this report the histological and ultrastructural features of a congenital mixed hepatoblastoma in a female infant are described. The tumor was obtained by radical surgery at the eighth day of life. Histologically the mass was characterized by a mixture of variously differentiated hepatic tissue and mesenchymal tissue. The epithelial component is represented by liver cells of the embryonic and fetal type. Electron microscopical investigation revealed a changing organelle composition of the epithelial tumor cells showing a decreased number of cytoplasmic organelles in the cells of the embryonic type. At cellular junctions, however, these cells are connected by desmosome-like structures without lumina as well as empty canaliculi-developing structures of cholangiolar microvilli. At light microscopical level the former corresponded to pseudorosettes , the latter to so-called rosettes. Furthermore, in places sinus-like structures are encountered but in the absence of true sinusoidal channels with Disse's spaces. Some areas consisted of undifferentiated cells lacking cytoplasmic organelles and resembled a multipotential blastema. The ultrastructural findings suggest that hepatoblastoma may arise from an undifferentiated progenitor cell capable of various forms of differentiation.

[Morphology and differential histological diagnosis of BCG histiocytosis].

Two cases of mycobacterial histiocytosis in infants with the morphological signs of a combined immune defect are reported. In the first case the seven months aged male infant died of a generalized BCG infection. In the second case the mycobacterial histiocytosis of mesenterial lymph nodes was an incidental finding of autopsy in a female infant four month of age, whose cause of death was a generalized cytomegaly. The morphological picture and the histological differential diagnosis of mycobacterial histiocytosis are discussed.

[Morphology of neuroblastoma. Light and electron microscopic studies as a contribution on diagnosis and differential diagnosis].

The neuroblastoma is one of the most frequent malignant solid tumors in childhood and is thus of great practical importance. The origin of neuroblastoma cells from neural crest derivatives is generally accepted now, and this histogenesis explains some biochemical and morphological characteristics of the tumor. The cytological and histological features of neuroblastomas can be rather varying and, therefore, the diagnosis and differential diagnosis may be difficult. Our study presents the findings of 48 neuroblastomas after light microscopic examination and the ultrastructural characteristics of 8 neuroblastomas and 1 ganglioneuroma. At light microscopic level, completely undifferentiated neuroblastomas and tumors with variable degrees of differentiation were identified. The differentiation of the tumor tissue to ganglion cell-like elements was indicated by an increasing amount of cellular cytoplasm with development of a cytoplasmic process as well as an alteration of the picture of the nucleus (nuclear enlargement and a clearly visible nucleolus). Differentiation to Schwann cell-like elements was occasionally observed, too. Electron microscopically, in all tumors neurosecretory granules could be recognized, and in the better differentiated areas neurite-like cytoplasmic projections were detectable. Thus, the electron microscopy can be a valuable aid in establishing an unequivocal diagnosis. The histology of neuroblastomas is said to be of prognostic significance. Therefore, grading schemes of malignancy were developed. At present, the grading procedure after Hughes and coworkers is mostly used. The criteria of this grading system are presented and interpreted. Finally, those tumors are briefly characterized which play the main role in the differential diagnosis, i.e. juvenile rhabdomyosarcomas, Ewing's sarcoma (including the extraskeletal type), lymphoblastic lymphoma and histiocytic reticulosarcoma. The most important clinicopathologic differences in comparison to neuroblastomas are referred to. Using a large scale of morphologic methods as well as considering clinical and paraclinical parameters, the exact diagnosis of neuroblastoma should be possible in nearly every case.

[Germ cell tumors of testis: histological classification of 552 cases according to the WHO-nomenclature].

Using the WHO-nomenclature 552 germ cell tumors of testis were reclassified. The analysis revealed the following: germ cell tumors occur in early childhood at the age of 3 months to 3 years, in adult hood (26-35) the occurrence shows a peak, and in old aged men only some tumors are registered. No germ cell was found between 4 to 13 years. Our material comprised 245 seminomas (44.4%) and 307 non-seminomatous germ cell tumors (55.6%). In the group of tumors of one histologic type, 245 cases of seminomas represent the main part. The following subgroups were recognized: typical seminomas--229 cases (93.5%), spermatocytic seminomas--9 cases (3.7%), and anaplastic seminomas--7 cases (2.8%). In 10 seminomas (4.1%() we have found syncytiothrophoblastic giant cells. In comparison to other investigations, the number of anaplastic seminomas is very low, but on the other hand the diagnosis Of this tumor is problematic, especially, the differential diagnosis of solid variants of embryonal carcinomas. The most common non-seminomatous tumor in this group is the embryonal carcinoma (16.8%). No polyembryoma and choriocarcinoma were observed, and only 2 yolk-sac-tumors occurred in infants. The occurrence of teratomas was also low, i.e. 15 cases of mature teratoma and 13 cases of immature teratoma. In the group of tumors of more than one histologic type, cases showing embryonal carcinoma and teratoma prevail (83 tumors). In addition to this combination other common tumors of this group (contained structures of seminomas (25 cases), yolk-sac-tumors (22 cases), an choriocarcinomas (19 cases) and embryonal carcinomas and yolk-sac-tumors (13 cases). Altogether, this group comprised 72.8% of the non-seminomatous tumors. Furthermore, the peculiarities of germ cell tumors of childhood are described. In this group the mature teratomas prevail. No seminoma could be registered in infancy. The distribution of our cases among the various types and the subgroups agreed in the main classes with that reported in literature but in special differentiations there are some discrepancies. The problem of classification of germ cell tumors is discussed.
