The future need for geriatric manpower in the United States.

Additional medical manpower will be required to meet the growth in the elderly segment of the United States population over the next 50 years. This paper provides estimates of the needs for medical geriatric manpower under four different models: continuation of the status quo; academic geriatricians only; provision of care by academic and consultant geriatricians; and provision of care by academic, consultant, and primary-care geriatric practitioners. Each option is further analyzed in terms of three levels of delegation to nonphysician clinicians. We project the implications of these 12 patterns through the year 2030, assuming present usage levels and allowing for improved care of the elderly. The need for geropsychiatric care is explored separately. We think that geriatricians should provide improved care, including both consultant and primary care, to people aged 75 years and older, and that they should delegate a moderate amount of responsibility to nurse practitioners, physician assistants, and social workers. On the basis of these assumptions, and allowing for an academic role as well, we estimate that the United States will require between 7000 and 10,300 geriatricians by the year 1990; the best intermediate figure is about 8000.

Transduction of a bacterial gene into mammalian cells.

The transduction of an Escherichia coli gene into mammalian cells is described. A supressor tRNA gene was linked to a simian virus 40 (SV40) vector in vitro and the recombinant was used to transfect rat embryo cells and monkey kidney cells. The hybrid SV40 genome, SV40-su+ III, retained genetic information required for autonomous replication and cellular transformation and had a 1300-base-pair DNA segment in the late gene region (between the restriction endonuclease sits Hpa II at 0.735 and EcoRI at 0/1.0 on the SV40 genetic map) replaced by an 870-base-pair bacterial DNA segment containing the suppressor tRNA gene, su+ III (tRNATyrsu+III). The structure and fate of the SV40-su+III chimera were determined by DNA reassociation kinetic analysis and restriction enzyme cleavage of the total cellular DNA from transformed rat embryo cells and persistently infected monkey cells. Hybridization with radiolabeled probes specific for vector (SV40) or su+III DNA sequences revealed primarily nonintegrated or free hybrid genomes. In cloned lines of both cell types, the bacterial DNA segment was recovered intact, as judged by the length of the segment excised by restriction endonucleases and its ability to hybridize to the radiolabeled bacterial DNA probe and not to the SV40 probe.

High-dose benzodiazepines in the treatment of severe neurotic anxiety.

There has been a paucity of research in the use of high-dose benzodiazepines in the treatment of severe neurotic anxiety. The author presents five cases, three with excellent and two with poor outcomes. The English and Fench language literature is reviewed and reveals evidence that high-dose benzodiazepines may be useful in the treatment of certain neurotic patients. However, the lack of controlled studies is noted. The author poses several questions important to the future of research on benzodiazepines.

Inclusions in the sural nerve in metachromatic leukodystrophy.

A 2 year old girl develops a fever, anorexia and a cough which last three weeks. Following this event, she presented with intention tremor and increasing difficulty with crawling. At the age of 3, a nerve biopsy was done which made the diagnosis of metachromatic leukodystrophy. EM examination of the nerve biopsy revealed large quantitites of abnormal structures in the cytoplasm of Schwann and phagocytic cells. Virus like spheroid bodies were present in a few axons and cytoplasm of Schwann cells.

Presumptive D2a point mutants of bacteriophage T4.

Mutants of bacteriophage T4 have been isolated that show phenotypes seen previously with those rII deletions that extend into the nearby D2a region. Analysis indicates that rII mutations are not necessary in order to get the D2a phenotypes.
