Femoral neuropathy after renal transplantation.

We report nine cases of neuropathy occurring after renal transplantation, seven of them femoral and two lateral femoral cutaneous. The average time of onset of symptoms is 2.2 days after surgery. Quadriceps weakness, hypoesthesia, and abnormal results of EMG and nerve conduction studies are common and occur on the same side as the operation. The symptoms usually resolve within weeks, but motor and sensory changes can persist. The lesion is thought to be secondary to compression caused by a hematoma at the operative site.

Routine myocardial revascularization with the radial artery: a multicenter experience.

BACKGROUND: Current literature documents use of the radial artery (RA) for myocardial revascularization only as an alternative conduit in cases where the saphenous veins have been previously harvested or are unsuitable for use. Large-scale routine clinical use of the RA as the conduit of choice has not been reported. METHODS: This prospective study evaluated the harvest of the RA from 933 patients and the subsequent use of the conduit as a preferred coronary artery bypass graft second only to the left internal thoracic artery in 930 of these patients. RESULTS: Unilateral RA harvest was performed in 786 patients and 147 patients had bilateral RA harvest. A total of 1080 RAs were harvested; 214 (19.8%) originated from the dominant forearm. There was a mean of 3.30+/-0.93 grafts per patient of which 2.43+/-0.83 were arterial grafts. The mean number of RA grafts was 1.43+/-0.53. Operative mortality was 2.3% with none due to the RA graft(s). There was no ischemia nor motor dysfunction in the operated hands. Thirty-two (3.4%) patients experienced transient thenar dysesthesia that resolved in 1 day to 6 weeks. CONCLUSIONS: Our results demonstrate that routine total or near total arterial myocardial revascularization may be achieved safely and effectively with the use of one or both RAs in conjunction with the internal thoracic artery.

Effect of cataract surgery and foldable intraocular lens implantation on retinal nerve fiber layer as measured by scanning laser polarimetry with variable corneal compensator.

PURPOSE: Scanning laser polarimetry (SLP) enables the measurement of retinal nerve fiber layer (RNFL) thickness in vivo. As SLP measurements can be influenced by several conditions such as corneal birefringence, the new version GDx Access is implemented by the variable corneal compensator (VCC). The aim of this study was to evaluate the influence of cataract extraction and foldable intraocular lens (IOL) implantation on SLP parameters measured by GDx Access. METHODS: Sixty-eight eyes of 68 patients undergoing phacoemulsification with foldable IOL implantation (silicone and acrylic) were examined by SLP before and after surgery. SLP was performed using GDx Access (with VCC). Mean values for SLP parameters were compared before and after 30 days from surgery with paired two-tailed Student's t-test. RESULTS: No statistically significant differences were found between SLP parameters before and after cataract surgery, and regardless of the type of IOL implanted. CONCLUSIONS: Phacoemulsification and foldable IOL implantation do not influence RNFL parameters, as measured by GDx Access.

Evaluation of a stented polyurethane mitral valve prosthesis.

In the search for the perfect valve prosthesis, polyurethane has been used for its excellent physical properties, but thrombosis and embolism have been real problems. The authors investigated the performance and durability of a 23 mm vacuum formed polyurethane Tricuspid Semilunar Valve, in the mitral position. The valve was implanted in eight sheep using standard cardiopulmonary bypass and intermittent aortic cross clamping. Two early failures were due to valve tears. After modification of the manufacturing process, six further valves were implanted in six sheep. All were anticoagulated with warfarin. The valves were calcified on echocardiography and had increasing transvalvular gradients beginning 3 months postoperatively. All six survived and were killed 1-2.5 years after implantation. Despite gross calcification of the leaflets, medium-term function was good. Careful serial sections of brain showed no evidence of thromboembolism. Both kidneys contained small calcium emboli, but no evidence of infarction. Despite calcification, with good medium-term function and no cerebral emboli, this valve may find application in temporary cardiac assist devices.
