Liver span and weight in health and disease.

BACKGROUND: The liver mass, made up of independent functional units called acini, correlates with liver functions. Hepatic morphological changes causing alteration in its size and volume occur in various hepatic disorders. METHODS: Antemortem clinical liver span and postmortem liver weight was studied in 23 critically ill patients. Out of these, 8 had fulminant hepatic failure, 7 had chronic liver disease with encephalopathy and 8 had other serious illnesses (chronic renal failure 3; ischaemic heart disease 3; cerebrovascular accidents 2). We also studied postmortem liver span and liver weight in 173 healthy individuals dying of accident/terrorist attacks. RESULTS: Antemortem liver span in 23 critically ill patients was 9.9 +/- 4.7 cm (mean +/- S.D.) and postmortem liver weight was 1536.9 +/- 704.7 g (mean +/- S.D.). Antemortem liver span correlated well with the postmortem liver weight (r = 0.9635). Postmortem liver span in 173 healthy individuals was 16.3 +/- 2.7 cm (mean +/- S.D.) and liver weight was 1301.5 +/- 358.8 g (mean +/- S.D.). There was very good correlation between the postmortem liver span and liver weight (r = 0.516), height (r = 0.410) and body weight (r = 0.340) in these individuals. CONCLUSION: Liver span fortells the liver weight, hence, the functional liver mass in diseased and healthy individuals.

Perindopril.

Perindopril is the newest angiotensin-converting enzyme inhibitor to be approved in the United States. It is indicated for the treatment of essential hypertension either alone or in combination with other antihypertensives, especially thiazide diuretics. Perindopril must be converted to perindoprilat, its active metabolite. The drug's prolonged duration of action allows for once daily administration. This article reviews clinical trials performed using perindopril alone and in combination to treat essential hypertension. The use of perindopril for congestive heart failure is reviewed. The influence of perindopril on arterial wall properties is discussed. Tolerability, drug interactions, and dosage and administration of the drug are reviewed.

Explanatory models and common mental disorders among patients with unexplained somatic symptoms attending a primary care facility in Tamil Nadu.

BACKGROUND: Patients with unexplained somatic symptoms are commonly seen in primary care. We assessed the explanatory models and common mental disorders in patients with unexplained somatic symptoms attending a primary care facility in a rural area of south India. METHODS: One hundred consecutive patients diagnosed to have unexplained somatic symptoms attending a primary care facility were examined. The Tamil version of the Revised Clinical Interview Schedule was used to assess common mental disorders and the Tamil version of the Short Explanatory Model Interview was used to assess their explanatory models. RESULTS: Ninety-eight patients thought that their problem was serious. Sixty-nine, 41 and 40 claimed that it affected their work, family and social lives, respectively. Forty-two of the patients believed in black magic. Forty-four patients satisfied the ICD-10 PC criteria for common mental disorders. Depression was the commonest diagnosis (63.6%). Patients with psychiatric diagnoses admitted to emotional problems (chi2 = 46.9; p < 0.001), and felt that their problems affected their family (chi2 = 38.5; p < 0.001), social life (chi2 = 28.6; p < 0.001) and work (chi2 = 34.4; p < 0.001). More patients who satisfied criteria for psychiatric diagnosis felt that their problems were moderate or very serious (chi2 = 21.5; p < 0.001) and they feared they might become disabled or die (chi2 = 12.2; p < 0.001). Psychiatric syndromes were significantly associated with belief in black magic (chi2 = 7.01; p < 0.01). These associations remained statstically significant after adjusting for age, gender, literacy and socioeconomic indices using logistic regression. CONCLUSION: The majority of patients held strong beliefs regarding the physical nature of their complaints, believed In the serious nature of the problem and feared disability or death. There is a need to elicit specific explanatory models regarding the nature of illness in patients who present with somatic symptoms without organic causes. Understanding the patient's perspectives is a prerequisite for providing the necessary treatment and to dispel fears.

Functional proteomics of receptor-Ck in the developmental stages of human atherosclerotic arterial wall.

BACKGROUND: The study was undertaken to understand the relationship between the functional proteomics of receptor-Ck and developmental stages of human atherosclerotic aortic wall. METHODS AND RESULTS: Gene expression study of 25 aortas was undertaken and the results revealed a gradual increase in receptor-Ck gene expression paralleled by the regulatory response of its effector genes coding for sterol response element-binding protein, p27, cyclin D, interleukin-6 and CD40 from a normal to atherosclerotic arterial wall (viz. fatty streak and fibrofatty/fibrous plaque). CONCLUSIONS: Based upon this and our earlier studies, we propose that cholesterol-specific receptor-Ck-dependent gene regulation may be of crucial importance in atherogenesis.

Gangliogliomas: A report of five cases.

Gangliogliomas are rare tumors of the Central Nervous System. Five gangliogliomas were diagnosed out of 1560 brain tumours surgically resected out in a period of 5 years accounting for 0. 32%. We have tried to discuss in detail the pathological features of these tumours and have mentioned the clinical and radiological features associated with them. All the slides, tissue blocks and pathology reports of the surgical specimens of gangliglioma were reviewed and the clinical and radiological data reviewed. The ages of the patients ranged from 7-65 years with 4 males and 1 female. The tumors were located in the lateral ventricle (a rare site), temporal, parietal and the frontal lobes with duration of seizures varying from 1-9 years. The tumors were diagnosed by the presence of a dual population of neoplastic ganglionic and glial components. The glial components consisted of pilocytic astrocytes (l case), fibrillary astrocytes (2 cases), oligodendrocytes (1 case) and anaplastic astrocytes and oligodendrocytes (1 case). There was one-grade I GG, three-Grade II GGs and one-grade III GG. Astrocytes were the commonest glial component of GGs, either pilocytic or fibrillary. Oligodendrocytes as the glial component of GGs was seen in 2 cases one of which was anaplastic and this is a rare finding.

Pyaemia in pigs.

Pyaemia is by far the most important cause of condemnation in pigs slaughtered in Singapore abattoirs. Between 1983 and 1987, 1757 from a total of 4,899,731 pigs were condemned by meat inspectors for pyaemia, accounting for 0.036% of the total condemnations. The common post-mortem findings of affected pigs during the 5-year period are presented. Abscesses were most commonly seen in the liver (22%), spleen (21%), gastrohepatic lymph node (20%) and bronchial lymph node (17%). Pseudomonas pseudomallei was the most predominant organism isolated, accounting for 39% of the pyaemic cases. The public health significance of abscesses in pigs is discussed with particular reference to melioidosis.

Studies on circadian periodicity of plasma 17-hydroxycorticosteroids (17-OHCS) in carcinoma of the breast.

Circadian periodicity of plasma 17-hydroxycorticosteroids (17-OHCS) was studied in 25 histopathologically proved breast cancer patients pre operatively and on the 9th postoperative day and in 15 healthy women under tropical conditions. A marked rhythm in plasma 17-OHCS concentration was noticed in healthy controls. The maximal concentration occurred at 06:00 followed by a consistent decline throughout the daytime, reaching a minimum at 00:00 with an amplitude statistically significantly different from zero and an acrophase around 08:56. The plasma 17-OHCS concentration was found to be elevated at all sampling hours in breast cancer patients, irrespective of the stage of the disease. However, the degree of elevation and disruption of rhythm was more pronounced in advanced sages, preoperatively, in comparison to other groups. Moreover, the values declined markedly at all collection hours after mastectomy, approaching usual values and rhythm characteristics on the 9th postoperative day.

Studies on circadian periodicity of urinary corticoids in carcinoma of the breast.

The circadian periodicity of urinary 17-ketogenic steroids (17-KGS), 17-ketosteroids (17-KS) and creatinine (Cr) was studied preoperatively and on the 9th postoperative day in 25 histopathologically proved breast cancer patients and in 15 healthy Indian women under tropical conditions. A statistically significant rhythm was observed in healthy participants for all three variables. Urinary corticoids were markedly elevated in breast cancer patients irrespective of the stage of the disease in comparison with healthy controls. The degree of elevation was more pronounced preoperatively in advanced stage breast cancer in comparison with other groups. After mastectomy, the values of all three variables declined markedly, approaching usual values with a circadian rhythm resembling the pattern found in clinical health.
