Unilateral spatial neglect: biases in contralateral search and fine spatial attention.

Contralateral search and fine spatial attentional asymmetries, two aspects of hemispatial neglect, were examined in a total of 91 right brain damaged (RBD) and 40 non-aphasic left brain damaged (LBD) stroke rehabilitation inpatients. Fine spatial attentional asymmetries within a hemispace were found in both RBD and LBD patients on the LAVA figure-match test. This finding is in contrast to Gainotti's hypothesis that such problems in extracting contralateral visual information are uniquely characteristic of RBD patients. Furthermore, the study suggests that the gross search and fine attentional aspects of neglect are dissociable and may reflect different underlying mechanisms of hemispatial neglect.

Growth and precocious puberty.

Treatment of precocious puberty of central origin is aimed at controlling the development of sexual characteristics and improving final height. Increased growth rate is one of the major clinical symptoms, accompanied by an even more marked advance in bone age. Medroxyprogesterone acetate and cyproterone acetate have provided almost satisfactory control of pubertal characteristics, but with accompanying adrenal insufficiency. The data with regard to growth and bone maturation are contradictory. LHRH analogues have recently become available, and provide good control of gonadotrophin secretion. In a series of 21 cases (13 girls, 8 boys), a significant decrease in growth rate was achieved in both sexes with an LHRH analogue, with a significant increase in the height age/bone age ratio; control of gonadal secretions was also obtained. These results are only preliminary, but provide hope that the final height of these children will be improved.

[Treatment of endemic and sporadic goiter].

Early continuous hormonal treatment, inhibiting TSH secretion, reduces the volume of recent homogeneous goiters. This treatment is usually well tolerated, but can induce thyrotoxicosis in subjects with blunted response of TSH to TRH stimulation before treatment. Surgery is used for large goiters, compressive goiters or suspicion of cancer and after failure of the hormonal treatment. In all cases, hormonal treatment must be associated to surgery. Radio-iodine can be used for large or compressive goiters, when surgery is inadvisable. Despite the risk of thyrotoxicosis, iodine addition in food intake is useful to prevent goiters. The frequency of thyrotoxicosis can be decreased by ruling out subjects over 50 years old, with nodular goiters or with blunted response of TSH to TRH stimulation.

The development of a role charter for nursing unit administrators.

Issues of decentralization and changes in the structure of the traditional Head Nurse role were motivating factors for the proactive stance taken by the Medical-Surgical Head Nurse Group at The Wellesley Hospital. Increasing frustration with the conventional role and the need for a reflective performance appraisal tool provided further impetus for the development of a revised job description. The three departments of nursing each submitted their perception of the role. A task force was established whose mandate was to review and revise the three separate submissions and combine them into a composite document. This article describes the process from the initial motivating factors to the development of a cohesive Total Head Nurse Group and finally a 'Role Charter for Nursing Unit Administrators.'

Methylation of the enhancer region of avian sarcoma virus long terminal repeat suppresses transcription.

The effect of methylation of an enhancer on transcription was studied. A 245 bp enhancer-containing a fragment of the LTR of the avian sarcoma virus was methylated in vitro and ligated back into a vector which lacked the upstream enhancer sequence. The transient expression in QT6 cells indicated that methylation of the enhancer-containing sequence severely reduced the extent of transcription.

Production and partial characterization of monoclonal antibodies against 3,3',5-triiodo-L-thyronine.

Spleen cells of Biozzi HL mouse (selection V) immunized with bovine albumin-triiodothyronine conjugate were fused with P3-X63-Ag8.653 mouse myeloma cells. Thirteen monoclonal antibodies, selected by an enzyme-linked immunosorbent assay and a radioimmunoassay, were produced in mouse ascites fluid, purified and analyzed. All of them were IgG1 (kappa). The cross-reactivity of all these monoclonal antibodies with thyroxine (T4) was less than 0.2%. The association constant determined by Scatchard analysis ranged from 1.5 x 10(9) M-1 to 2.7 x 10(10) M-1.

Leukoerythroblastosis with blasts in a patient with alcoholic hepatitis.

Leukemoid reactions are common in alcoholic hepatitis. We report a case of biopsy-proven alcoholic hepatitis with leukoerythroblastosis and blasts on the peripheral smear. We take this to be the second case of blasts and the first case of leukoerythroblastosis reported in a patient with alcoholic hepatitis.

Changes in the sexual behavior of couples after prostatectomy. A prospective study.

Sexual behavior after prostatectomy was observed in 90 couples, with a follow-up of 3-6 years. Retropubic prostatectomy was performed on 53 patients (58.7%), transvesical prostatectomy on 12 patients (13.7%), and transurethral prostatectomy on 25 patients (27.7%). In order to have a reliable figure of the sexual behavior after surgery, all the couples underwent personal interviews separately before and 3-6 years after surgery. The interview included 4 main subjects: erection allowing sexual intercourse; antegrade ejaculation; orgasm, and libido. The separate identical double interviews were performed in order to achieve a high reliability in answer to sexual performance, this being the main unsolved question in previous studies. The results of the interviews were assessed by a computer-processing program and statistically analyzed. Our results imply that prostatectomy has an unfavorable influence on sexual activity: 33% of the potent preoperative patients became impotent after surgery.

Measurement of active renin by the 4G1 anti-human renin monoclonal antibody.

Three monoclonal human renin antibodies have been selected to settle two immunoradiometric assays of human plasma renin (IRMA). The first pair of antibodies 3E8-4G1 recognizes exclusively active renin (AR), as demonstrated by the lack of increase either of the number of AR molecules or the renin enzymatic activity, when plasma is set free of inactive renin (IR) by immunoaffinity chromatography with human renin prosegment monoclonal antibody. The second pair of monoclonal antibodies 4E1-3E8 gives results which are very significantly correlated to those obtained with the 3E8-4G1 pair after trypsin activation. The standardization of renin measurements with use of monoclonal antibodies, and the standardization of renin will allow the exchange of comparable informations between the various laboratories involved in the investigation of the renin-angiotensin system in humans.

The value of Rowatinex in the treatment of ureterolithiasis.

A randomized, double-blind study was carried out on forty patients with acute renal colic in order to evaluate the efficacy of Rowatinex in the treatment of patients with ureteric stones. Patients were randomized to Rowatinex and placebo tablets. In the Rowatinex-treated patients there was a significantly higher rate of expulsion of stones which were greater than or equal to 3 mm in diameter when compared with the placebo group (61% vs. 28%, respectively). A higher success rate (spontaneous stone expulsion and/or disappearance of dilatation of the collecting system on IVU) was noted in the Rowatinex group when compared to the placebo group (78% vs. 52%, respectively), although this was not statistically significant. No significant side effects were noted during the treatment with Rowatinex.

The enzyme rhodanese can be reactivated after denaturation in guanidinium chloride.

For the first time, the enzyme rhodanese has been refolded after denaturation in guanidinium chloride (GdmHCl). Renaturation was by either (a) direct dilution into the assay, (b) intermediate dilution into buffer, or (c) dialysis followed by concentration and centrifugation. Method (c) preferentially retained active enzyme whose specific activity was 1140 IU/mg, which fell to 898 IU/mg after 6 days. The specific activity of native enzyme is 710 IU/mg. Progress curves were linear for the dialyzed enzyme, and kinetic analysis showed it had the same Km for thiosulfate as the native enzyme, but apparently displayed a higher turnover number. Progress curves for denatured enzyme directly diluted into assay mix showed as many as three phases: a lag during which no product formed; a first order reactivation; and an apparently linear steady state. An induction period was determined by extrapolating the steady-state line to the time axis. The percent reactivation fell to 7% (t1/2 = 10 min) as the time increased between GdmHCl dilution and the start of the assay, independent of the presence of thiosulfate. The induction period, which decreased to zero as the incubation time increased, was retained in the presence of thiosulfate. There were no observable differences between native and renatured protein by electrophoresis or fluorescence spectroscopy. Previous reports of some refolding of urea-denatured rhodanese (Stellwagen, E. (1979) J. Mol. Biol. 135, 217-229) were confirmed, extended, and compared with results using GdmHCl. A working hypothesis is that rhodanese refolding involves intermediates that partition into active and inactive products. These intermediates may result from nucleation of the two rhodanese domains, which exposes hydrophobic surfaces that become the interdomain interface in the correctly folded protein.

Analysis of inactive renin by renin profragment monoclonal antibodies.

Two peptides were synthesized, corresponding to the sequences (-19 to -7) and (-26 to -17) of the prorenin prosegment. Monoclonal antibodies were raised to these sequences and used to characterize human plasma inactive renin. Only anti (-19 to -7) reacted with inactive renin, as measured by direct assay or affinity chromatography. The data were used to evaluate two possible inactive renin stuctures: plasma inactive renin is a truncated prorenin lacking the prosegment N-terminal portion; its spatial conformation masks the N-terminal extremity, preventing interaction of this region with specific antibodies.

The use of laser in rectal surgery.

Ten patients with rectal tumors were treated by surgical excision of the tumor using a CO2 laser Sharplan 733 CO2 laser, Laser Industries Ltd., Israel. The laser permitted sharp excision, evaporization, or fulguration of tissue. Good hemostasis was achieved. Anesthesia was unnecessary in cooperative, calm patients. There was no mortality or morbidity related to the procedure. In five patients with villous adenoma the tumor was resected with one recurrence as yet (followup 4-16 months). Five patients with inoperable carcinoma were treated for palliation with good results. Our preliminary results indicate that the CO2 laser can be a useful asset in rectal tumor surgery, especially in elderly, moribund patients and in the definite treatment of benign tumors of the rectum.
