A quantitative analysis of the human bone marrow granulocytic cell lineage using the SAMBA 200 cell image processor. I. The normal maturation sequence.

A quantitative image analysis of the human normal bone marrow granulocytic line was performed using the SAMBA 200 image analyzer. The steps of image acquisition, preprocessing, segmentation and parametrization are described. Forty-one parameters were computed on 941 cell images belonging to the various maturation stages. The automated classification of these cells based upon a stepwise linear discriminant analysis resulted in 77% correctly classified cells; the five most discriminating parameters were the nuclear area, the nuclear convexity degree, the average cytoplasmic hue, the regularity of the nuclear boundary and the average cytoplasmic luminance. The evolution of the parameters correlates well with the cytologic evolution and the biochemical and functional events during the maturation process. It can be inferred from our results that the maturation sequence can be subdivided into two phases according to the evolution of the cell profiles. The first phase, from myeloblast to myelocyte, is discontinuous and appears as the critical point with regard to the expression of genes. The second phase, from myelocyte to polymorphonuclear cell, is a continuous sequence of transformations leading to the functional granulocyte.

[Refractory anemia with an excess of blasts: evidence for 2 evolving groups in a group of 56 patients].

Cytological and kinetic findings in 56 patients with refractory anemia with excess blasts (RAEB) were studied. The different statistical methods used produce similar results: our group of patients can be separated by cytological parameters in two populations (overt leukemic or hematologically stable evolution). Different functional lesions may be responsible for these two types; pluripotential stem cell dysfunction in the first case and abnormality of the granulocytic committed stem cell in the second. A score, based on a small number of simple and reliable parameters, is able to distinguish these two populations.

Herpes simplex virus glycoprotein D: human monoclonal antibody produced by bone marrow cell line.

Normal bone marrow cells from a donor positive for herpes simplex virus were transformed with Epstein-Barr virus. The resulting lymphoblastoid cell line has secreted immunoglobulin G1 of the kappa type continuously for 2 years. This immunoglobulin, detected both on the cell surface and in the cytoplasm, reacts with cells infected with herpes simplex virus. It defines an antigen that comigrates with the 55-kilodalton glycoprotein D of herpes simplex virus type 1 and neutralizes the infectivity of herpes simplex viruses 1 and 2.

Auer rods in refractory anemia with excess of blasts: presence and significance.

Auer rods were found in the bone marrow of nine out of 56 patients having all the criteria of refractory anemia with excess of blasts (RAEB). No differences were found in Auer-positive or Auer-negative subgroups according to cytology, kinetics, type of growth in agar, or disease course. Therefore, the presence of Auer rods in the bone marrow of patients with RAEB does not imply overt leukemia.

Interference of hyperleukocytosis on Coulter Counter Model S blood counts: methods for correction.

Hyperleukocytosis leads to an overestimation of the determination by Coulter Counter Model S of erythrocyte count, hemoglobin, MCV and packed cell volume. These overestimations are proportional to the number of leukocytes above a certain limit, and, for the MCV and PCV, depend on the type of leukocytes present. Regression lines have been calculated which allow for correction of the values obtained.

Non-Hodgkin's malignant lymphomas: a cytologic classification based on statistical analysis and correlated with prognosis.

On the basis of cytologic studies of 144 patients with non-Hodgkin's malignant lymphomas (NHML), a cytologic classification was established that was composed of three groups (1, 2 and 3) containing nine classes (1A, 1B, 1C; 2A, 2B, 2C; 3A, 3B, 3C). Statistical analyses were carried out using 52 well-defined elementary cytologic characteristics. All the results given by the data-gathering procedures (hierarchical and dynamic clustering methods), by the descriptive analyses (multifactorial and canonical analysis) and by the stepwise discriminant analysis were in agreement with the proposed cytologic classification system. The most discriminating cytologic characteristics of the classes were the cell size, the nuclear-cytoplasmic ratio, the degree of cytoplasmic basophilia, the homogeneity of the cytoplasm and the number of mitoses. The use of these properties renders this classification system reproducible and applicable to clinical practice. A comparison was made between the cytologic classes and the NHML patients as grouped according to their clinical courses. Of the patients in classes 2A, 2B and and 2C, 94% showed acute leukemic characteristics. Of those contained in classes 3A, 3B and 3C, 78% showed poor prognosis or metastatic patterns. Of those contained in classes 1A, 1B and 1C, 76% showed a good prognosis pattern.

A comparison on 3 cell separation media: application to the enrichment for immature cells of the granular myeloid line.

Three common cell separation media (Bovine Serum Albumin Percoll and Lymphoprep), used for enrichment for immature cells of the granular myeloid line, were compared for simplicity of use, maximum recovery of Granulo-Macrophagic colony forming cells and myeloblasts, and minimum contamination with other cell types; Percoll, at a density of 1.078 g/cm3, was found to be best suited to our uses.

Use of the tritiated thymidine-labelling index of the myeloblast-promyelocyte pool for the identification of the leukemic population in oligoblastic leukemia.

The bone marrow myeloblast-promyelocyte-labelling index of 32 patients with oligoblastic leukemia was studied at diagnosis and during the course of the disease. A low value (less than 0.17) or a decrease in the labelling index is suggestive of a poor prognosis with development into acute myeloblastic leukemia. Leukemic myeloblasts are characterized by their inability to differentiate and their low labelling index. The low labelling index or the decrease in this labelling index are, thus, an indication of an increase in the leukemic population.

[Neutrophil migration in patients with rheumatoid arthritis (author's transl)].

The leukocyte mobilization "in vivo" was assayed in 16 patients with rheumatoid arthritis and 10 normal subjects using the modified Senn's technique. The rate of leukocytes mobilization (LMR) is variable: in 5 cases lower than normal. No correlation is found with latex or Waaler-Rose titer, total complement in the serum, but the lowest rates of leukocyte mobilization are in previously treated patients. After 48 hours of indomethacin treatment, the LMR decreases significantly both in the patients and in the normal subjects. These results show that the abnormal "in vivo" LMR is more related to drug therapy than disease.

The reproducibility of two methods of studying granulopoiesis: agar culture and labelling index.

The overall reproducibility of two methods of studying granulopoiesis has been investigated, i.e. study of the granulocytic progenitor cells by the agar-culture technique and study of the myeloblast labelling index by autoradiography. The results obtained indicate that these two methods are reproducible for each step involved, from the time of marrow sampling right through to the final interpretation of the results.

Monoclonal cryoimmunoglobulin with anti-cytomegalovirus activity associated with T cell chronic lymphocytic leukaemia.

A patient with chronic T cell lymphocytic leukaemia developed a monoclonal immunoglobulin (IgG3 kappa = 14 g/l) which was in part cryoprecipitable. At the same time, a subclinical CMV infection occurred which was associated with a neutropenia and thrombocytopenia, and which led to a rise in anti-CMV antibodies. The F(ab')2 fragment of IgG3 kappa, obtained by enzymatic cleavage, was examined for several antiviral activities and it was found to have a strong anti-CMV activity using the immunofluorescence test with anti-kappa conjugate. This is one of the few examples of a cryoglobulin with specific antiviral activity. The leukaemia, possibly together with immunosuppressive therapy, may have been responsible for the uncontrolled proliferation of the clone producing the cryoimmunoglobulin.

[Plasma cell beta-glucuronidase and monoclonal gammapathies (author's transl)].

beta-glucuronidase activity, estimated cytochemically, is, in more than half cases of myeloma, very increased or decreased, compared to that found in benign dysglobulinemias, where this activity is normal. A very high or very low score makes sure the diagnostic of myeloma. The relations between the beta-glucuronidase activity and the myeloma clinical stade according to Durie and Salmon and the bone lysis are discussed.

A comparative study of the Coulter Counter Model S, for the enumeration of peripheral leukocyte counts below 3.0 x 10(9)/l using whole blood and prediluted blood.

In this study low leukocyte counts were carried out using several methods. The results obtained by the visual method, the Coulter Model "S" using "whole blood", and the Coulter DN, show close correlation, but discrepancies exist when the Coulter S uses "prediluted blood". This is explained by excessive bubbles. This does not allow the Coulter Model S to be used with confidence for low leukocyte counts if "whole blood" is not available.

[Results of a one year study of Trombolab (author's transl)].

The Trombolab apparatus was purchased to measure thromboplastin time and recalcification clotting time automatically. For this last test, we have not been able to get satisfactory results. However, Thrombolab gives dependable and reproducible results for the thromboplastin time. Its use is easy and its maintenance simple.
