Nonunion.

Nonunion can be classified into hypertrophic and atrophic types solely on radiographic appearance. The etiology of the former is uncontrolled motion at the fracture site and of the latter is devitalization of bone at the fracture site. The orthopedic management of each type differs markedly. Early diagnosis and proper orthopedic management of nonunion are predicated on proper interpretation of the radiographs. The radiographic appearance of each type is discussed and the pathophysiology is explained.

The role of the lateral malleolus as a stabilizing factor of the ankle joint: preliminary report.

A preliminary report evaluating the role of the distal fibula as the lateral supporting structure of the ankle is presented. The study involved loading the knee of an above the knee amputation specimen and recording the resultant strains on the distal fibula. This was done with the ankle of the specimen in neutral, dorsiflexion, and eversion. Results show the susceptibility of the lateral fibula to compressive strains, with the greatest compression noted when the ankle is in eversion. Early findings also indicate that the fibula and lateral malleolus play an insignificant role in the weightbearing process in the lower limb.

Tension band fixation of acute cervical spine fractures.

Eight patients had posterior cervical spine fusion utilizing the principle of tension band fixation. Six patients with associated neurologic symptoms improved postoperatively. Solid fusion was achieved and a functional range of motion of the cervical spine was maintained in all of the living patients.

The effects of naloxone on methylphenidate-induced mood and behavioral changes: a negative study.

The effects of naloxone on IV methylphenidate-induced mood, behavior, and neurohumoral changes were assessed in eight psychiatric inpatients. While methylphenidate alone produced anticipated changes, the indications for significant naloxone-methylphenidate interactions were minimal. It is speculated that larger doses of naloxone might be required to produce modification of the psychostimulant effects.

Acute bilateral anterior dislocation of the shoulders.

In 3 cases of simultaneous bilateral anterior dislocation of the shoulders, considering the forces which predispose to anterior dislocations, it is postulated that in each case forced abduction and external rotation or abduction with hyperextension causes these injuries to occur.

Unresponsiveness of catatonic symptoms to naloxone.

A case report of a catatonic patient is presented. Because of the similarity of catatonia to the animal behavior produced by intracerebral administration of beta-endorphin, an endogenous opioid, naloxone was administered. No effect was observed with naloxone, whereas both amobarbital and diazepam were able to reverse catatonic behavior. Testing of catatonic patients with naloxone may help further elucidate naloxone's reported anti-psychotic properties.

Metabolic acidosis after hyperalimentation with casein hydrolysate. Occurrence in a starved patient.

A 29-year-old woman with short bowel syndrome and prolonged starvation developed hyperchloremic metabolic acidosis after initiation of hyoeralimentation with a casein hydrolysate solution. The acidosis was not due to bicarbonate loss but was associated with diminished ability of the kidney to increase urinary acid excretion, particularly titratable acidity. Supplemental parenteral bicarbonate administration was necessary for two weeks until urinary acid excretion rose to normal.
