Childhood leukemia and statistical characteristics in 0- to 18-year-old patients in Hungary, diagnosed between 1988 and 1992.

A population-based cancer registry for childhood leukemia was started in Hungary in 1971. Data processing and analysis have been done at the Second Department of Pediatrics, Semmelweis University Medical School in Budapest, which is the main center for the treatment of childhood malignancies in Hungary. In 1992 a new computerized database structure was created in collaboration with Kansas University Medical Center, Kansas City, Kansas, United States. This work presents childhood leukemia frequency distribution and treatment results between 1988 and 1992 in Hungary. The number of patients diagnosed with leukemia under 18 years of age fluctuates between 69 and 82 cases per year. We present the main causes of death and outline our future objectives to improve the survival rate and quality of life.

Glucocorticoid-induced apoptosis and treatment sensitivity in acute lymphoblastic leukemia of children.

Sensitivity of leukemic blasts to steroid therapy is one of the prognostic factors in acute lymphoblastic leukemia (ALL) in children. We examined the number of steroid receptors and the increase in the apoptotic index in peripheral blast cells after administration of prednisolone monotherapy in 21 children with ALL. A new diagnostic method was established based on determination of the apoptotic index in peripheral blood lymphoblasts to evaluate the steroid sensitivity of leukemic cells during the first day of therapy. The increase in apoptotic ratio, analyzed by morphologic and/or flow cytometric studies, was most expressed in the first 6 hours of treatment. The apoptotic ratio showed a good correlation with the clinical response. The number of steroid receptors (gcRs) on the blast cells was also examined, but it proved to be less informative than the in vivo steroid response itself.

Antibody-dependent and spontaneous cell-mediated cytotoxicity in children with acute lymphoblastic leukemia.

Antibody-dependent cell-mediated cytotoxicity (ADCC) and spontaneous lymphocyte-mediated cytotoxicity (SLMC) of the peripheral blood mononuclear cells (PBMC) of 12 children with acute lymphoblastic leukemia (ALL) in an active clinical stage and of 15 ALL patients in bone marrow remission were studied on allogeneic and xenogeneic targets. Leukemic children with clinically active disease had low SLMC and ADCC activity while the PBMC of patients in remission exerted normal cytotoxicity as compared to the corresponding mean values of an age and sex matched healthy control group.
