Correction of anterior mandibular vertical excess: the functional genioplasty.

The functional genioplasty is an osteotomy-ostectomy of the anterior mandible which reduces excessive lower anterior facial height to 55% of the total anterior facial height. The procedure is indicated particularly when one factor contributing to the long-face syndrome is vertical excess of the anterior mandible. The surgical technique involves creation of a tenon and mortise which not only preserves the insertions of both the labiomental muscles and at least some of the suprahyoid muscles but also improves the stability of transosseous fixation. Superior repositioning and advancement of the chin and myocutaneous structures produce both functional and esthetic benefits for the patient.

Orthognathic surgery for children. Analysis of 88 consecutive cases.

A retrospective analysis of 88 consecutive pediatric patients who underwent surgical correction of their dentofacial deformities is presented. There were twice as many female as male patients and the most common deformities were of the skeletal Class II type. The LeFort I osteotomy was the most frequently performed operation. The effect of controlled hypotensive anesthesia on the duration of both anesthesia and surgery, estimated blood loss and incidence of transfusion is discussed. Surgery for the correction of dentofacial deformities can be performed on children and adolescent patients with little morbidity and few complications.

Correction of dentofacial deformities in children and adolescent patients.

Modern orthognathic surgical techniques can complement orthodontic treatment of children and adolescent patients. A wide variety of deformities can be surgically corrected at a young age and examples selected from more than 100 treated pediatric patients are presented. As with all surgical procedures, the risks of surgery and anesthesia must be weighed against the benefits which are expected to result from the outcome of the surgery.

Treatment of traumatic bone cyst of mandible by injection of autogeneic blood.

The traumatic bone cyst is usually successfully treated by surgical exploration and curettage of the walls of the bony cavity. This article presents a case of traumatic bone cyst which not only failed to respond to surgical curettage but also demonstrated a marked increase in size and a multilocular appearance on the radiograph. Injection of autogeneic blood into the bony cavity produced rapid bony filling and resolution of the lesion.

[Adenomatoid odontogenic tumor].

A case of odontogenic adenomatoid tumor is presented in which it is stressed that conservative surgical enucleation of the lesion is the treatment of choice. The clinical, radiographic and histopathologic features of this tumor are discussed. Close attention and supervision of the developing dentition allows early diagnosis and treatment thus avoiding the needless sacrifice of adjacent teeth and bone.

[Apyretic malignant hyperthermia. Apropos of a case].

A case of a febrile malignant hyperthermia in a patient who was undergoing surgical treatment of recurrent traumatic bone cyst of mandible is presented. M. H. crisis can occur in the absence of both muscle rigidity and elevation of temperature. A potentially fatal outcome of M. H. crisis was avoided by aggressive opportune therapy.

Clinicopathological conference. Case 29, part 2. Follicular thyroid carcinoma metastatic to the mandible.

A case of follicular carcinoma of the thyroid metastatic to the mandible is reported and the literature is reviewed. Present conventional methods for determining the presence of secondary deposits, both in living patients and in cadavers, may account, in part, for the small number of cases reported in the literature involving metastases to the jaws. Careful attention to the medical history, clinical examination, scintigraphy, and results of the biopsy will help to establish early diagnosis of which definitive treatment can be based.
