Computed tomography findings in tuberculous mediastinitis.

Tuberculous mediastinitis can be an elusive diagnosis. Computed tomography may be performed in patients before establishing this diagnosis, and one should consider this disease when mediastinal mass with infiltration of adjacent fat planes is noted. Five such cases are described.

Unusual variant of left superior vena cava.

The first computed tomography-documented case of a persistent left superior vena cava that drains directly into the right superior vena cava is presented. The venous embryology is reviewed and the differential diagnosis of other pertinent venous anomalies is discussed. The patient also had an incidental finding of a congenitally absent left lobe of the thyroid gland.

Preoperative chest radiography: value as a baseline examination for comparison.

An important use of the preoperative chest radiograph is as a baseline for comparison when complications occur after surgery. Many authors have commented on the value of preoperative chest radiography as a screening examination, but no statistics have been presented regarding its impact on postoperative management. In 369 consecutive general surgical patients, the need for a baseline chest radiograph was evaluated. In 65 patients undergoing chest radiography postoperatively, a preoperative baseline was essential in making an accurate interpretation in 33 (51%). The value of preoperative chest radiography proved to be twofold. Some minimal abnormalities on postoperative radiographs were demonstrated to be clearly new, necessitating treatment or further evaluation. Second, significant abnormalities detected on postoperative radiographs that may have otherwise been subjected to expensive evaluation were often shown to have been present preoperatively. In approximately 9% of patients the preoperative chest radiograph had a significant impact on postoperative management.

Relapsing polychondritis studied by computed tomography.

Computed tomographic findings in a patient with relapsing polychondritis are described. Collapse of the cartilage of the nose and calcification in cartilages of the ears were clearly demonstrated. CT scanning was also helpful in evaluating the tracheobronchial tree for airway compromise, which could prove fatal in this condition.

Pulmonary complications of AIDS: radiologic features.

Fifty-two patients with pulmonary complications of acquired immunodeficiency syndrome (AIDS) were studied over a 3-year period. The vast majority of the patients were homosexual; however, a significant number were intravenous drug abusers. Thirteen different organisms were noted, of which Pneumocystis carinii was by far the most common. Five patients had neoplasia. Most patients had initial abnormal chest films; however, eight patients subsequently shown to have Pneumocystis carinii pneumonia had normal chest films. A significant overlap in chest radiographic findings was noted among patients with different or multiple organisms. Lung biopsy should be an early consideration for all patients with a clinical history consistent with the pulmonary complications of AIDS. Repeat biopsy is also indicated to ascertain the progression of radiographic findings. Unfortunately, even with documentation of the nature of the pulmonary process, treatment often is ineffective. Of the 52 patients, 41 had died by the time this report was completed.

Extraskeletal Ewing sarcoma with computed tomography correlation.

Extraosseous Ewing sarcoma--a rare clinical entity--is described and the radiographic and histologic features discussed. Histologically this type of tumor may be differentiated from other soft tissue malignancies, but the ultrastructural glycogen content of the cytoplasm makes it frequently inseparable from the conventional osseous form of Ewing sarcoma. In spite of the presence of a huge paraosseal mass with bone erosion or periosteal new bone, computed tomography was the only preoperative modality able to provide evidence of non-involvement of the marrow cavity. However, histologic documentation is necessary to prove that one is dealing with a totally extraosseous Ewing sarcoma.

Horseshoe kidney mimicking adenopathy.

Horseshoe kidney may simulate adenopathy on ultrasound when the examiner is unaware of its presence. If an ultrasound examination demonstrates an isolated preaortic mass, this diagnosis should be considered. Sonographic features and confirmation of this diagnosis with a urogram or CT are discussed.
