Chronic pain management.

In the past ten years developments in social psychology, neurochemistry, learning theory, and psychophysiology have expanded the concept of chronic pain into a biopsychosocial model, in which pain is viewed as a form of abnormal illness behavior influenced by a wide range of biological, social, and psychological factors. Using the literature on chronic pain, the authors discuss the evolution of the chronic pain concept into the multidisciplinary multimodal approach used by pain clinics today, and the factors identified in the literature that influence a patient's perception of pain. Finally, based on their experiences in setting up a pain clinic and on the literature on leading pain management programs, they discuss in detail the evaluation, treatment, and management strategies of a comprehensive chronic pain management program.

Diagnosis of Alzheimer's disease.

Alzheimer's disease is one of a number of conditions that can cause the syndrome of dementia. In the absence of a specific and simple test for Alzheimer's disease, the diagnosis is one of exclusion. There are several pitfalls in the diagnostic process, including failure to recognize depressive pseudodementia, focal brain disorders, and acute organomental syndromes. With an increasing proportion of elderly persons in the population, it is even more imperative to pursue accurate diagnosis with vigor, since several of the causes of dementia are treatable, whereas Alzheimer's disease is not.

Nemaline myopathy rod bodies. Structure and composition.

Ca2+-activated protease (CAF) digestion of glycerinated nemaline myopathy muscle removed the electron-dense material covering rods and Z-lines and exposed longitudinal backbone filaments, 6-7 nm wide, which span the lengths of the original rods. Decoration of the exposed filaments (which are responsible for the periodicity parallel to the long axis of intact nemaline rods) with heavy meromyosin (HMM) proved they are actin filaments. After CAF treatment, cross-striated periodical patterns in longitudinal sections and Z-filament-like proteins connecting actin filaments seen in cross-section disappeared. This suggests that alpha-actinin may be involved in formation of this pattern because of the specificity of CAF toward alpha-actinin. Gel electrophoresis of CAF-treated nemaline muscle showed that most alpha-actinin is released into the supernatant, whereas the residue is mainly actin and myosin. Electron microscope examination of longitudinal sections of intact rods shows an oblique filament pattern, thin (7 nm) lines, thick (11 nm) lines, and an amorphous-appearance previously observed in normal Z-lines, patterns observed depend on sectioning angle and section thickness. In cross-section, rods show small square net (SS) and basket-weave (BW) forms. The SS form predominates and coexistence of the 2 forms, which also occur in normal Z-lines, is observed. Results support the idea that rods are lateral polymers of Z-line units. We think that the length of rods, as well as the width of Z-lines, is determined by the amount of overlap of actin filaments of opposite polarity. Initiation of rod formation may be due to deregulation of actin filament length.

Single stage repair of rectourinary fistula.

The treatment of 2 prostato-rectal fistulas by a single stage York Mason repair and cystostomy is described. We realize that the therapy of rectourinary fistulas should be individualized and for the ill or septic patient the 3-stage treatment including colostomy is safe and conservative. However, for the patient in good condition the 1-stage repair seems to be an effective shortcut.

Tourette syndrome, an acquired encephalopathy? A report of two cases with epileptiform dysrhythmia.

The electroencephalogram, although providing no specific diagnostic information for Tourette syndrome, may be useful in evaluating the pathology in which the syndrome occurs, since it appears that at least some cases may occur in the setting of an underlying acquired encephalopathy of diverse etiology. A neurochemical basis for the Tourette syndrome expressed as an extrapyramidal circuit imbalance may also relate to a discharge threshold for epileptiform activity. A more detailed electroencephalographic evaluation of such patients using nasopharyngeal or sphenoidal recording may be useful in this regard.

Pelvic lymphadenectomy for the staging of apparently localized prostatic cancer.

Staging pelvic lymphadenectomy has been done on 87 patients with clinically localized prostatic carcinoma. With this method nodal metastases can be discovered, although they are undetectable by any other means. There were 44 patients with negative pelvic lymph nodes by surgical staging subjected to radical prostatectomy. Only 6 patients (14 per cent) had microscopic invasion of the prostatic capsule and there was just 1 instance of microscopic seminal vesicle invasion in those with negative pelvic lymph nodes.

Lipid disturbances associated with spiral muscular atrophy. Clinical, electromyographic, histochemical, and lipid studies.

Twelve patients with juvenile- and adult-onset spinal muscular atrophy have been studied. Eleven of the twelve patients had either type II, type IV, or borderline abnormal phenotypes, suggesting a possible relationship between serum lipid abnormalities and neuronal degeneration in the spinal muscular atrophies. Muscle enzyme histochemical studies provided valuable diagnostic information. Extensor toe signs and talipes cavus were common clinical observations.

Intratesticular epidermoid cyst. A rare benign tumor.

Epidermoid cysts rarely arise within the testis. The incidental discovery of a distinct, intratesticular epidermoid cyst in a nineteen-year-old man is reported. Management by radical orchiectomy is stressed because grossly the tumor is indistinguishable from a malignant testicular tumor.

Staging of localized prostatic carcinoma by pelvic lymphadenectomy: technique using sequential, unilateral extraperitoneal incisions.

A technique for sequential, unilateral, extraperitoneal pelvic lymphadenectomy is described. The procedure is advocated for staging patients with localized prostatic carcinoma in whom all laboratory test results for metastases are negative and in whom definitive radiation therapy or radical prostatectomy is contemplated. This procedure allows complete excision of the regional lymphatic drainage of the prostate with minimal surgical morbidity.

Bilateral complete renal duplication with total obstruction of both lower pole collecting systems.

Sepsis and a flank mass developed in a twenty-two-year-old primagravida two days after a normal delivery. Urography showed normal upper pole collecting structures bilaterally. A spherical mass containing curvilinear calcification occupied the left lower pole, and a large inflammatory mass filled the right inferior renal fossa. Angiography and retrograde pyelography demonstrated marked bilateral lower pole hydronephrosis with complete obstruction of the water to each inferior duplicated collecting system.
