Hypertonic stress induces c-fos but not c-jun expression in the human embryonal EUE epithelial cell line.

Recent evidence has indicated a role for the two early response genes c-fos and c-jun in transcriptional regulation of genes acting in osmoregulation. On this basis we investigated their expression in response to hypertonic stress in the human embryonal EUE epithelial cell line. EUE cells have proven to be a useful tool for studying long-term in vitro adaptation to hypertonic stress. After culturing EUE cells in hypertonic medium a marked c-fos induction was observed, both at the mRNA and the protein level. Northern analysis of fos-mRNA showed a peak expression at 4 h, followed by a progressive decline till complete extinction at 8 h. Immunofluorescence analysis of FOS protein evidenced a similar, although slightly delayed kinetics of expression. Conversely, neither c-jun nor c-myc up-regulation could be detected. The treatment of EUE cells with cycloheximide led to superinduction of c-fos expression, (with high levels up to 12 h), and to a c-jun expression that was just detectable. Hypertonic stimulation of the transformed cell lines A549, MCF7 and JR induced both c-fos and c-jun only in JR cells. Hypertonic shock was also effective in inducing c-fos expression in fetal human diploid fibroblasts, although the response was earlier and more transient than in EUE cells. These findings indicate that c-fos is a primary response gene in hypertonic stress-activated cells, although the pattern and kinetics of its induction may differ according to the type of cell.

Differential induction of the two early genes c-jun and c-fos in weakly and strongly metastatic murine lymphoma cell lines.

Induction of the 2 early-response genes c-jun and c-fos was investigated in the weakly metastatic T-lymphoma Eb line and the related strongly metastatic lymphomacrophage ESb line to find possible correlations with their different in vitro and in vivo phenotypes. The response of c-jun was elicited by the protein kinase-C activators TPA and A23187 in ESb but not in Eb cells. A much lower response of c-fos was also found in Eb than in ESb cells, in this case by means of serum and the cAMP elevator forskolin. However, both TPA and the calcium ionophore A23187 were similarly effective in inducing fos-mRNA in both cell lines. The uncoupling of c-jun and c-fos induction in Eb, but not in ESb cells, as well as the uncoupling of c-fos response to different stimulators, point to a differential activation of these 2 early-response genes by the main signal transduction pathways in the 2 cell types. The coordinate/uncoordinate availability of the fos/jun heterodimer may confer distinct regulatory patterns on different target genes in ESb/Eb cells. Activation of these genes may underlie the distinct differentiation phenotypes and in vivo behavior of Eb/ESb cells.

Subcutaneous metastases after coelioscopic lymphadenectomy for vesical urothelial carcinoma.

The authors report an exceptional case of subcutaneous metastatic dissemination, located at the puncture points of a coelioscopic lymphadenectomy for a vesical urothelial carcinoma. Few cases of parietal metastasis have been described after coelioscopic check-up for gynaecological and digestive tumours. The onset of this new type of serious complication suggests a need to reassess the benefits of this technique in tumours with lymphatic invasion.

[Stage M1b prostatic adenocarcinoma: prognostic factors, value of bone scintigraphy].

Based on a retrospective study of 52 patients with prostatic adenocarcinoma and bone metastases (stage M1b), the authors analysed the following prognostic factors at the time of diagnosis: age, general status, bone pain, haemoglobin, local tumour volume, ureteric repercussions, pre and post-treatment PAP and PSA levels, Gleason score, and metastatic spread on bone scan. This study demonstrated two predominant prognostic factors for the appearance of early or late therapeutic escape: tumour differentiation established by the Gleason score (P = 0.003), stage of the disease, i.e. local tumour volume (p = 0.001) and bone mass invaded on bone scan (p = 0.0002). The other prognostic factors can be deduced from these two parameters. Qualitative analysis of the initial bone scan allowed patients with peripheral bone metastases to be distinguished from those with exclusively axial involvement. The two-year survival was 50% in patients with peripheral metastases versus 93% in patients without peripheral metastases (p < 0.05). Although bone metastasis constitutes a decisive prognostic factor, the detection of peripheral bone metastases appears to be a factor of poor prognosis.

Management of intra-abdominal organ injury following blunt abdominal trauma in children.

OBJECTIVE: To evaluate the strategy of a combined diagnostic and therapeutic approach in children with intra-abdominal organ injury following blunt abdominal trauma. DESIGN: Retrospective clinical study. SETTING: Pediatric intensive care unit of an university hospital. PATIENTS: 38 children with documented intra-abdominal injury. INTERVENTION: Initial non-surgical treatment by a team of pediatric intensivists, radiologists and surgeons. MEASUREMENTS AND RESULTS: Physical examination, oriented blood and urine tests, plain abdominal film, abdominal ultrasound (US) and computed tomography (CT) with contrast. US documented intra-abdominal fluid in 30 and initial organ lesion in 14 out of 31 patients evaluated. Abdominal CT demonstrated the precise organ lesion in 34 out of 36 patients examined with solid organ lesion. Early laparotomy was needed in 7 because of severe shock, pneumoperitoneum and ruptured diaphragm, and delayed surgery in 6 patients. All 38 patients regained a normal life. CONCLUSIONS: The stepped diagnostic approach combined with initial non-surgical treatment by a team provided accurate diagnosis and appropriate treatment. Abdominal US, by demonstrating free intra-abdominal fluid is very sensitive to detect patients with intra-abdominal organ injury, CT scan with contrast is needed to give precise information of specific organ lesions.

Nine-membered cyclodepsitripeptides containing the retroisomeric sequence of ergot peptides.

In order to examine factors influencing cyclodepsitripeptide formation and stability, three cyclic peptide models containing the alpha-hydroxyacyl-alpha-aminoacylprolyl sequence, corresponding to the peptide lactone cyclo(-Xha-Pro-Xaa-) (Xha = Hiv, Lac; Xaa = Phe, DPhe, DAla), retroisomeric with respect to the ergot oxa-cyclolic peptide moiety, have been considered. Starting from 2,5-dioxomorpholines, aminoacyl incorporation reaction led, through intermediate tetrahedral adducts (aza-cyclols), to the corresponding nine-membered cyclodepsitripeptides cyclo(-Hiv-Pro-DPhe-) 4a, cyclo(-Lac-Pro-DPhe-) 4b, and cyclo (-Hiv-Pro-DAla-) 4c. Compounds 4a-c contain a CONH peptide bond in the nine-membered ring and are stable on standing. Solution conformations of 4a-c, as inferred by NMR spectra and NOE experiments, have been studied. A cis-cis-trans backbone conformation, analogous to that observed in the case of previously studied nine-membered cyclodepsitripeptides containing two proline residues in the ring, is proposed for 4a-c.

A controlled clinical study of the efficacy and tolerability of nimesulide vs naproxen in maxillo-facial surgery.

The anti-inflammatory and analgesic efficacy of nimesulide was assessed and compared with that of naproxen in the postoperative treatment of inflammatory complications of maxillo-facial surgery in a double-blind study. A total of 60 patients were randomly assigned to treatment with nimesulide (100mg twice daily) or naproxen (250mg twice daily) for 6 to 14 days. Treatment with either drug prevented the development of, or ameliorated the signs and symptoms associated with, the inflammatory process (spontaneous pain, difficulty in chewing and swallowing, swelling, hyperaemia, muscle contracture, poor sleep quality). Indeed, most patients experienced complete resolution of their signs and symptoms. The efficacy of nimesulide was considered to be superior to that of naproxen, although both drugs were tolerated equally well.

[The inflatable penile prosthesis. Experiences apropos of 58 implants].

Prosthetic surgery for impotence has been transformed by the development of inflatable prostheses in the place of older semirigid models. Despite the popularity of this new technique, it raises certain questions concerning the functional results and complications of this surgery, which have only occasionally been studied concomitantly in the literature. The sexuality of patients has generally been evaluated on the basis of answers to a questionnaire sent to the patients. This study reports our experience, compared to the data in the literature, based on 58 insertions of inflatable prosthesis between October 1987 and October 1991. After rigorous patient selection and a mean follow-up of 30 months, the surgeon evaluated the objective results (mechanical functioning of the prosthesis, complications) and subjective results (sexuality) in 51 patients. 69.4% of patients presented an anomaly of prosthesis function, 10.3% developed an infection of the prosthesis, 34.5% required removal of the prosthesis, 59% declared that they were satisfied sexually and sexual activity was restored in 55%. This study therefore appears to contradict the optimistic data reported in the literature.

Inhibitors of Zn-dependent metallopeptidases: synthesis and activity of N-(2-furoyl)-(Z)-alpha, beta-didehydroleucyl-L-tryptophan.

In order to gain information on structure activity relationships of peptidic inhibitors of Zn-dependent metalloproteinases "hemorrhagins", the conformationally restricted model N-(2-furoyl)-(Z)-alpha,beta-didehydroleucyl-L-tryptophan 2 was synthesized and its activity compared to that of related previously studied substrates. The new model 2 exhibits an inhibitory activity on proteinase II from Crotalus Adamanteus snake venom, sensibly lower than that of related substrates. This result indicates that the reduction of the conformational space due to the presence of the alpha,beta-didehydro-amino acid residue in 2 does not favour the fitting and binding at the enzyme active site.

[Supernumerary urethras in men. Report of supernumerary blind epispadias].

Supernumerary urethra, a rare anatomical anomaly, can be explained by various embryological theories, accounting for the multitude of classifications proposed. Its diverse, minor or very severe clinical manifestations suggest the diagnosis, which is confirmed by retrograde and voiding cystourethrography. Treatment is surgical or, more rarely, endoscopic.

Activation of c-fos and c-fes in metastatic lympho-macrophage hybrids.

The expression of proto-oncogenes involved in myelomonocytic differentiation was studied in 3 metastatic murine lympho-macrophage hybrids (ESb, EbF1, EbF2-C4) which display several antigenic and functional macrophage properties. Constitutive expressions of fos and fesmRNA, which were not detectable in the weakly metastatic parent lymphoma (Eb), was shown in all the proliferating hybrid lines. By contrast, c-myc and c-myc expressions were shared by the tumor parent and the hybrids. The c-fos transcripts observed in the hybrids were atypical in size, 2.3 kb, but standard 2.2-kb transcripts could be detected in serum-induced cells, even in the non-expressing Eb lymphoma. The kinetics of the induced fos mRNA was found to conform to the features reported in the literature for a large variety of cells. No structural alteration of the c-fos gene was detected by Southern analysis. PCR amplification of the 2.3- and 2.2-kb fos mRNA showed that they are identical in size in both the coding and the 3' untranscribed region. A longer poly A tail is thought to account for the additional 100 bp in the 2.3-kb fos mRNA, as both the 2.3- and 2.2-kb transcripts attain the same size following de-adenylation.

Chlamydia trachomatis in neonatal respiratory distress of very preterm babies: biphasic clinical picture.

We observed 12 very preterm infants (10 males) with a peculiar respiratory syndrome characterized by early onset soon after birth and by a biphasic course. The severe first phase was characterized by a clinical pattern mimicking the idiopathic respiratory distress syndrome of prematurity. Gradually, respiratory symptoms decreased and assisted ventilation with oxygen therapy was reduced. In the second phase, a significant worsening of respiratory signs and the appearance of apneic spells were observed. Chest X-ray showed hypoexpansion of the lungs and the prevalence of a fine reticular pattern. Chlamydia trachomatis was identified in this second phase in conjunctival and pharyngeal swabs and/or on tracheal aspirates. Our data suggest that in the very preterm infants, chlamydial infection shows different clinical and laboratory features if compared with Chlamydia trachomatis pneumonia of infants born at term.

Performance of crosses among Hereford, Angus, and Simmental cattle with different levels of Simmental breeding: VI. Maternal heterosis of 3- to 8-year-old dams and the dominance model.

Data from Hereford, 25% Simmental-75% Hereford, 50% Simmental-50% Hereford, and 75% Simmental-25% Hereford dams were used to estimate maternal heterosis and level of agreement with the dominance model. Cows were located at the Northern Agricultural Research Center near Havre, MT and were managed consistent with practices for western range environments. Sample halves of dam breed groups were bred to Charolais and Tarentaise sires to produce calves at 3 to 8 yr of age. There were 766 exposures to breeding that resulted in 581 calves. Breed group means for most traits supported the dominance model. Maternal heterosis was estimated by regression techniques for 22 cow and calf traits. Maternal heterosis was not significant for day of conception, number of services, gestation length, or calving difficulty. Estimates of maternal heterosis for calf growth traits ranged from .7% for weaning height to 5.2% for weaning weight and 7.5% for weaning condition score. Calf weight per unit of cow weight at weaning showed significant maternal heterosis (7.1%). Higher levels of maternal heterosis were exhibited for milk production (8.2 to 11.1%) and the negative, but nonsignificant, estimate of maternal heterosis for early minus late milk production suggested more persistent lactation for crossbred cows. Maternal heterosis was 11.5% for proportion of dams that calved and 10.4% for proportion of dams that weaned calves. Calf weaning weight per cow exposed to breeding, a characteristic combining calf growth and dam reproduction, exhibited 17.9% maternal heterosis.

[Risk stratification in patients with a first myocardial infarct based on simple clinico-instrumental variables].

UNLABELLED: It is an acknowledged fact that the prognosis for patients with a first myocardial infarction depends mainly on the degree of residual left ventricle function. We wanted to evaluate the importance that certain simple clinical and instrumental variables can have in stratifying post-infarction cardiovascular risk with particular emphasis on chronic obstructive lung disease (COLD). We selected 97 out of the 512 patients treated in the coronary intensive care unit (CICU) from February 1, 1988 to October 31, 1990 according to the following criteria: First myocardial infarction; no cardiogenic shock; no serious concomitant diseases with considered negative prognosis within 6 months. The following variables were considered for all the patients: age; sex; positive family history for ischemic heart disease; history of diabetes mellitus; arterial hypertension; previous cerebrovascular incident; history of obstructive arteriopathy of the lower limbs, of angor and COLD. The following tests were performed on all the patients: echocardiogram prior to discharge form the CICU; angiocardioscintigraphy with Tc-99 between the 20th and 30th day following the acute event; bicycle ergometer stress test on the 30th day. END POINTS: general mortality; cardiac mortality; non-fatal reinfarction; residual angina at 3 months. All the patients were treated with aspirin (325 mg/die) and/or heparin (12,500 units subcutaneously). All 97 patients were monitored for a mean follow-up time of 19.8 months. General mortality was 2.08% (for reinfarction) 24 (24.7%) non-fatal cardiac events.(ABSTRACT TRUNCATED AT 250 WORDS)

[Contribution of the ambulatory urodynamic monitoring to the study of micturition disorders].

An outpatient urodynamic study allows simultaneous recording of bladder, urethral and abdominal pressures over a long duration with physiological bladder filling and under outpatient conditions. One hundred and eighteen outpatient urodynamic recordings were performed in patients presenting with disturbances of micturition such as urinary incontinence or dysuria. In the group of incontinent patients, the outpatient urodynamic recording was superior to the standard urodynamic assessment for the diagnosis of unstable bladder (p < 0.001). In the group of dysuric patients, the outpatient urodynamic recording allowed a precise evaluation of the detrusor activity and a simplified assessment of micturition. Its use combined with a urecholine test allows analysis of the onset, duration and intensity of action of the drug in comparison with a standard test.

Epididymal sperm aspiration in conjunction with in-vitro fertilization and embryo transfer in cases of obstructive azoospermia.

Epididymal sperm aspiration is a new treatment for vasal agenesis. In previous reports, epididymal spermatozoa resulted in pregnancy by utilizing in-vitro fertilization (IVF) or gamete intra-Fallopian transfer (GIFT). We sought to investigate the efficacy of epididymal sperm aspiration in conjunction with IVF in patients with congenital absence of the vas deferens or with secondary extended obstruction of spermatic ducts. Fifty-eight attempts were performed in 23 patients (25-50 years). Eight patients (34.7%) had vasal agenesis and 14 (60.8%) presented with vasal secondary extended obstruction. The sperm count was adequate (greater than or equal 20 x 10(6)/ml) in 13.8% of sperm retrievals and sperm motility of 20% was obtained in 15.5% of sperm retrievals. Fourteen attempts at IVF were performed with epididymal sperm counts of 2-44 x 10(6)/ml and motilities of 0-45%. A mean of six mature oocytes (0-13) were inseminated in each case. Five embryo transfers were performed in five patients' wives (35.7%) and two couples had an early pregnancy loss (14.2%). Epididymal sperm aspiration is an advance in treating such patients, as an adequate number of mature spermatozoa can be obtained and used for IVF. However, spermatozoa directly aspirated from the proximal epididymis and with fertilizing capacity in vitro, gave a high rate of embryo degeneration (greater than 50%) after embryo transfer.
