Immunocytochemical localization of plasminogen activators in carcinomas in the cervix and vulva.

The presence of plasminogen activators (PA) in a variety of solid tumours appears to correlate, in a number of instances, with enhanced invasive and/or metastatic ability. Urokinase and tissue-type plasminogen activators (u-PA and t-PA) in normal and neoplastic tissues of cervix and of vulva were immunohistochemically identified by means of polycyclonal antibodies. In addition, frozen sections were analysed for u-PA and t-PA activity by in-situ zymography technique. Data collected in our study showed that in invasive cancer u-PA increased more in malignant cells as compared to normal cells in both the inactive and active enzymatic forms. The t-PA distribution pattern was related to angiogenesis while it did not relate to the degree of tumor differentiation. A synergic interaction between proteolytic tumoral activity and proteolytic inflammatory action could be hypothesized.

The vitelline coat spikes: a new peculiar structure of Mytilus galloprovincialis eggs with a role in sperm-egg interaction.

In the course of this study we found that in Mytilus galloprovincialis eggs long filamentous protrusions never described before, which we have termed "vitelline coat spikes," could be clearly detected using the lectin from Dolichos biflorus, which recognizes the GalNAc residues. The spikes could be also observed by transmission electron microscope but only in some fortuitous sections could their origin in the vitelline coat be clearly observed. The spikes were also clearly visible using the scanning electron microscope. Observations of the sperm-egg interaction very few seconds after insemination or using fixed eggs suggested that the spikes could play a role in a primary binding to the unreacted sperm. Experiments have been done to test the effect of GalNAc on the sperm-egg binding and on the fertilization process which seem to confirm this hypothesis.

Basement membrane components in normal, dysplastic, neoplastic laryngeal tissue and metastatic lymph nodes.

The immunohistochemical localization of the basement membrane (BM) components laminin, type IV collagen and fibronectin was analyzed in normal, dysplastic and neoplastic laryngeal specimens. The distribution of these macromolecules was also investigated in metastatic lymph nodes. A regular and continuous staining for laminin and type IV collagen was present in normal and mild dysplastic epithelium (LIN I); interruptions and reduplications were more evident in severe dysplasia (LIN III), together with an increased positivity for fibronectin in the subepithelial connective tissue. In squamous cell carcinomas the distribution of BM components was related to the degree of cellular differentiation, with a decreased immunostaining being evident in moderately and poorly differentiated carcinomas. Furthermore, the positivity for laminin and type IV collagen was influenced by the pattern of neoplastic growth, being continuous around the "pushing" border and discontinuous where the neoplastic front had an "invading" appearance. Similar changes were present in cervical metastatic lymph nodes. These observations tend to support the theory that the neoplastic growth is a cyclic process, with BM component synthesis and reabsorbtion related to the shifts of cellular metabolism.

Differentiation of the vitelline coat and the polarized site of sperm entrance in the egg of Unio elongatulus (Mollusca, Bivalvia).

We studied the differentiation of the polarized site of sperm entrance in the egg of a freshwater bivalve, Unio elongatulus. As previously shown, in this egg model the property of sperm recognition and binding is restricted to a region characterized by a wrinkled surface which surrounds a truncated cone or crater region, at the vegetal pole of the egg. The crater is formed during oogenesis at a site opposite the oocyte attachment to the ovarian wall. It first appears as a small bleb, and later detaches, leaving in the oocyte an open narrow process filled with an orderly array of microtubules. Here the vitelline coat differentiates from the rest of the oocyte. The role of the microtubules in the formation of the region and the differential distribution of the vitelline coat components is discussed. The synthesis during development of fucosyl containing glycoproteins and their presence in an electrophoretic pattern of isolated vitelline coats were also studied using Lotus tetragonolobus (LTA) conjugated with fluorescein and peroxidase.

Treatment of nonunion in fractures of the carpal navicular by the Matti-Russe operation.

The factors favouring delayed and nonunion in fractures of the carpal navicular are discussed. These may be anatomical, biological, mechanical, or related to diagnosis and treatment. Twenty cases are presented which were treated between 1980 and 1985 by the Matti-Russe operation. Unlike Matti-Russe, the authors remove the transplant from the distal metaphysis of the homolateral radius, which is easily exposed by prolonging the skin incision by a few centimetres. This reduces the operating time. The authors conclude by emphasizing the reliability and simplicity of this method, which resulted in consolidation in 87% of the cases.

Source assessment of hexachlorobenzene from the organic chemical manufacturing industry.

The Office of Solid Waste of the US Environmental Protection Agency determines the hazards of wastes produced by the organic chemical manufacturing industry. Based on these determinations, regulations have been laid down for many wastes under the Resource Conservation and Recovery Act. Additional wastes from this industry will be considered for regulation following further study and evaluation. Among the wastes under study are hexachlorobenzene (HCB)-containing wastes. Although HCB is no longer manufactured in the USA, the US Environmental Protection Agency has estimated that approximately 4130 t are generated annually as a by-product in the production of chlorinated organics and pesticides. Of this total, about 77% (3178 t) is generated from the production of three chlorinated solvents: tetrachloroethylene, carbon tetrachloride and trichloroethylene. All wastewater containing HCB is treated biologically in surface impoundments, resulting in HCB accumulation in the biological sludge. Disposal methods for HCB-containing sludges and distillation bottoms are incineration and landfilling, with 81% (3345 t) being incinerated and 19% (785 t) going to landfills.

[Cancer of the right colon: biological features and extent of the surgical resection].

Right-sided colon cancer therapy is based upon right hemicolectomy, which is a well established surgical technique. Considering the possibility of biological diversity in cancer arising from the caecum, ascending colon and transverse colon, this retrospective study analysed age, sex, grading, T, N, M and mortality to evaluate the biological characteristics of the cancers and their level of local and general diffusion with a view to establishing any correlation between these characteristics and the advisability of employing different surgical procedures for the different sites. Cancer arising from the sites considered does not seem to present different biological aspects and therefore a different surgical procedure is not authorized. Our data show that a significant majority of cancers are diagnosed at an advanced stage as compared to earlier stages (P < 0.05) and that early detection of these cancers is the best way of improving the prognosis.

Locked posterior dislocation of the shoulder.

What the authors mean by locked dislocation of the shoulder is an inveterate posterior dislocation of the humeral head which remains locked within the glenoid cavity as a result of anatomopathological lesion. Based on an analysis of four cases a diagnostic-therapeutic protocol is proposed in which the clinical and radiographic data obtained allows for a reliable diagnosis and suitable treatment.

Long-term progression of a case of trans-epiphyseal fracture-dislocation of the proximal epiphysis of the femur.

Epiphyseal detachment-dislocation of the proximal femur constitutes a traumatic lesion, the immediate resolution of which always requires surgical treatment. How early treatment is carried out is important in improving the prognosis of the lesions which, however, in most cases is poor. In our case, clinical examination made 6 years post-surgery shows satisfactory preservation of range of movement with no important subjective disorders and with a minimal amount of hypometria (1 cm) even if structural epiphyseal changes visualized on radiographic examination and CT scan predict progression towards early arthritic degeneration.

[Immunohistochemical localization of laminin, collagen IV and fibronectin in dysplasias and neoplasms of the cervix uteri].

The distribution of intrinsic components (Laminin and Type IV Collagen) and extrinsic component (Fibronectin) of the Basement Membrane (BM) has been studied in normal uterine cervix (16 cases), in cervical dysplasia (14 cases) and in invasive carcinoma (45 cases). We found that BM staining for Laminin and Type IV collagen is linear and continuous underlying normal and dysplastic epithelium (CIN 1-CIN 2), it shows minor breaks in continuity and alterations of linearity in situ carcinomas (CIN 3), and it disappears in microinvasive areas. In well differentiated invasive carcinomas only focal disruptions of BM around neoplastic nests is noted; in contrast, in moderately and poorly differentiated neoplasias, is found a progressive reduction and loss of staining for Laminin and Type IV Collagen. The results of this study suggest that the distribution patterns of BM intrinsic components are in relation to the histological grade of cervical neoplasias and their invasion ability. On the contrary Fibronectin doesn't show any distribution pattern related to the grade of cervical neoplasias, its immunostaining increased with the rise of both connective tissue stroma production and of vascularity.

[Distribution of laminin, type IV collagen and fibronectin in normal, dysplastic and neoplastic laryngeal tissue].

The main components, both intrinsic (laminin and type IV collagen) and extrinsic (fibronectin), of the basement membrane (BM) were analyzed by immunohistochemical methods in normal, dysplastic and neoplastic laryngeal mucosa specimens. The material was obtained from 35 patients who had undergone surgery for glottic or supraglottic cancer. Fibronectin proved to be absent from normal mucosa whereas an immunopositivity was observed close to the dysplastic epithelium, especially around inflammatory cells. Positivity increased as the degree of dysplasia increased from LIN I to LIN III. A strong staining was also seen around nests of well and moderately differentiated squamous cell carcinoma. These findings are in agreement with the theories about the main sites of origin for fibronectin, both from plasma and connective tissue. Laminin and type IV collagen showed the same staining characteristics. In normal and mild dysplastic samples a regular and continuous positivity was found at the boundaries between the epithelium and the mesenchymal stroma. Focal discontinuities were present in areas of intense subepithelial inflammation only. Interruptions and reduplications were more evident in severely dysplastic epithelium. In invasive squamous cell carcinomas a strong correlation has been found between the degree of cell differentiation and the pattern of distribution of the intrinsic BM components. Immunostaining was usually evident and continuous around nests of well differentiated squamous cell carcinoma, whereas positivity progressively decreased in moderately and poorly differentiated neoplasms. Furthermore, staining for intrinsic BM components was also related to the pattern of tumor growth: continuous around the "pushing" edge of neoplastic growth and discontinuous when the neoplastic front had an "invading" appearance. These observations tend to support the theory which considers neoplastic growth a cyclic process. BM components are most likely synthesized during the phases of quiescence and reabsorbed during the phase of invasiveness, following shifts in neoplastic cell metabolism.

[A modified Putti-Platt surgical method in the treatment of habitual luxation of the anterior shoulder: our experience].

Fifty cases of recurrent dislocation of the shoulder operated according to the Putti-Platt method modified by lateralization of the insertion of the coracoid processes are reported. Excellent results were obtained in 86% of the cases. There was neither incidence of recurrence, nor degenerative joint alteration. Mild limitation in extrarotation (less than 10 degrees) was observed in 7 cases.

[Instrumental diagnosis in shoulder instability].

The authors call attention to the pathology caused by glenohumeral instability and, in particular, to painful shoulders in athletes which so often cause problems in diagnosis. An instrumental protocol for diagnosis is suggested, based on several specific radiographic views, Ct scan and arthro-Ct scan, with double contrast medium, the latter having the task of determining lesion which would not otherwise be able to be studied.

[A cost-benefit evaluation of prenatal diagnosis performed at a referral center with a population with a genetic or abnormality risk].

Prenatal diagnosis (PND) has assistencial, psychological and economical consequences; it is very important to evaluate cost effectiveness for the population. A prospective study was done (longitudinal and descriptive) on a population referred to Maternidade Dr. Alfredo da Costa, divided into two groups. Group one with PND performed on proper time and second group a PND procedure performed on a adequate time. The general characterization, indications for reference and consequences were evaluated. The conclusions are that the indications for reference were correct and the diagnosis was effective in 17% of patients. Social, economical levels and multiparity were the most important factors affecting adequate PND. However concerning the results, the termination of pregnancy was more performed on the group 1 and more abnormalities were seen on the newborns of group 2.
