[Preoperative localization of insulinoma is necessary].

Many insulinomas because of their small size are not localized by routine investigations: abdominal ultrasound, angiography, computed tomography, nor at the step of laparotomy. Biological diagnosis is far more encouraging. Percutaneous transhepatic sampling of blood in the portal venous system is very predictable for adenoma localization. Absolute reliability of this method remains to be established, nevertheless the observance of some basic requirements are already known: withdrawal of any drug interfering with insulin release several days before performing the catheterism; need of a steady, preferably low, glucose level during the whole sampling; selective samples in tiny draining veins of the whole gland, and adequate radio-immunoassay. Transhepatic sampling can detect multiple localisations and diffuse hyperplasia but angiography alone is able to show hepatic metastasis and is helpful in giving the surgeon information on local vascularization. Confrontation of both angiography and transhepatic sampling gives the best criteria of localization. In case of discrepancy, transhepatic sampling seems to be more reliable.

Serotoninergic dysfunction in the 47, XYY syndrome.

In six XYY patients suffering from aggressiveness and admitted by order of law into a security setting for acts of violence, the estimation of cerebrospinal fluid amine metabolites before and after a probenecid test revealed a clear decrease in the turnover of central serotonin (5-HT) while that of dopamine (DA) was unchanged. The treatment with the 5-HT precursor, L-5-hydroxytryptophan (L-5-HTP), in five of the XYYs resulted in a clinical status equivalent to that observed when the patients were previously treated by conventional neuroleptics.

Biogenic amines in 47, XYY syndrome.

47, XYYs represent a high percentage of patients admitted in security settings for aggressiveness. By using a polygraphic technique and amine metabolite estimation in the cerebrospinal fluid (CSF), an attempt was made to evaluate the functional activity of the central aminergic system of these patients. No drastic change was observed in sleep patterns of XYYs. The estimation of CSF amine metabolites revealed a normal value for homovanillic acid, but a significant decrease of 5-hydroxyindoleacetic acid turnover.

Intestinal spirochetosis in children: report of two cases.

Two cases of intestinal spirochetosis in two children are reported. The first patient, a 7 1/2-year-old boy, presented with diarrhea and rectal bleeding. After the diagnosis was made by a rectal biopsy, he was given metronidazole and neomycin. Symptoms persisted despite a further rectal biopsy that showed clearance of the organisms. Appendiceal spirochetosis was an incidental finding in our second patient, an 8-year-old girl who presented with acute abdominal pain and in whom mesenteric adenitis was diagnosed clinically at laparotomy. The possible pathogenic mechanisms causing clinical symptoms are discussed.
