[Results of a systematic study of drepanocytosis in 1,500 Gabonese using hemoglobin electrophoresis].

Hemoglobin electrophoresis realised as a routine screening test on 1 500 gabonese individuals revealed the existence of a sickle cell abnormality n 24.40% cases (22.27% were AS heterozygotous and 2.1% were SS homozygotous). Bapounov and Fang ethnies appeared to be the most involved. The importance of such disturbance (I gabonese individual out of 4 is concerned) could justify the creation of Prenatal Counseling Centers mainly dealing with this genetic problem.

[Digestive cryptosporidiosis in young children in an urban area in Gabon].

A 27 month epidemiological survey of cryptosporidiosis in stools was conducted in 288 Gabonese children aged between 0 and 2 years (mean 10.2 months). By at least one of two staining techniques, acid-fast and direct immunofluorescence, the rate of infection was determined to be 24%. The rate was 28% in cases of acute diarrhea, in striking contrast with the low prevalence of 14.8% in uninjured gastroenteritus subjects (p < 0.05). The notion of asymptomatic carriers was clearly demonstrated. The maximum of 34.4% was observed for infants aged between 6 and 12 months (p < 0.02). In cases of malnutrition, this rate increased to 31.8%, whereas it was 16.8% in eutrophic children (p < 0.01). For the rainy and dry seasons, it was 31.7% and 19.4%, respectively (p < 0.02). Water is emerging as an important vehicle for the transmission and spread of Cryptosporidium sp. in tropical areas.

[Chromoblastomycosis in Gabon. Study of 64 cases].

During the last 25 years, 64 cases of chromoblastomycosis were diagnosed in Gabon. They came from the entire territory, where living in rural zones men as well as women were affected. The evolution of the disease has lasted for more than 10 years for the majority of the cases. The preferential location is the lower limb for 78.5% of the patients, with a double localization for one case and an exceptional malignant transformation for another case. Two clinical aspects emerged, despite an apparent polymorphism: extensive patches and warty nodules, spread apart from or adjacent to the others. The histological images were classical and the fumagoid cells, sometimes budding and filamentous, were easily detected during examination of the squama and scabs. Fonsecaea pedrosoï was the only identified species. The 5-fluorocytosine treatment constantly led to an improvement of the lesions. The major problem for these destitute patients is economic, concerning the cost of the therapy.

[Lipid profile during specific malaria therapy in Gabonese children].

We report specific dyslipidemia in Gabonese children aged 18 months to 4 years old treated with Halfan for malaria. This is observed in addition to the hypoglycemia typically associated with malaria. C-HDL (high density lipoprotein) fell on day 0 (D0), then increased during the treatment. Triglycerides (TG), total cholesterol (TC) and non-esterified fatty acids (NEFA) rose on D0. CT continued to rise evenly throughout the treatment, whereas TG declined. The differences with respect to normal values are significantly different as assessed by the Student test. We report metabolic variation and tolerance to Halfan.

[Serologic study of 51 cases of human trichinosis].

During the course of a recent epidemic of trichinosis two immunodiagnostic techniques were tested. Firstly indirect immunofluorescence (IF) using sections of the diaphram of infected mice and isolated trichine larvae. Secondly gel precipitation reactions, immunodiffusion and immunoelectrophoresis. It was observed that the induced specific antibodies appeared later than the clinical symptoms and the hypereosinophilia; the delay in appearance of the first positives detected being at least two to three weeks. Subsequently the titres rose rapidly and raised levels persisted for several months. Indirect immunofluorescence of excysted larvae remains the technique of choice, being rapid, sensitive and quantitative. The association of fluorescent and precipitation technique reduces the errors caused by cross reactions with other helminths.

[Tests for evaluating the effectiveness of pediculicides: importance and limitations].

The obligation to prove pediculicides efficacy is relatively recent. Two tests are required by the authorities to obtain registration. In vitro test (with Pediculus humanus humanus) is the first step to evaluate the efficacy of new molecules. It must be followed by bio-clinical tests (with infested children by Pediculus humanus capitis). For those tests the authors advise to respect the "three units rule": unit of time (no more than 2 days for the test), unit of place (same environment) and unit of action (only one team to apply and evaluate products). This type of tests is possible only in countries with high infestation (at least 60% of prevalence). A third test is consequently advised, the ex vivo test using Pediculus humanus capitis obtained from infested children in France. This test can be useful also to re-evaluate current products in the light of increased resistance or decreased sensitivity of the lice. Finally some other tests can be advised to prove some other activities like remanence or repellent properties or to estimated lice resistance.

[Onchodermatitis (sowda) in patients in Yémen. Clinical and histologic course after treatment with ivermectin].

Sowda skin lesions are of medical and social importance in Yemen. Ivermectin (Mectizan) chose as a control strategy plan in onchocerciasis is active during 3 months for the less on clinical and histological data. After a short increase of itching and oedematous skin aspects clinical signs decrease. Some patients notice an itching rebound after 90 days. Histologically, localized ingratiates, presence of mononuclear cells and melenin loaded histiocytes and eosinophils decreased. The rythm cure has to be studied on a longer period but 3 to 6 months repetition between oral treatment with 200 micrograms/kg dose during two years could be effective.

[Immuno-electro-diffusion on cellulose acetate membrane for serological testing in human malaria (author's transl)].

An immuno-electro-diffusion assay for human malaria, using cellulose acetate gel, is described. More than 3500 sera have been tested by this method, which proved very useful for epidemiology since 400 sera a day can be tested by one person, with simple equipment. Its sensitivity compared well to the IFA test, although the antibodies revealed are quite different. A constnat precipitin line appeared, against 56 degrees C labile antigens, in sera of subjects infected by P. falciparum, and up to 8 other precipitins per serum tested. The test may be useful in analyzing these multiple antibodies occurring in patients with malaria.
