Small artery reconstruction with a new vascular prosthesis.

In the Surgical Department of the University of Erlangen, 307 reconstructions in all areas of the arterial tree were performed with PTFE (GORE-TEX) grafts, of which 188 were performed in the femoral-popliteal and femoral-crural areas. Femoral-popliteal bypass grafts above the knee joint remained patent in 93% of the patients during an average follow-up time of 6.7 months. Below the knee joint, the patency rate was 85% during an average follow-up time of 5.6 months. In the proximal aorta, aorta-renal, and aorta-femoral areas, long-term results with the PTFE graft have been excellent.

Indications, technique, and results in 336 tibioperoneal reconstructions.

At the University of Erlangen-Nürnberg, a retrospective study was done of 336 femorotibial bypasses for limb salvage, with a follow-up of 7 years. The indication for arterial reconstruction was severe ischemia in 80.7%. Early good results were obtained in 89.6%, and the mortality rate was 1.5%. The best bypass grafts were the autologous vein and the composite graft, with patency rates of 92%. Five and 7 years after surgery, the cumulative patency rates were 50.7% and 49.1% respectively.

[Rare vascular complications after resection of goiters].

Two patients developed a neurological deficit after an iatrogenic lesion of the common carotid artery in thyroid gland resection. In one patient the artery could be successfully reconstructed by graft interposition. In every patient who develops a neurological deficit after thyroid gland resection a lesion of the carotid artery should be reminded.

[Pelvic venous thrombosis in childhood].

Iliofemoral thrombophlebitis is a rare disease in childhood, although it is probably more frequent than diagnosed. By means of one of our own cases the course and treatment are described. The outlook of this disease is at least as serious in childhood as in adults. The etiology and general prognosis are discussed and compared to the available literature.

[Surgical therapy of arterial occlusions. Indication and results of revascularization in 5370 peripheral arteriosclerotic occlusions].

The results of 5370 revascularizations in peripheral arteriosclerotic occlusive diseases are reported. The operative mortality in 532 carotis interna desobliterations with ischaemia stage II was 2.3%. In stage I of 98 prophylactic operations we lost no patient. Instead of the transthoracic reconstruction we now prefer the extrathoracic bypass because of the lower risk involved. The abdominal aortic aneurysm always is an indication for resection because of the danger of rupture. In cases of 100 operated patients with aneurysm we had a mortality rate in the elective stage of 5.3%, in stage of covered rupture of 47.1% and in ruptured aneurysm 85.7%. The complications in vascular reconstruction of the lower extremity are low. The mortality rate after implantation of aortic bifemoral prosthesis is 3.7%, after an unilateral aortic-femoral prosthesis 2.8% and after all operations below the inguinal ligament 1.0%. Also the femoro-tibial occlusions could be revascularized in bypass-technique with a risk of 1.5% and good early results of 90.3%.

[Survey after correction of carotid stenosis (author's transl)].

Since 1962 until 1976 we have performed 335 operations on the carotid artery in 268 patients. In 1977 we were able to obtain information from 265 patients. The early mortality in stage I, II and IV was one to three percent, in stage III 40 percent. The cumulative survival rate after four years was 74 percent. The main causes of death were cardiac diseases. Often the patients also had heart failure, peripheral arterial diseases, hypertension, and diabetes. Following the operation at least one third of the patients had an improvement of their symptoms.

[Alloplastic vascular substitution with a novel prosthesis. First clinical experiences (author's transl)].

216 reconstructions with Gore-Tex-PTFE prostheses in all vascular regions are reported, particularly in the femoro-popliteal and femoro-crural regions. The best early and early longterm results are in the supra-aortal, aorto-renal and aorto-femal areas. Femoro-popliteal bypasses above the knee joint remain functional in 97 percent at an average followup observation time of 3.8 months. Below the knee, the patency rate is 84 percent at an average followup observation time of 4 months (femoro-popliteal/crural).

[Amaurosis fugax in obliterations of the carotid artery].

Lesions at the carotid bifurcation frequently cause transient cerebral ischemia and especially visual disturbances by microembolization. An early diagnosis is vital in order to prevent definitive blindness. Transient monocular blindness is an early diagnostic sign of ulcerated atheroma of the extracranial carotid artery. Symptomatology, diagnosis and therapeutic aspects are discussed.
