Phenotypic characterization of Lith genes that determine susceptibility to cholesterol cholelithiasis in inbred mice: integrated activities of hepatic lipid regulatory enzymes.

There is no consensus whether hepatic lipid regulatory enzymes play primary or secondary roles in cholesterol cholelithiasis. We have used inbred mice with Lith genes that determine cholesterol gallstone susceptibility to evaluate the question. We studied activities of regulatory enzymes in cholesterol biosynthesis (HMG-CoA reductase), cholesterol esterification (acyl-CoA:cholesterol acyltransferase) and the "neutral" (cholesterol 7alpha-hydroxylase) and "acidic" (sterol 27-hydroxylase) pathways of bile salt synthesis in strains C57L/J and SWR/J as well as recombinant inbred (AKXL-29) mice, all of which have susceptible Lith alleles, and compared them to AKR/J mice with resistant Lith alleles. We determined hepatic enzyme activities of male mice before and at frequent intervals during feeding a lithogenic diet (15% dairy fat, 1% cholesterol, 0.5% cholic acid) for 12 weeks. Basal activities on chow show significant genetic variations for HMG-CoA reductase, sterol 27-hydroxylase, and acyl-CoA: cholesterol acyltranferase, but not for cholesterol 7alpha-hydroxylase. In response to the lithogenic diet, activities of the regulatory enzymes in the two bile salt synthetic pathways are coordinately down-regulated and correlate inversely with prevalence rates of cholesterol crystals and gallstones. Compared with gallstone-resistant mice, significantly higher HMG-CoA reductase activities together with lower activities of both bile salt synthetic enzymes are hallmarks of the enzymatic phenotype in mice with susceptible Lith alleles. The most parsimonious explanation for the multiple enzymatic alterations is that the primary Lith phenotype induces secondary events to increase availability of cholesterol to supply the sterol to the hepatocyte canalicular membrane for hypersecretion into bile.

[Mitochondrial DNA length variation and heteroplasmy in Chinese sturgeon (Acipenser sinensis)].

The D-loop of mitochondrial DNA from Chinese sturgeon (Acipenser sinensis) was amplified using PCR. The length variations among individuals and within individual (heteroplasmy) were detected. DNA sequence analysis revealed that interindivadual and intraindividual length variation occurred within D-loop near tRNA(pro), and caused by tandam repeat with 82 bp in length. Out of 47 individuals, 27(57.4%) individuals were heteroplasmy whereas 20 individual (42.6%) were homoplasmic. The length variation was also found among homoplasmic individuals. In homoplasmic individual four different size classes (number of tandem repeats) e.g. 2, 3, 4 or 5 repeats were noted. The frequencies of size classes from the highest to the lowest were 3-->2-->4-->5. In heteroplasmy two molecular size classes coexisted in an individual was most common (77.78%). The second type of heteroplasmy was three size classes coexisted (18.52%) and the lowest one was four size classes together in an individual (3.70%). No five different size classes coexisted in an individual was found. Through densitometry of DNA bands the frequencies of mtDNA size classes were determined for the sample of individuals, and the frequency distribution of repeats from the highest to the lowest was similar to that in homoplasmy (3-->2-->4-->5-->1). The diversity within each heteroplasmic individual was carried out and genetic diversity at hierarchical level was analyzed. 65.3% of genetic diversities were from among individuals in population and 34.7% of genetic diversities were from within individual. The mtDNA length variation within individual (heteroplasmy) is another genetic diversity resource for Chinese sturgeon.

Sterol carrier protein 2 participates in hypersecretion of biliary cholesterol during gallstone formation in genetically gallstone-susceptible mice.

In inbred mice, susceptibility to cholesterol gallstone disease is conferred by Lith genes, which in part promote hypersecretion of cholesterol into bile in response to a high-fat/cholesterol/cholic acid (lithogenic) diet. Because cytosolic sterol carrier protein 2 (SCP2) is believed to participate in cellular cholesterol trafficking and is elevated in the liver cytosol of cholesterol gallstone patients, we defined the hepatic expression of SCP2 during cholesterol gallstone formation in gallstone-susceptible C57L and gallstone-resistant AKR mice fed the lithogenic diet. Steady-state cytosolic SCP2 levels in C57L, but not AKR mice increased as a function of time and were correlated positively with biliary cholesterol hypersecretion, cholesterol saturation indices of gall-bladder biles and the appearance of liquid and solid cholesterol crystals leading to gallstone formation. Steady-state mRNA levels increased co-ordinately, consistent with regulation of SCP2 expression at the transcriptional level. Our results suggest that overexpression of SCP2 contributes to biliary cholesterol hypersecretion and the pathogenesis of gallstones in genetically susceptible mice. Because of the different chromosomal localizations of the Lith and Scp2 genes, we postulate that Lith genes control SCP2 expression indirectly.

Phenotypic characterization of Lith genes that determine susceptibility to cholesterol cholelithiasis in inbred mice: physical-chemistry of gallbladder bile.

Lith genes control susceptibility to cholesterol gallstone formation in inbred strains of mice on a lithogenic diet containing high fat, high cholesterol and 0.5% cholic acid. Our study defines the physical-chemical phenotypes of C57L, AKR, and (C57L x AKR) F1 mouse gallbladder biles during 56 days on the lithogenic diet. We found enhanced cholesterol supersaturation, accumulation of mucin gel, and larger gallbladders in all C57L and F1 mice, as well as more frequent gallstone formation in male C57L and F1 mice (80%) compared to females (40%) or AKR mice (15%). In male C57L and F1 mice, mucin gel accumulated at 3 days, followed by cholesterol supersaturation and phase separation of liquid crystals, solid monohydrate crystals, and, in 43% of mice, anhydrous cholesterol crystals; whereas, in females, phase separations were delayed 2 to 9 days, and anhydrous crystals did not form. In AKR mice, cholesterol supersaturation and phase separations were infrequent and delayed, and gender did not influence the phenotype. Taurocholate invariably replaced endogenous bile salts, especially tauro-beta-muricholate, with crystallization sequences matching taurocholate-containing model bile systems. We conclude: i) Lith genes determine biliary cholesterol supersaturation, mucin gel accumulation, gallbladder size, phase-separation, and prevalence of cholesterol gallstones. ii) Identical phenotypes in C57L and F1 mice indicate susceptibility to cholesterol gallstones is genetically dominant, favoring males 2:1. iii) Mucin gel accumulation, crystallization, and stone formation are rare in AKR mice. This definition of the physical chemistry of lithogenesis should aid in further elucidation of the Lith genes and the proteins they encode.

Complete mapping of crystallization pathways during cholesterol precipitation from model bile: influence of physical-chemical variables of pathophysiologic relevance and identification of a stable liquid crystalline state in cold, dilute and hydrophilic bile salt-containing systems.

Using complementary physical-chemical techniques we defined five different crystallization pathways as functions of time (30 days) and increasing lecithin (egg yolk) content in pathophysiologically relevant model biles super-saturated (cholesterol saturation indices, 1.2 - 2.7) by dilution of approximately equal to 29 g/dl bile salt-lecithin-cholesterol micellar solutions. As evidenced by quasi-elastic light-scattering spectroscopy, supersaturation was heralded by the appearance of unilamellar vesicles. With the lowest lecithin contents, arc-like crystals with habit and density (d 1.030 g/mL) consistent with anhydrous cholesterol appeared first and evolved via helical and tubular crystals to form plate-like cholesterol monohydrate crystals (d 1.045 g/mL). With higher lecithin fractions, cholesterol monohydrate crystals appeared earlier than arc and other transitional crystals. With typical physiological lecithin contents, early liquid crystals (d 1.020 g/mL) were followed by cholesterol monohydrate crystals and subsequent appearances of arc and other intermediate crystals. With higher lecithin contents, liquid crystals were followed by cholesterol monohydrate crystals only, and at the highest lecithin mole fractions, liquid crystals appeared that did not generate solid crystals. Added calcium increased solid crystal number in proportion to its concentration (5 - 20 mM) but did not influence appearance times, crystallization pathways, or micellar cholesterol solubilities. Decreases in temperature (37 degrees --> 4 degrees C), total lipid concentration (7.3 --> 2.4 g/dL), and bile salt hydrophobicity (3 alpha, 12 alpha --> 3 alpha, 7 alpha, 12 alpha --> 3 alpha, 7 beta hydroxylated taurine conjugates) progressively shifted all crystallization pathways to lower lecithin contents, retarded crystallization, and decreased micellar cholesterol solubilities. The lecithin content of mother biles decreased markedly during crystallization especially where liquid crystals were a coexisting phase at equilibrium. This systematic study provides a framework for understanding cholesterol crystallization in human and animal biles and for examining factors that influence the kinetics of phase separation.

Characterization of crystallization pathways during cholesterol precipitation from human gallbladder biles: identical pathways to corresponding model biles with three predominating sequences.

In model biles, five crystallization sequences are present as functions of bile salt/lecithin (egg yolk) ratio and their positions on phase diagrams are influenced by bile salt hydrophobicity, temperature, and total lipid concentration (D. Q-H. Wang and M.C. Carey. J. Lipid Res. 1996.37: 606-630). To determine whether the same pathways occur ex vivo during cholesterol precipitation from human gallbladder biles, we examined 22 cholesterol gallstone (CSI = 1.56 +/- 0.26), 4 pigment gallstone (0.69 +/- 0.06), and 4 control biles (0.85 +/- 0.22) by microscopy and lipid analytic techniques for 30 days. Temperature was varied (4-45 degrees C) to move relative compositions into adjacent pathways or supersaturated zones to test whether the same bile could be forced to crystallize in different sequences. Sequences in native bile were identical to those in model systems composed of mixed bile salts-lecithin-cholesterol mixtures, and three corresponding pathways (B, C, D; op. cit.) were observed at 37 degrees C. With increasing lecithin content, we found i) B: plate-like cholesterol monohydrate crystals appeared before arc-shaped (putatively anhydrous cholesterol) crystals which transformed via helices and tubules into plate-like crystals and no liquid crystals formed; ii) C: lamellar liquid crystals, typified by birefringent multilamellar vesicles, were detected before cholesterol monohydrate crystals, and subsequently arc, helical and tubular crystals appeared; and iii) D: precipitation of lamellar liquid crystals was followed by cholesterol monohydrate crystals and no arc crystals were detected. Added EDTA prevented calcium bilirubinate formation, but crystallization sequences in these biles were identical to those without EDTA. We conclude that i) cholesterol crystallization pathways and sequences in human gallbladder biles are identical to model biles matched for appropriate physical-chemical conditions; ii) three of the five sequences observed in model biles were found in native bile; and iii) calcium bilirubinates neither promote biliary cholesterol crystallization nor influence crystal growth.

Lith1, a major gene affecting cholesterol gallstone formation among inbred strains of mice.

The prevalence of cholesterol gallstones differs among inbred strains of mice fed a diet containing 15% (wt/wt) dairy fat, 1% (wt/wt) cholesterol, and 0.5% (wt/wt) cholic acid. Strains C57L, SWR, and A were notable for a high prevalence of cholelithiasis; strains C57BL/6, C3H, and SJL had an intermediate prevalence; and strains SM, AKR, and DBA/2 exhibited no cholelithiasis after consuming the diet for 18 weeks. Genetic analysis of the difference in gallstone prevalence rates between strains AKR and C57L was carried out by using the AKXL recombinant inbred strain set and (AKR x C57L)F1 x AKR backcross mice. Susceptibility to gallstone formation was found to be a dominant trait determined by at least two genes. A major gene, named Lith1, mapped to mouse chromosome 2. When examined after 6 weeks on the lithogenic diet, the activity of hepatic 3-hydroxy-3-methylglutaryl-CoA reductase (EC 1.1.1.88) was downregulated as expected in the gallstone-resistant strains, AKR and SJL, but this enzyme failed to downregulate in C57L and SWR, the gallstone-susceptible strains. This suggests that regulation of the rate-limiting enzyme in cholesterol biosynthesis may be pivotal in determining the occurrence and severity of cholesterol hypersecretion and hence lithogenicity of gallbladder bile. These studies indicate that genetic factors are critical in determining gallstone formation and that the genetic resources of the mouse model may permit these factors to be identified.

An investigation of pedigrees of 110 patients with Graves' disease and the clinical significance of determinations of antithyroid antibodies of their first-degree relatives.

Eight hundred and ten pedigree members of 110 patients with Graves' disease were studied. In 700 first-degree relatives, inquiry of medical history, physical examination (including eyes, thyroid, heart rate, etc), thyroid function tests (serum T3, T4 and TSH levels), determinations of thyroglobulin antibodies (TgAb) and thyroid microsomal antibodies (TmAb) were performed. For male (female) probands, the incidence of Graves' disease in male (female) first-degree relatives were investigated and their serum TgAb and TmAb were analysed. The incidence of these two kinds of autoantibodies in the male (female) first-degree relatives of familial and nonfamilial Graves' disease were analysed. Eighteen persons with positive TgAb and TmAb from 5 pedigrees had been followed up one year after initial determinations. Our results suggest that the positive rates of TgAb and TmAb in the first-degree relatives of Graves' disease were coincident with the incidence of Graves' disease, and the positive results of TgAb and TmAb in the first-degree relatives of Graves' disease may be an indicator of pre-Graves' disease or pre-autoimmune thyroid diseases.

[The effect of honey-frying on anti-oxidation activity of Astragalus mongholicus Bunge].

The anti-free radical effects of water extracts of crude Astragalus mongholicus (CAWE) and honey-fried Astragalus mongholicus (HAWE) have been studied. Both extracts have similar effects in scavenging 0.2 in Xan/Xo system. The effect of CAWE is stronger than that of HAWE in scavenging reactive oxygen species (ROS) produced by PMA and stimulated by PMN and also in scavenging OH engendered by Fentons reaction. This suggests that frying process may decrease the ROS scavenging activities of Astragalus mongholicus.

Myocardial protection of cold crystalloid and warm blood cardioplegia. A comparative study.

Twenty patients undergoing open-heart valvular operations were divided randomly into two groups. Intermittent perfusion of cold crystalloid (St. Thomas Hospital solution) with hypothermic cardiopulmonary bypass (CPB) in the hypothermic group and continuous administration of warm blood cardioplegia with normothermic CPB in the normothermic group were used respectively. The results of warm blood cardioplegia were superior to those of cold crystalloid. 70% of patients treated with the warm technique had spontaneous return of normal sinus rhythm shortly after removal of the aortic cross-clamp, compared with only 10% of the hypothermic group (P < 0.05). The extracorporeal support time from releasing of aortic clamp to the weaning of CPB was significantly shorter in the normothermic group (33.50 +/- 3.78 min vs. 25.00 +/- 4.64 min, P < 0.05). The postoperative ventilation support time was also much shorter than that of the hypothermic group (19.84 +/- 1.11 h vs. 38.98 +/- 16.55 h, P < 0.05). More atrial beating occurred in the normothermic group (80% vs. 20%, P < 0.05) during aortic clamping, and it is showed that continuous warm blood cardioplegia might not efficiently prevent the atrium from damage.

[Multiple organ failure after valve replacement].

Multiple organ failure (MOF) after valve replacement during 1980-1991 showed. Incidences involving 2, 3 and over 3 organs were 17.1%, 5.6%, 4.3% with related mortalities 12.0%, 18.5%, 76.2%. This analysis focused on cases of more than 3 organ failures. More than 3 organ failure, the most dreadful complications, can be divided clinically into three types: acute (type I), deteriorating (type II) and pulmonary infection (type III). In MOF the function of heart and lung is primarily depressed after surgery. So the predisposing factors of MOF are acute pump failure and hypoxemia, and likely the "triggering factor" is infection especia pulmonary infection. The latter re-endangers the organs just recovering from the primary blows. So prevention of pulmonary infection is of same importance as treatment of pump failure and hypoxemia.

Computers against disease.

In order to improve the efficiency of the collection, compilation and dissemination of information on 35 communicable diseases in China's Sichuan Province, a computerized network has been established. In 1987 a microcomputer link was set up between the Provincial Health and Anti-epidemic Centre at Chengdu and the Health Information Centre in Beijing, and in 1990 a microcomputer network was established between Chengdu and the 21 prefectures in the province. Substantial progress in disease control is already apparent as a consequence of these developments.

Delayed changes of chromogranin A immunoreactivity (CgA ir) in human striate cortex during postnatal development.

The changes in chromogranin A expression in the human striate cortex from birth till 67 years were studied by immunohistochemical method in 18 autopsied patients. The first chromogranin A immunoreactivity (CgA ir) was identified at birth in layer IV (especially IVc) mainly as fine nerve terminals. By 6 months, the first perikaryal reactivity was noted in the large pyramidal neurons of layer V. The smaller neurons in layers IV, V and VI showed a progressive increase in CgA ir from 15 months to about 17 years. At approximately 9 years, immunoreactivity began to be noted in supragranular neurons in layers II and III. The final laminar distribution of CgA ir seemed to be attained at about 25 years with relatively little change thereafter. The CgA ir in the striate cortex demonstrates a prolonged period of developmental changes, lasting from birth to about 25 years.

Chigger mites of the genus Leptotrombidium: key to species and their distribution in China.

Chigger mites of the genus Leptotrombidium (Acari: Trombiculidae) transmit scrub typhus, caused by Rickettsia tsutsugamushi (= R.orientalis) in South-East Asia. In China, eighty-two species of Leptotrombidium have been recorded; these are listed with the names of Provinces where they were found. Five species, L.deliense, L.insularae, L.kaohuense, L.rubellum and L.scutellare, have been implicated as Chinese vectors of scrub typhus. A brief key is given to the larvae of all but three of the Leptotrombidium mites known in China.

Serum steroid hormonal profiles by reversed-phase liquid chromatography in patients with 17-hydroxylase deficiency and in an affected family.

We used an optimized isocratic reversed-phase high-performance liquid-chromatographic procedure to separate and measure 12 steroid hormones, and studied the steroid hormone profiles in sera from three patients with 17-hydroxylase deficiency (17-OHD). Two of the patients were sisters, one of whom (II-3), expressing normotension and primary amenorrhea, was diagnosed on the basis of chromatographic data and followed up for seven years. The untreated patients had obvious abnormalities on chromatograms of serum extracts, characterized by markedly increased corticosterone (B) and decreased or undetectable cortisol (F) and cortisone (E). The concentration of 11-deoxycorticosterone was much greater in the patient with classical symptoms than in the normotensive patient. In all three patients, concentrations of aldosterone were within the normal range, but concentrations of progesterone were much lower than in the patients with 21-hydroxylase deficiency. We evaluated the responses to corticotropin and dexamethasone. HPLC evaluation of the serum steroid profiles before and after corticotropin stimulation in the affected family showed that in the parents and one other sibling, concentrations of F before and after stimulation were within the normal ranges. The sums of the ratio of B to F before and the ratio of B to F after corticotropin stimulation (sigma B/F) in the parents and the other sibling were 0.292, 0.496, and 0.614, respectively, all much higher than the normal value (mean +/- SD: 0.164 +/- 0.038). Thus the sigma B/F value may be a hormonal marker of heterozygotes carrying this defect.

Expression of Hanganutziu-Deicher antigen in activated human T lymphocytes.

Hanganutziu-Deicher (HD) antigen is a heterophile antigen that is widely distributed in many animals other than humans and chickens and is highly immunogenic in humans and chickens. In the present study, we demonstrated expression of HD-antigenic glycoproteins in activated T lymphocytes by SDS-PAGE and immunoblotting. Treatment with IL-2 plus PMA induced 29kD glycoprotein antigen detected under reducing condition. It contained sialic acid epitope of HD antigen because of the expression being neuraminidase-sensitive. Treatment with PMA plus A 23187 or PHA treatment and then PHA plus IL-2 treatment also induced two proteins of Mr 50kD and 70kD. These expressions were not detected in all individuals examined. These results indicate that HD antigen is an activated T cell antigen and expressed as an isoantigen as it is expressed in cancerous tissues from some patients.
